TheΒ Transitions of Care Nurse (RN)Β is a field-based role focused on patients experiencing an admission, discharge, or transfer (ADT) event. This nurse responds to real-time ADT alerts, engages patients during hospitalization, and coordinates seamless transitions across care settings. The role ensures safe discharges, prevents avoidable readmissions, and supports patients through the critical first 90-day post-discharge.
Key Responsibilities
- Respond to ADT alerts in real time and deploy to the hospital at admission to enroll patients into Upward Health services.
- Collaborate with hospital staff, providers, and discharge planners to create safe transition plans.
- Conduct a home visit within 2 business days of discharge to reconcile medications, confirm follow-up appointments, and assess home safety.
- Address post-discharge needs, including arranging home health, physical therapy, or durable medical equipment.
- Provide care management for up to 90 days post-discharge, with a focus on preventing readmissions and supporting patient goals.
- Educate patients and caregivers on care plans, treatment adherence, and community resources.

Urban Dove Team Charter School (UD Team) is a distinctive network of sports-based high schools catering to over-aged, under-credited youth. We fuse a robust academic curriculum with sports-based youth development, ensuring our graduates receive a diploma, a stepping stone for higher education, and essential job skills.
CORE RESPONSIBILITIES:
- Provide individual and group therapy to ~80 students, either in response to crisis or ongoing, depending on the needΒ
- Establish partnerships with families, teachers, coaches and outside agencies to provide necessary social and emotional support to students.

POSITION SUMMARY
The Medical Director works closely with the Chief Medical Officer to provide vision for and implementation of medical programs and services across Fora Healthβs Departments. The Medical Director is an integral part of a collaborative leadership team, which includes the Director of Psychiatric Services, Medical Operations Director and Nurse Manager. This team, in turn, collaborates with Fora-wide leadership, representing other programs in the continuum of care to support Fora Healthβs mission of providing excellent care for those with substance use disorders.
The Medical Director will provide direct patient care in 3.7 level withdrawal management and medically managed residential programs. Additionally, the MD will direct expansion of both outpatient and residential MAT services. The MD will also direct the development of expanded medical services for patients enrolled in Fora Residential and Outpatient SUD programs. The MD will collaborate with Operations & Admissions leadership to develop and strengthen rapid access pathways from local hospitals into appropriate levels of care at Fora.
The Medical Director will join the Chief Medical Officer in nurturing a program culture that centers excellent medical care within a framework of loving kindness, harm reduction, and trauma informed care.
Expected allocation of time is approximately 60% administrative and 40% clinical.
ESSENTIAL JOB DUTIES
- Completing medical intakes on patients admitting to withdrawal management and residential treatment
- Formulating treatment plans for those patients and communicating with nurses and case managers as necessary regarding implementation of medical orders and ongoing assessment of patients
- Reviewing medical referrals to Medically Managed Residential Program and communicating with referents, including hospitals and outpatient settings.
- Continuing expansion of MMR program to include patients on IV antibiotics, with complex wound care needs, and those newly dependent on insulin
- Developing curricula & structure for residential medical groups to address PAWS, tobacco cessation, and MAT for AUD
- Providing clinical education to nurses
- Sharing overnight phone call with Chief Medical Officer and advance practice providers, typically one week per month.

β’ Develop and sustain a workplace that mirrors our Employee Value Proposition through the following: Advise associates and management on the interpretation of human resources policies, programs, procedures, and collective bargaining agreements.
Participate in investigations of associates concerns, prepare summaries, and collaborate with Area HR Manager on recommendations.

HR Expertise & Compliance β’ Remain aware of employment laws applicable to locations, including but not limited to state and local paid sick leave laws.

The Director of Trade will work collaboratively with diverse members of Supply Chain, Commercial, and Corporate functions to achieve individual, group, and organizational goals.
Responsibilities Strategic Account Management: Lead and advance strategic account relationships with assigned specialty distributor and specialty pharmacy customers.
Develop and implement comprehensive account plans to align distribution strategies with brand objectives and patient access goals.
Develop and lead negotiation strategies with Trade accounts that secure favorable market aligned terms, conditions, and fees for DSI and its products while ensuring optimal patient, HCP, and pharmacy experience when sourcing DSI medications Oncology Channel & Distribution Strategy: Engage directly with key customers, vendors, and internal stakeholders to identify new ways of addressing channel challenges and create win-win-win value for DSI, the account, and the patient and their provider.
Develop, implement, and manage channel and account-based product distribution models aligned to Brand strategy, Market Access, and Finance goals.
Models and solutions may include: oOrder fulfillment and transportation models oAlternative distribution methods (e.g.
direct distribution, drop-ship, other) oLimited or exclusive Specialty Pharmacy arrangements Accountable for channel performance, pull-through, inventory levels, and order patterns to ensure channel integrity and product availability.
Cross-Functional Collaboration: Drive collaboration with home office teams (e.g.
Supply Chain, Finance, Forecasting, Market Access, Brand Marketing, Sales Operations, and Legal) to address a broad range of customer or channel challenges and opportunities and ensure timely analysis, selection, and implementation of strategies and solutions.
Partner with Patient Services and Field Reimbursement teams to ensure providers and patients have timely access to DSI products.
Provide strategic insights and channel intelligence to support Commercial (e.g.
Forecasting and Brand Marketing) planning initiatives in order to achieve channel readiness for new products launches and maintenance of appropriate inventory levels post launch.
Partner with Regulatory, Compliance, and Quality teams to ensure all trade activities adhere to legal and industry standards.
Business Analytics & Market Intelligence: Analyze trade partner data and distribution trends to inform strategic decisions and address barriers to access.
Deliver regular performance reviews, including adherence to KPIs , to strategic accounts.
Maintain expertise in oncology and rare disease market trends, evolving provider needs, and channel participants (Distributors, Pharmacies, PBMs, IDNs, GPOs, Community Oncology Providers, other).
Qualifications Education Qualifications Bachelor's Degree required MBA preferred Experience Qualifications 10 or More Years overall related experience required 7 or More Years Demonstrated experience and proven success leading contract negotiations with (and managing) large commercial accounts, developing and implementing innovative distribution models and/or limited distribution networks, and collaborating with diverse stakeholders to resolve complex challenges in Pharmaceutical Managed Care, Trade, Supply Chain, or similar environments.
required 7 or More Years Developing deep understanding of the oncology product lifecycle, including buy-and-bill and pharmacy benefit models, specialty pharmacy, white/brown/clear bagging, and patient access services and models.
required 7 or More Years Building relationships with Trade and Channel participant executives that have enabled and driven successful strategic initiatives with aligned goals and shared value.
required Familiarity with REMS programs and cold chain logistics preferred Experience managing or collaborating with third-party logistics (3PL) providers preferred Travel Requirements Ability to travel up to 30% of the time.
Travel requirements of at least 5-8 days per month and occasional weekend commitments.
