Responsibilities

As a Home Health Clinical Manager, You will:

  • Conducts/participates in quarterly quality improvement meetings, reviews pertinent available data, identifies trends, and oversees the implementation of improvement plans
  • Collaborates with the Branch Director in the implementation of quality improvement activities and corrective action plans
  • Responsible for the oversight of and response to external and internal surveys and audits within the assigned area relating to the quality of care and clinically related KPIs in collaboration with operations leadership
  • Completes documentation and chart reviews to identify variances in standardized care and care processes and to identify areas for improved performance related to patient care and the organization
  • Holds case conferences to ensure oversight of care, coordination of services and that standards are met
  • Facilitates the development of performance improvement action plans, based on available data (e.g., survey results, QAPI and clinical record reviews). Accomplishes staff results by communicating job expectations; planning, monitoring, and appraising job results; coaching, counseling, and disciplining employees; developing, coordinating, and enforcing systems, policies, procedures, and productivity standards
  • Ensures assigned staff are evaluated fairly and timely in accordance with company performance evaluation procedures

Qualifications

  • Bachelor’s Degree preferred in Nursing from an accredited school of nursing
  • Licensed Registered Nurse (RN) in good standing and currently licensed by the State
  • Less than two years of clinical experience, and at least one year in a related management/supervisory role in home health care
  • Current CPR certification
  • Must meet all agency requirements for pre-employment as required by the company and/or State regulations
  • Ability to use company documentation systems
  • Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company
  • Ability to work independently as well as part of a team

EDUCATION

Certified Registered Nurse Anesthetist Completion of Certified Nurse Anesthetist Training Program Graduate of a Nurse Anesthesia Educational Program accredited by the American Association of Nurse Anesthetist Council on accreditation of Nurse Anesthesia Programs. Current State of Connecticut Nursing License, Advanced Practice Registered Nurse (APRN) licensure in the State of Connecticut.

EXPERIENCE

Prefer clinical working experience as a CRNA within the given specialty/service line recommended, but none required. APRNs, PAs, CRNAs and CNMs are required to be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process. Evidence of current competence to practice as {an APRN, PA, CRNA, CNM} via appropriate reference letters from physicians and other practitioners must be able to be obtained during the Medical Staff appointment process in order to be eligible.

SPECIAL SKILLS

CRNA's must have sharp concentration and focus, along with the ability to stay calm under stressful conditions. CRNAs must be able to communicate clearly and effectively with surgeons and the surgical support team, as well as with patients and their families. A calm and reassuring manner is one way to inspire trust in patients, who may be apprehensive about surgery, as well as the surgical team. Anesthetic patient management must be immediate and responsive. Flexibility and adaptability are required.

PHYSICAL DEMAND

A CRNA must be able to demonstrate intellectual-conceptual, integrative and quantitative abilities; skills in observation, communication and motor functions; and mature behavioral and social attributes. Sensory- CRNA must be able to detect and interpret changes in monitoring alarms and equipment. Functional vision, hearing. no impairment of sense of smell and tactile sensation must be adequate. CRNA must be able to observe a patient accurately at a distance and close.Communication-CRNA should be able to speak, hear and observe. CRNA must be able to communicate effectively- includes speech, reading and writing.Motor -CRNA should have sufficient motor function to elicit pertinent information. CRNAs should be able to stand for long periods of time and wear lead as needed for fluoroscopy, X-Ray cases. CRNAs must have the ability to lift 25 pounds and reach above and below shoulder height. CRNA must be able to negotiate patient care environments and be able to move self/patients between anesthesia locations. Coordination of both gross and fine muscular movements, equilibrium, and functional use of the senses of touch and vision is required. Intellectual-Conceptual, Integrative and Quantitative Abilities-Abilities include measurement, calculation, reasoning, analysis and synthesis. Problem-solving, the critical skill demanded requires all of these intellectual abilities. CRNA should be able to comprehend 3-dimensional relationships and understand the spatial relationships. Behavioral and Social Attributes -CRNA must possess the emotional health required for full utilization of intellectual abilities, the exercise of good judgment, the prompt completion of responsibilities.   • Develop and facilitate comprehensive onboarding programs for new hires, fostering a positive organizational culture both virtually and on-site.
  • Design and implement engaging team programs to motivate and inspire employees.
  • Lead recruitment efforts for exempt, nonexempt, and temporary positions, including creating and placing job advertisements.
  • Identify staffing needs, conduct interviews, and select qualified applicants for various roles.
  • Manage the posting process for all open positions and communicate company policies, benefits, and procedures to employees and job applicants.
  • Administer compensation, benefits, and performance management systems, along with safety and recreation programs.
  • Support employee relations matters, including documentation, progressive discipline, and leave administration (FMLA, ADA, personal leave).
  • Write and implement Progressive Discipline Documentation, ensuring adherence to company policies.
  • Participate in setting department goals, objectives, and the implementation of new systems.
  • Accurately process and interpret both written and verbally communicated information, ensuring clear follow-up and documentation.
  • Prepare payrolls for all companies and oversee timecard accuracy.
  • Provide detailed information to current and potential employees about job duties, policies, working conditions, promotions, and employee benefits.
  • Conduct open enrollment sessions for the BCV team and maintain records, compiling HR-related statistical reports.
  • Assist in creating and updating job descriptions and HR policies.
  • Ensure compliance with labor posters in all office locations.
  • Leverage HRIS and business technology platforms, including Microsoft Office Suite, Paylocity, Navigator, insurance carrier systems, Outlook, and Microsoft Teams, to support HR operations and reporting.
  • Travel as needed to support onboarding and employee engagement initiatives.
  • Perform additional duties as assigned to support department growth and organizational success.
