  • Responsible for oversight of the facility’s infection control/prevention program.
  • Responsible for ensuring that there is an on-call nursing staff designated.
  • Receive facility-related calls and come in for uncovered shifts when all other options have been exhausted (per-diem staff, part-time staff, full-time staff, and the Agency are unavailable).
  • Attend Nursing Leadership calls/meetings monthly, and as needed.
  • Assist with insurance issues as needed or designated by the Vice President of Nursing Operations, or designee.
  • Coordinate and ensure that all admission requirements are completed per organization policy.

Position Overview: Licensed Practical Nurse (LPN)

As an LPN, you will provide direct nursing care to residents while collaborating with RNs, physicians, and the interdisciplinary care team. You’ll play a key role in supporting rehabilitation goals, ensuring comfort, safety, and dignity in a long-term care setting.

Key Responsibilities:
  1. Administer medications and treatments as prescribed
  2. Monitor, document, and report changes in residents’ health status, including vital signs
  3. Assist residents with activities of daily living (ADLs) as needed
  4. Collaborate with rehabilitation team members to support recovery goals
  5. Maintain accurate and timely documentation in compliance with facility policies and regulations
  6. Provide emotional support and education to residents and families
  7. Ensure a clean, safe environment by following infection control protocols
Qualifications:
  1. Current and valid Licensed Practical Nurse (LPN) license
  2. Strong clinical skills, including medication administration and wound care
  3. Excellent communication and interpersonal skills
  4. Ability to work effectively in a team-oriented environment

  6. BLS/CPR certification required

Job Summary

Charge Nurses include:

  3. Observing the patient for changes in their health, mood, and behavior, and subsequently reporting any concerns to an RN Unit Manager or the Physician on duty
  4. Listening to a patient as they tell you about their family
  5. Preparing patients for their care, describing procedures ordered by their physicians, and listening to any concerns or grievances they have
  6. Administering medication to patients as prescribed by their physicians
  7. Providing patients with immunizations
  8. Working with or mentoring CNA’s or NA’s to provide optimal care to our patients

Qualifications/Requirements:

  1. Licensed Practical Nurse in good standing and meets all applicable Federal and State licensure requirements
  2. Basic nursing competency skills
  3. Experience with electronic medical records and documentation
  4. Graduate of accredited school of licensed practical nursing
