Opportunity Details:

  • Responsible for clinical oversight of the outpatient division.
  • Develop and expand outpatient rehabilitation service and resident education and training, including EMG/NCS, joint injection, ultrasound, and chemo denervation procedures.
  • Assist in growing the outpatient Interventional Pain Program by collaborating closely with orthopedic surgery and anesthesiology.
  • Responsible to cover approximately 1 weekend call per month and 2-3 holidays per year.
  • EMG/NCS experience preferred, or ability to become proficient.



Qualifications:

  • Valid license to practice in New York State
  • Board Certification issued by the American Board of Physical Medicine & Rehabilitation
  • Five years of full-time licensed experience as a physician



Wages and Benefits include:


Annual Base Salary: $251,000*, based on 40-hour work week.


The annual total value of compensation package is estimated at $316,100**, which includes the baseline salary, 401(k) contribution and other factors as set forth below:

  • Projected bonus of up to $40,000 (based on previous year’s average), contingent upon meeting quality and productivity targets.

Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City’s efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support.

Position Overview

Under supervision of the Site Medical Director, the Physician Assistant will provide comprehensive, compassionate, and thoughtful care to our patients in the New York City jail system. The Physician Assistant will provide general primary care including conducting histories and physicals, diagnosing and treating acute and chronic illnesses, and evaluating the need for consult services

Responsibilities include:

  • Diagnose and treat acute and chronic illnesses. Evaluate the need for consult services and submit the prioritized consult when indicated.
  • Complete comprehensive histories and physicals on all new admissions including documentation of problem list, diagnosis, orders (e.g. labs, imaging and referrals) and ordering appropriate medications where applicable.
  • Evaluate patients requesting sick call, schedule follow-ups and update medication orders. Update problem lists and reconcile patient orders at all visits.
  • Implement plans for patientcare utilizing protocols approved by the Medical Director and/or treatment plans reflecting the current standard of care.
  • Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate.
  • Review all specialty consults and hospital returns to ensure that the standard of care is met and recommendations of the consultant are implemented.
  • Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed.
  • Generate special needs referrals and documentation as needed (for patients with (disabilities, dietary restrictions, heat sensitivity, or other relevant flags).
  • Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care.
  • Ensure that all progress notes and orders are signed before the end of the shift.
  • Respond to emergencies in a timely and professional manner.
  • Notify the staff MD and Urgicareabout 3 hour runs and EMS activation.
  • Complete special housing rounds when assigned.
  • Be familiar with quality of care and population health indicators. Take appropriate action to meet or exceed standards.
  • Maintain clinical competency by participating in all CME and CHS training and in-service requirements.
  • Maintain your schedule as directed with particular attention to punctuality and timely notification of absences.
  • Adhere to policies and procedures of CHS and be familiar with them by reviewing them as needed.
  • Complete tasks as delegated by a Site Medical Director or another supervising clinical team member.
  • Maintain and provide documentation of all required credentials.
  • Maintain professional attitude and appearance.
  • Adhere to Occupational Health Services requirements.


Minimum Qualifications

  1. Valid license and current registration to practice as a Physician Assistant issued by the New York State Education Department (NYSED); and
  2. Current certification issued by the National Commission on Certification of Physician Assistants (NCCPA); and
  3. One (1) year of full-time, satisfactory clinical experience as a Physician Assistant; and
  4. Possession of a valid Life Support certification (e.g., Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS)) is required in the appropriate practice area through the American Heart Association (AHA) within sixty (60) calendar days of appointment to the title.

This position works under the direction of the Chief of Medicine and exercises wide latitude of independent judgment and initiative in supervision of overall administration and management of the Department of Clinical Education. The position assumes full responsibility and authority in areas delegated. May direct a group of services across multiple jail facilities

Responsibilities include: (Detailed Task)

The Department of Medicine emphasizes various educational initiatives tailored for both clinical and non-clinical personnel, as well as educational programming for clinical students, residents, and fellows.

  • Directing, overseeing, and tracking departmental educational initiatives.
  • Overseeing the implementation of clinical and non-clinical workflows and protocols within the department.
  • Coordinating and directing weekly departmental educational conferences.
  • Supervising monthly in-facility departmental educational conferences and the creation of educational materials.
  • Organizing and effectively disseminating information gathered from morning educational conferences.
  • Leading interdisciplinary and cross-departmental educational initiatives, which may include emergency response, general medicine, complex care, young adult health, substance use, infectious diseases, medical care for patients with severe mental illness, women's health, and correctional health.
  • Directing and overseeing clinical electives and visits for clinical students, residents, and fellows.

