The Pediatric NP Provider Hospice/Palliative Care has two primary responsibilities.

- Provide NP-appropriate service to patients enrolled in hospice and may also assist in staffing pediatric palliative care consultation services that function as a program of MJHS Hospice and Palliative Care
- Work in the community environment, and may, on occasion, provide consultation in the long-term care environment or hospital.
- In all settings, provide in-person visits or telehealth visits, as appropriate.
- Providing comfort care consistent with best practices, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation
- In the role as hospice NP, you will coordinate with the team physician and manager and will be responsible for face-to-face evaluations for hospice eligibility, and the assessment and management of hospice patients in need of exigent medical care
- Contribute to the training of the hospice team, providing support for the interdisciplinary team, as needed
- Help build a teaching curriculum on pediatric aspects of hospice care that will be of use to our hospice workers who are engaged in pediatric care through cross coverage for when young adolescents are incorporated into adult hospice teams
- In providing both direct care and support to other clinicians, the NP will use an evidence-based approach to management and to the use of materials and policies/procedures
- Other activities related to quality improvement projects may also be required, as appropriate

The Nurse Practitioner (NP) provider is one of a group of MDs and NPs who staff palliative care consultation services that function as programs of MJHS Hospice and Palliative Care. The NP Provider may work in a nursing home, hospital or community environment. Specialist skills in symptom management, communication including goal setting discussions, and hospice eligibility review, and may work independently or as part of a treatment team. Initial patient encounters are in response to consultation requests, and the initial consultation may be followed by one or more visits, as appropriate to the referral questions and status of the patient and family. The care plan advanced by the NP Provider aims to prevent or mitigate illness burden for the patient and family. Each encounter comports with best practices of specialist palliative care, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation. Other responsibilities include developing quality initiatives, educating staff, and supporting hospice operations through participation in the provider on call rotation and performance of face to face encounters as needed.
