- Capped patient panel size
- The risk-adjusted patient panel size for physicians is capped at 2,100 patients and advanced practitioners at 1,800
- 36 clinical hours and four hours for administrative work

- Base salary model:

ITION SUMMARY
The Ambulatory Care clinical pharmacist provides direct patient care within an ambulatory setting,
practicing under approved Collaborative Practice Agreements (CPAs). This role focuses on
comprehensive medication management, chronic disease state management, medication access
optimization, and population health initiatives. The pharmacist collaborates closely with primary care
providers and the interdisciplinary care team to initiate, modify, or discontinue medication therapy in

POSITION QUALIFICATIONS
Education/Licensure/Certification:

Role Overview:

The salary range for this role is $105,000-$115,000. This range represents a good-faith estimate of the compensation we expect to offer for this position at the time of posting. Actual compensation will be based on factors such as relevant experience, skills, qualifications, and internal equity. In exceptional cases, compensation may exceed the stated range.
