Role Description:
The Medical Social Worker is a key member of Oak Street Health Centers, contributing a unique and valuable perspective to care teams to support the psychosocial needs of our patients. In collaboration with the interdisciplinary care team, they will provide assessments, advocacy, and comprehensive care coordination services to our highest-risk patients and their caregivers to help them improve their health status and avoid hospitalizations.
Core Responsibilities:
- Proactively support our highest-need patients by providing comprehensive assessments, telephonic check-ins, and in-person visits.
- Partner with the patient and their support persons (if necessary) to identify goals, increase engagement in their healthcare, address barriers, and drive the execution of their care plan.
- Maintain extensive knowledge of community resources and make referrals, as appropriate, to help ensure the patient's healthcare and psychosocial needs are met.
- Navigate patient insurance benefits in collaboration with the Patient Relations Manager (PRM) to ensure mindful utilization and maximization of plan benefits
- Provide both longitudinal and episodic comprehensive care coordination with the ultimate goal of empowering patients to independently manage their health conditions and have increased quality of life.
- Educate patients on supportive care options (including advance care planning and end-of-life care) and provide goals of care counseling, support, and advocacy at all stages of the patient journey.
- Provide psychoeducation to patients and families surrounding life stage and disease-specific issues, including dementia, adjustment to illness, aging, caregiver support, etc.
- Meet with care teams to collaborate and generate creative solutions to patient issues, and spearhead intervention in situations with complex psychosocial dynamics.
- Complete concise and comprehensive documentation on time to ensure that all care team members have visibility into the status of MSW interventions
- Partner with Community Health Workers and provide clinical guidance and consultation regarding complex patient needs to these team members.
- Respond to crises and urgent needs with the keen ability to prioritize work, triage situations, and multitask.
- Other duties as assigned.
This role reports to the Practice Manager.
What we're looking for:
Required Qualifications:
- Master of Social Work degree required from a school of social work accredited by the Council on Social Work Education (CSWE)
- Minimally licensed in the state where OSH Center is located
- Proficient computer skills (including Microsoft, GSuite, and electronic medical records)
- US work authorization
Strongly Preferred Qualifications:
- Prior experience with complex care coordination and/or population healthcare management
- Fluency in Spanish, communities we serve (where necessary)
- Familiarity with using metrics and data to guide practice and drive outcomes
- Minimum one year of medical social work experience in a healthcare environment, preferably within the last two years
- Experience working on a multidisciplinary team of other healthcare providers

The typical pay range for this role is:

Job Summary:
Reporting to the Manager, Air Operations, this role is responsible for the supervision, administration, and work management of Retail Air Team. Collaborate with Management across departments, including Outside Sales, to generate retail revenue, maintain a healthy profit margin and provide optimum customer service in the retail channel. Assume responsibility of first tier air emergency response on and off hours on a rotating basis.
Primary Functions:
- Supervise, train and develop direct subordinates.

Job Summary:
This position reports to the Manager of Air Operations and is responsible for securing air using our airline contracts and accurately calculating the price to the customer for all retail passengers on all of Collette's tour series. Each coordinator's goals are to obtain the best possible air schedule for our clients, at a reasonable cost to the customer and provide a timely turn around on all requests.
Primary Functions:
- Create air reservations for all standard bookings in NEO, where the air server was unable to successfully make an air reservation.
- Calculate pricing for all air reservations created to ensure appropriate margins are met.
- Work all special requests made on retail reservations in a timely manner.
- Responsible for proofing all air reservations created by the Air Server in NEO to ensure all standards of quality are met.
- Verify that the reservation was priced correctly according to our contracts.
- Verify that the flights chosen meet all time restrictions on the package our customers are booked on.
- Verify that all connection times meet standards described in our policies as well as each airlines minimal connection policy.
- Request Seat Assignments, Wheelchair Request, Special Meal request or any other special needs with the carriers booked.
- Process Frequent Flyer information over to the airline.
- Coordinate any changes on retail air reservations and provide a timely turn around on those requests.
- Assist with re-protecting passenger's air due to any schedule changes on retail air reservations when the airline has protected the passengers on schedules that no longer meet minimal connection times and/or time restrictions on the tour package booked.
- Research all service issues and determine best action for recovery resolution and report back to Manager.
- Responsible for covering emergencies on pre-scheduled Saturdays/Holidays throughout the year.
- Perform special projects and other related duties as assigned or directed by the VP of Operations, Manager of Air Operations and Retail Air Team Lead.

EDUCATION
Certified Registered Nurse Anesthetist Completion of Certified Nurse Anesthetist Training Program Graduate of a Nurse Anesthesia Educational Program accredited by the American Association of Nurse Anesthetist Council on accreditation of Nurse Anesthesia Programs. Current State of Connecticut Nursing License, Advanced Practice Registered Nurse (APRN) licensure in the State of Connecticut.
EXPERIENCE
Prefer clinical working experience as a CRNA within the given specialty/service line recommended, but none required. APRNs, PAs, CRNAs and CNMs are required to be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process. Evidence of current competence to practice as {an APRN, PA, CRNA, CNM} via appropriate reference letters from physicians and other practitioners must be able to be obtained during the Medical Staff appointment process in order to be eligible.

SPECIAL SKILLS
CRNA's must have sharp concentration and focus, along with the ability to stay calm under stressful conditions. CRNAs must be able to communicate clearly and effectively with surgeons and the surgical support team, as well as with patients and their families. A calm and reassuring manner is one way to inspire trust in patients, who may be apprehensive about surgery, as well as the surgical team. Anesthetic patient management must be immediate and responsive. Flexibility and adaptability are required.
PHYSICAL DEMAND
A CRNA must be able to demonstrate intellectual-conceptual, integrative and quantitative abilities; skills in observation, communication and motor functions; and mature behavioral and social attributes. Sensory- CRNA must be able to detect and interpret changes in monitoring alarms and equipment. Functional vision, hearing. no impairment of sense of smell and tactile sensation must be adequate. CRNA must be able to observe a patient accurately at a distance and close.Communication-CRNA should be able to speak, hear and observe. CRNA must be able to communicate effectively- includes speech, reading and writing.Motor -CRNA should have sufficient motor function to elicit pertinent information.
