Job Description: Provides clinically based concurrent and retrospective review of inpatient medical records to evaluate the utilization
and documentation of acute care services. The goal of concurrent review includes facilitation of appropriate physician documentation of care delivered to accurately reflect patient severity of illness and risk of mortality. Specific reviews are both determined internally and by requirements/requests of external payers or regulatory agencies and play a significant role in reporting quality of care outcomes and in obtaining accurate and compliant reimbursement for acute care services.
Qualifications
Education: Associates or Bachelors Degree in Nursing or International Medical Graduate
Experience: Minimum of five (5) years of clinical experience in an acute care setting; critical care,
medical/surgical or Emergency Department nursing preferred.
Technical Skills: Computer proficiency to include word processing, databases, and spreadsheets. Familiarity
with the operation of basic office equipment.
License/Certification: Current RN license in the state in which they reside/practice if Registered Nurse.
Other Knowledge of age-specific needs and elements of disease processes and related procedures
required. Strong broad-based clinical knowledge and understanding of pathology/physiology
of disease processes. Working knowledge of inpatient admission criteria, Medicare
reimbursement system and coding systems preferred, but not required. Must possess
excellent written and verbal communication skills and critical thinking skills. Ability to work
independently in a time oriented environment is essential.

Job Summary
Assesses, plans, implements and evaluates the nursing care of residents within the skilled nursing setting. Responsible for ensuring continuity of care of the residents between shifts by providing direct care as well as supervising the care given by CNAβs, LPNβs and supportive staff members. Participates in resident and family teaching/conferences. Maintains role as resident advocate with a focus on the facilityβs mission.

- Notifies physician of changes in residentβs condition and follows through until appropriate action is taken.
- Gives a thorough report to oncoming shift and participates in walking rounds.

- Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors. Supervises nursing staff to promote this.
- Implements plan of care for the resident based on assessments and goals as established by the interdisciplinary care team.
- Supervises and redirects other members of the healthcare team, as needed, to accomplish the Plan of Care which has been developed.
- Reviews staffing for own shift and upcoming shifts, takes action to obtain adequate staffing.
- Administers medications, following the five medication rights and reduces the potential for medication errors.
- Functions as a leader for the CNAβs and LPNβs by delegating in a manner that contributes to the excellence of the nursing department and maximizes teamwork.

The salary range for the role is $106108 - $159161 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need.