GENERAL DUTIES:

The Patient Care Coordinator (PCC)/Supervisor of Nurses (SON diagnosis and treats human responses to actual or potential health problems through such services as data collection,

physical assessment, case-finding, health teaching, health counseling

and provision of care supportive to /or restorative of life and well-being as data collection, physical assessment, health teaching, health counseling, and provision of care supportive to or restorative of life and well-being and executing medical regimens prescribed by a licensed physician, dentist or other health care provider legally authorized under this title and in accordance with the commissioner’s regulation.

Under the guidance of the On-Site Director of Nursing (OS-DON), the Patient Care Coordinator/Supervisor of Nurses is responsible for directing and administering nursing care to patients. He/she coordinates activities through collaboration with physicians, administration, pharmacy, clerical, and correctional staff, etc.

He/she is guided by a thorough knowledge of principles of nursing based on biological, physical, and psychosocial sciences. He/she possesses a working knowledge of principles of leadership and utilizes that knowledge in guiding and directing nursing staff.

He/she assists the OS- DON in ensuring compliance with standards, policies and procedures of NYC Health and Hospital Corporation Correctional Health Services contracted with Prison Health Services, Inc. and the Department of Nursing. He/she will be working with adult male and females between sixteen to eighty and infants from birth to one year of age.

RESPONSIBILITIES:

  • Provides direct patient care as necessary. Using the problem-oriented progress notes, written to match each problem documented on the Problem List and organized using the SOAP format.
  • Coordinates the nursing care provided for patients within a given unit by:
  • Assigning appropriate nursing staff for patient care activities such as admissions, sick call, treatments, medications, punitive segregation rounds, follow-up, etc. Problems in staffing must be communicated to the On-Site Director of Nursing.
  • Utilize therapeutic communication skills and effective interviewing techniques to elicit pertinent historical information and to make appropriate referrals as required. Screen patients with mental health problems in an immediate manner and makes appropriate referral as required.
  • Making rounds to patient care areas for the purposes of assessing, planning, implementing, documenting, and evaluating unit nursing care delivery. Assist OS-DON in staffing needs. Provides guidance and assistance to nursing personnel in the delivery of patient care.
  • Assuring continuity of care on other shifts through written and/or verbal communication. Communicates special care needs of particular patients to the psychiatrist, On-Site Director of Nursing or the On-Island Administrator.
  • Collaborates with the OS-DON in the orientation of new staff on the unit. Evaluates the new nurse on an ongoing basis; writes a formal evaluation at the end of the probation period, and makes recommendations to the OS-DON.
  • Evaluates and provides feedback to staff on an ongoing basis and in an annual formal evaluation conference. Collaborates with the OS-DON

and/or Deputy Director of Nursing. Provides appropriate documentation and records of nursing personnel activities.

  • Identifies staff development needs and plans strategies to meet the needs in collaboration with available nursing resources.
  • Conducts meetings with his/her staff, in collaboration with the On-Site Director of Nurses, to share information, discuss problems and potential solutions, and to promote cooperative professional relationships among the staff.
  • Monitors the quality of patient care through quality assessment - improvements mechanisms.
  • Communicates, documents and collaborates on an ongoing basis, with the On-Site Director of Nursing concerning patient care and unit administration.

COORDINATOR / SUPERVISOR RESPONSIBILITES:

  • Coordinates the intra-disciplinary team providing comprehensive patient care.
  • Familiarizes self with and participates in defining, maintaining and Interpreting standards of nursing practice through the application of nursing policies and procedures according to the Prison Health Services, Inc. Policy and Procedure Manual and specific policies of the Correctional Health Services.
  • Identifies those components of the nursing plan, which may be delegated to allied nursing and other non-professional personnel, and provides teaching, guidance and supervision as necessary.
  • Assists with the unit orientation of nursing personnel through participation in
  • the precepting function.
  • Counsels nursing staff and directs information to the On-Site Director of Nursing. Provides documentation as needed on a timely basis.
  • Supervise all care provided by LPN’s/NAs and assumes other related responsibilities as required.
  • Administer medication in a safe and efficient manner consistent with prescribed orders, mouth checks, and established standards
  • Participates in a system of ongoing monitoring activities, including peer, content, and quality reviews to ensure delivery of quality care. Makes recommendations for change where problems are identified.

The New York City jail system is the second largest in the nation. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD).

Responsibilities:

Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatrist will be matched with the population that most closely meets their clinical expertise, desired work location and schedule and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical.

