Monitors clinician’s progress towards action plan goals and evaluates clinician’s ability to maintain improved and sustained performance.

Evaluates clinicians individually and collectively as a team in their ability to provide care and services in a manner that facilitates the achievement of improved patient experience of care scores as reported by Press Ganey.

Provides oversight and direction related to the scheduling of patient care services by the appropriate level of clinician to ensure high quality care delivery, continuity of care, and staff productivity.

Reviews and processes various reports to assess staff adherence to agency stated workflows.

Any noted variances are acted upon in a timely manner to improve performance and avoid future deviations from established guidelines and workflows.

RN MUST have OASIS / clinical outcome measures.

Education BSN / Master’s degree preferred.

Experience Minimum of 3 years’ experience in a Certified Home Health Agency; supervisory experience preferred.
