ChristianaCare is seeking a detail‑oriented and patient‑focused Medication History Technician to join our team. In this critical role, you will collect and confirm precise home medication details shared by patients and their caregivers, helping ensure medication safety from the moment of admission. Your work will directly support pharmacists and physicians in delivering high‑quality, coordinated care.

This position is full time day/evening shift rotation. Every other weekend required; holidays as required.

Principal Duties and Responsibilities:

• Interview patients, caregivers, and other sources to acquire accurate and complete home medication histories.
• Ask clarifying questions regarding medication names, dosages, frequency, and last used to ensure accurate data collection.
• Enter collected information into the EMR following established procedures to produce admission medication reconciliation documentation.
• Generate medication history reports for pharmacist review and subsequent physician use.
• Collaborate closely with inpatient pharmacy staff and other care providers to support medication‑related needs.
• Navigate the ED tracking board to determine appropriate patient selection.
• Update or build medication records within the EMR with accuracy and attention to detail.
• Apply good clinical judgment when assessing medication lists; identify inconsistencies or concerns and communicate them to the pharmacist.
• Verify medication information using external sources when necessary (pharmacies, provider offices, etc.).
• Coordinate with charge nurses and patient nurses to determine optimal times for patient interviews.
• Demonstrate strong knowledge of both brand and generic medications, common dosages, dosage forms, and typical administration schedules.
• Communicate effectively with patients of all education levels in a fast‑paced Emergency Department setting.
• Assess the completeness of medication lists, including drug name, dose, frequency, indication, and time of last dose.
• Acquire accurate histories of PRN medications, including frequency and indications.
• Determine patient medication adherence prior to admission.
• Build rapport with patients and families to acquire reliable histories or identify alternative information sources when needed.
• Prompt patients for overlooked medications, including over-the-counter items such as herbals, vitamins, pain relievers, cold remedies, inhalers, patches, creams, eye drops, and more.
• Acquire and document medication allergy histories and associated reactions.

Education and Experience Requirements:

• High School diploma or equivalent required; Associate or Bachelors degree preferred
• Previous hospital or retail pharmacy experience preferred
• Pharmacy Technician Certification Board certification (PTCB) required
• Applicants enrolled in, or graduate of a 4-year bachelor program in the following health science majors are encouraged to apply and will be required PTCB certification in 1 year of employment: Applied Molecular Biology and Biotechnology Biology, Chemistry, Exercise Science, Health Behavior Science, Human Physiology, Medical Diagnostics, Medical Diagnostics Pre-PA, Medical Laboratory Science, Nutrition, Nutrition and Dietetics, Nutrition and Medical Sciences, Nursing, Occupational Therapy, Physical Therapy, Physician Assistant or Sports Health
• Computer experience preferred (e.g., window-based programs)

• Serves as pharmacy representative on multidisciplinary committees, as assigned
• Implements staff development and educational programs
• Assists with training and onboarding of new pharmacy personnel
• Assists with the coordination of residents and student learners
• Maintains competence required for current job title/position
• Performs other duties as assigned

Position Requirements:

• > 6 years practice experience, Pharm.D. with PGY1 and PGY2 training (or equivalent experience), Illinois licensure, and board certification (specialty certification, as appropriate)
• Excellent clinical, written/verbal communication, and problem-solving skills
• Experience supervising pharmacy personnel and precepting pharmacy learners
• CPR certified
• Must be creative and innovative in addressing challenges and opportunities.
• Will be part of overnight rotation when overnight pharmacists are on vacation

JOB DESCRIPTION: PHARMACY CLERK

Description of Responsibilities

The Pharmacy Clerk assists the Pharmacy Department and staff through a variety of duties including but not limited to clerical, customer service and inventory management.

Responsibilities Include the Following:

1. Clerical and administrative tasks such as answering phones, customer service, data entry.

2. Reviewing compounding sheets

3. Gathering necessary supplies and products to assist with fulfillment of orders.

4. Labeling finished product for pharmacist to check.

5. Maintains work areas and performs continual inventory to help maintain adequate stock.

Minimum Qualifications:

1. Organizational skills sufficient to maintain consistently accurate records.

2. Ability to evaluate options and to make efficient decisions.

3. Computer skills that include proficiency with spreadsheets, word processing, and efficient use of the internet and e-mail.

4. Ability to use good judgment and work with minimal direction.

5. Must possess excellent oral and written communication skills, with the ability to express technical issues in “layman” terms.

Education and/or Experience:

• Must have a High School diploma or Graduation Equivalent Diploma (G.E.D.) or Higher.
• Minimum of 1 year retail pharmacy experience preferred
• Home Infusion Experience preferred.
• Pharmacy Tech license is a plus but not required.

The Allied Health School (AHS) Pharm Tech Educator is responsible for the development and implementation of curriculum designed to prepare students for successful careers in various allied health fields. This role involves the development of educational programs that accounts for diverse learning styles aligned with student demographics and learning methods, revising the curriculum to align with current industry standards ensures course curriculum aligns with accreditation standards and ensures academic excellence and professional readiness in the dynamic healthcare landscape. The AHS Pharm Tech Educator is responsible for ensuring the delivery and teaching of the material as well as maintaining administrative components of education system including grading, reviewing polices & creating testing standards.

Qualifications:

• High School Diploma/GED required. Assoc. Degree/College Diploma preferred.
• 4 years of pharmacy related work experience required.
• 3 years of acute care pharmacy experience including prior experience in teaching, training, mentoring students/trainees/employees preferred.

Licenses and Certifications:

• Registered as a Pharmacy Technician with the California State Board of Pharmacy required.
• Accredited National Pharmacy Technician Certification through PTCB (Pharmacy Technician Certification Board) or NHA (National Healthcareer Association) required.
• Certified by American Medical Technologist as an Allied Health Instructor (AHI) preferred.

We are seeking an experienced Director of Pharmacy (PIC) to lead pharmacy operations at an acute care facility. This is a leadership role ideal for candidates with strong inpatient and regulatory experience.
