The Physical Therapist performs physical therapy evaluations and treatments as ordered by qualified physicians in accordance with company standards and federal, state, and local standards, guidelines, and regulations to ensure that the highest degree of quality care is maintained at all times.
Essential Activities and TasksΒ
Expectation and Care Management - 60%
Initiates contact with physician for orders and treatment plan formulation and/or revision as the patientβs condition requires.
Participates in the development and implementation of the patient OASIS assessments, Evaluations, and discharge OASIS documentation.Β
Performs physical therapy treatments as prescribed by the physician, evaluating and treating patients by applying appropriate therapy techniques and modalities in clientβs home environment.Β
Recommends modifications or changes in the patientsβ Plan of Care based on evaluation of their progress toward the patient goals.
Educates/instructs/recommends patient, family members and other health team members of the proper techniques, for exercise program, precautions and self care activities to assure safety for caregivers and patient.
Participates in the interdisciplinary team meeting (ITM) by exchanging team information on the progress and discharge plan in order to provide the highest level of continuity of care for the patient.
Communicates with client, family members, clientsβ advocate(s), caregivers and physicians on patientβs progress and discharge plan on a regular basis.
Conducts home visits to implement the patientβs Plan of Care in use of therapeutic and self care activities for the purpose of improving function and safety in order to allow the patient to return home at their highest level of function and independence.
Participates in discharge planning by exchanging information with staff members, family members and physician to promote continuity of client care, in accordance with corporate policies, federal, state and local guidelines.
Ensures that all work areas, treatment areas and equipment are maintained in a clean, sanitary and safe manner.
Administration and Reporting - 20%
Maintains current and accurate patient records through use of personal EMR device, and paper/chart documentation in accordance with company, federal, state, and local guidelines.
People Management and Development - 10%
Coaches and influences the work of the Licensed Physical Therapy Assistants (LPTA) regarding the performance expectations of their position.Β
Fosters a positive work environment that attracts, retains, and motivates employees.
Interviews, selects, and orients new employees.
Defines, establishes, and promotes performance expectations so staff understands and demonstrates excellence through high quality and consistent performance.

Participates in the development, planning, scheduling, conducting, and monitoring of in-service training programs, on-the-job training, and orientation programs for departmental staff.

Responds timely and thoughtfully to questions, requests, and work product from departmental staff that has been provided for review and approval.

Qualifications
Education
Current unencumbered license for the state of Ohio to practice as a Physical Therapist (PT) required.
Experience
One year experience as a Physical Therapist in a hospital, long-term care facility, or home health agency preferred. New graduates welcome.
Experience in the care of geriatric and physical therapy patients required.
Knowledge of treatment modalities.
Other Requirements
Must be able to read, write, speak, and understand the English language.
Must possess a valid driver's license and acceptable driving record. Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.

May be asked by the RN to send an employee home for the remainder of the shift if the RN determines that further investigation is needed. May document the performance concern on the appropriate form for review and use by the RN in the disciplinary process.
- Assist with lab request as ordered per facility policy using appropriate techniques. Responsible for re-posting lab work as ordered on unit, reviewing results and notifying attending MD per established policies, protocols, and physician orders.
- Assist with emergencies arising while on duty. Performs CPR if needed. Assist with emergencies on other units if needed.
- Assist with notifying attending MD of any change or illness of patients in the unit. Notifies attending physician of emergencies and death of patients in the unit. Implement order of changes in orders as instructed or ordered by MD.
- Assist with other general duties which include rounds with doctors; checking MD orders, including update of orders; verification of completeness of nursing summaries timely and verification that all nursing records are complete and timely.
- May be asked to provide hall coverage as needed.
- Assist with coordinating the health care team including the rehab aides duties and assignments.
- Other duties as assigned by Director of Nursing in accordance to NC Board Of Nursing Practice Acts.
Job Requirements:
- Must be a high school graduate
- Must be a Licensed Practical Nurse (LPN) graduated from an accredited School of Nursing, with a current, valid license as a Licensed Practical Nurse in North Carolina and submit proof of license renewal.

Strong knowledge of hospice philosophy and home health regulations

Care Management and Service Delivery - 90%
Provides ongoing assessment and nursing care to patient in homes and health care facilities. Ensures an accurate evaluation of patient needs, develops and implements a comprehensive care plan that addresses current needs. For home health, identifies when patients need or qualify for additional services and notifies appropriate team member or team leader.
Follows current nursing professional standards of practice and agency approved protocols for home health and hospice.
Provides quality plan and symptom management for the comfort of patients.
Communicates with other health care providers about changes in condition and patient needs.
Assesses patient and caregiver needs and provides education when necessary.
Commitment to high quality customer service for internal and external customers.
Oversees the implementation of the patientβs plan of care. Evaluates the effectiveness of the plan on an ongoing basis.
Prepares for IDG by reviewing Care Plan, reconciling medications and projecting patient/family needs, if applicable.

Education

Job Summary and QualificationsProvides coordinated skilled nursing care to patients of all age groups, in the home. - Completes OASIS, assessment and visit paperwork according to agency policy. Assures clinical notes accurately indicate continuing communication and coordination of services with the physician, other interdisciplinary team members and patient/family/caregiver.
- Communicates significant findings, problems and changes to Clinical Manager and physician, and documents all findings, communications, and appropriate interventions.
- Supervises and provides clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services provided.
- Responsible for participating in on-call rotation and emergency call according to agency policy.
- Graduate of an accredited school of professional nursing
- Minimum of one or more years of home health, public nursing or acute hospital nursing experience
- Familiar with Medicare home health regulations, documentation requirements, ICD-10 coding and PPS (Strongly preferred)
- Reliable transportation and proof of valid automobile liability insurance
- Must have valid driver's license