Responsibilities Include:

  • Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs.
  • Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information.
  • Evaluate patients for the need for a higher level of care, including hospitalization, and refer to a higher level of care as clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care.
  • Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Document all activities in the electronic medical record in a timely, concise and clear manner.
  • Integrate nursing and medical concepts in planning care for medically complicated psychiatric patients.
  • Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication.
  • Counsel patients using advanced psychotherapeutic techniques, including in group settings if needed.
  • Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics.
  • Engage in clinical supervision, as indicated, with the Supervising or Senior psychiatrist
  • Other duties as assigned by the Senior Psychiatrist or Mental Health Manager for the facility, Medical Director or Chief of Service for Psychiatry, or their respective designees.

The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city.

The Clinical Supervisor is responsible for maintaining the clinical integrity of the specific program or housing area to which they are assigned, including ensuring that all clinicians are providing appropriate and timely clinical interventions and documentation. Depending on the treatment area, s/he manages patient admissions and discharges, monitors and engages in treatment, oversees quality assurance, manages on-going training and education for treatment-area staff, and performs other managerial administrative tasks including liaising with DOC. In addition to direct supervisory and management responsibilities, the Clinical Supervisor also provides direct care to a small caseload of complex patients and is responsible for the accompanying medical record documentation. Clinical Supervisors who are qualified for and interested in providing care on the PACE or CAPS units (specialty MO units) for individuals with serious mental illness will report to a Program Director for direct supervision.

The Director of Infectious Disease Services will provide critical Infectious Disease expertise of infection control and supervise population health efforts to prevent morbidity and mortality from communicable illness. Responsibilities include: (Detailed Task)

Clinical Responsibilities:

  • Identify individuals admitted to correctional facilities with infectious disease (ID) needs through the utilization of electronic health records, report management systems, and various tracking tools.
  • Provide direct patient care to individuals experiencing acute or chronic infectious disease conditions, including but not limited to HIV, hepatitis C and B, sexually transmitted infections, tuberculosis, skin and soft tissue infections, wounds, and systemic bacterial processes.

Duties & Responsibilities


Purpose of Position:


Under the general supervision of the Director, Engineering and Support Services, assists in directing programs to

manage building maintenance, grounds, and equipment and procurement or generation of utilities and their

distribution systems. Formulates and evaluates programs and projects and supervises functional programming

activities. Ensures ongoing compliance with NYC Health + Hospitals and relevant health care setting policies, and other appropriate regulatory requirements.


Essential Duties and Responsibilities:


1. Directs, coordinates, and maintains control over project assignments and related activities including in-house

construction, renovation, and engineering activities. Follows standard methods and procedures for the

implementation of maintenance systems and operations, construction contracts, and/or capital projects development.


2. Assists in the direction of trade staff through the development processes and procedures for the

implementation of maintenance operations, construction contracts, and capital projects. Assists the Director in

identifying staffing needs to support service delivery.


3. Prepares reports, develops and prepares engineering management audits, studies and surveys for the Director,

and/or other senior management, detailing proposed new programs, statistical studies, and health care setting

planning.


4. Assists in the collection of data and preparation of reports that outline the scope and estimated cost of projects

proposed, implemented and developed. Organizes staff meetings, reviews capital and expense budget submissions and provides recommendations regarding proposed projects and programs. May represent the Director in budget

meetings.


5. Plans and implements programs to establish performance standards to achieve increased productivity and

accountability of trade staff. Reviews and evaluates existing programs, services, policies and procedures.


6. Provides departmental orientation, training and guidance to staff. Conducts in-service training programs, continually assessing staff training needs; suggests changes to Director to provide more efficient operations.


7. Coordinates efforts and cooperates with departmental leadership on matters of mutual concern and extends

functional guidance and assistance. Meets with health care setting clients and staff to plan, prioritize, coordinate

and/or communicate programs and policies, as needed.


8. May provide consultative services to senior leadership or liaise with Federal, State, and City agencies on matters pertaining to engineering, construction, design projects and/or other engineering support programs and functions.


9. In the Director’s absence, may represent the System in certain legal matters dealing with State, City, and Federal agencies.


10. Performs other related duties, as directed.


Minimum Qualifications


  1. Bachelor’s Degree from an accredited college or university in Engineering, Architecture, Construction Management, Construction Engineering or a related discipline; and two (2) years of responsible-level experience in project, facilities or construction management, capital project planning, development, and design or other related experience in a hospital or health care setting; or
  2. High school diploma or its educational equivalent, approved by a State’s Department of Education or a recognized accrediting organization; and six (6) years of experience, as described in 1 above.

Special Note: Valid license and current registration as a Professional Engineer or Architect issued by the New York State Education Department (NYSED) or a related Master’s Degree can be substituted for one (1) year of experience.
