The advanced practice provider provides health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions. Prescribes medication, examines patients, diagnoses illnesses, and provides treatment. Advises executives to develop functional strategies (often segment specific) on matters of significance. Exercises independent judgment and decision making on complex issues regarding job duties and related tasks, and works under minimal supervision, uses independent judgment requiring analysis of variable factors and determining the best course of action.

Patient Care:

  • Evaluates and treats center patients in accordance with standards of care.
  • Follows level of medical care and quality for patients and monitors care using available data and chart reviews.
  • Assists in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment and home health care.
  • Acts as an active participant and lead with the care team through daily huddles.
  • Helps Regional Medical Director (RMD), Physician and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor.
  • Completes all medical record documentation in a timely manner working with a quality- based coder to optimize coding specificity.
  • Follows policy and protocol defined by clinical leadership.
  • Meets with RMD about quality of care, review of outcome data, policy, procedure and records issues.
  • Participates in potential growth opportunities for new or existing services within the Center.
  • Participates in the local primary care ?on-call? program of Conviva Senior Primary Care.
  • Assures personal compliance with licensing, certification, and accrediting bodies.

Required Qualifications

  • Minimum of two to five years directly applicable experience
  • Successful completion of a nurse practitioner program with board certification
  • Active, unrestricted nurse practitioner license in state of practice location
  • Excellent communication skills, written and verbal; demonstrate a high level of skill with interpersonal relationships and communications with all colleagues; fully engaged in the concept of ?Integrated team-based care?.
  • Willingness and ability to learn/adapt to practice in a value-based care setting.
  • Superior patient/customer service.
  • Basic computer skills, including email and EMR.
  • Appropriate certification to write prescriptions under the authority of a collaborating physician/medical director.
  • This role is considered patient facing and is a part of our Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
  • Bilingual Spanish and English

  • Active and unrestricted DEA license
  • Medicare Provider Number/ Medicaid Provider Number
  • Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value based relationship environment.

How You Improve Our Patients Lives!

  • Conduct comprehensive patient assessments and obtain medical histories focused on allergy and sinus conditions.
  • Diagnose and manage acute and chronic allergic and sinus conditions, including allergic rhinitis, chronic sinusitis, nasal polyps, and asthma.
  • Order and interpret diagnostic tests, including allergy testing, sinus CT scans, and pulmonary function tests.
  • Develop individualized treatment plans including medications, immunotherapy, and procedural recommendations.
  • Perform and assist with in-office procedures such as:
    • Administration and supervision of allergy immunotherapy (SCIT and SLIT)
    • Nasal endoscopy
  • Educate patients on disease management, treatment options, and medication use.
  • Collaborate with physicians and staff to ensure continuity and quality of care.
  • Document all patient encounters accurately in EHR within 24 hours of their visit
  • Oversee and manage both food and environmental allergy testing for patients by utilizing both Percutaneous Allergy Skin Testing and Intracutaneous Allergy Testing.
  • Provide pre-op and post-op evaluation and treatment after a variety of surgeon performed sinus procedures.
  • Develop relationships with referring medical providers and the local medical community.
  • Attend lunches and events with the assigned territory provider relationship rep. to help garner credibility and market Aspire.
  • Adhere to standard metrics:
    • Testing: 50% minimum threshold
    • Treatment: 30% minimum threshold
    • IOP Conversion: 7% minimum threshold
  • Other duties as assigned.

Knowledge & Skills Needed to be Successful

Required education and experience

  • 3+ Years of Practicing as a Physician Assistant/Nurse Practitioner
  • Graduate from an approved Physician Assistant/Nurse Practitioner Program
  • Current Physician Assistant/Nurse Practitioner License
  • Basic Life Support (BLS) or Advanced Cardia Life Support (ACLS) Certified
  • Must have a valid driver?s license

Preferred education and experience

  • Previous experience working with an EMR/EHR system

Duties and Responsibilities: As a BPA, you will be part of our 60,000 workforce that strives to protect the American people by safeguarding our borders, deterring illicit activity, and enhancing the nation's economic prosperity.

Being a BPA makes you a valuable member of the Federal Law Enforcement Officer (LEO) profession.

Typical assignments include: Detecting and questioning people suspected of violating immigration and customs laws and inspecting documents and possessions to determine citizenship or violations.

Preventing and apprehending aliens and smugglers of aliens at or near the borders by maintaining surveillance from covert positions to include using infrared scopes during night operations.

Interpreting and following tracks, marks, and other physical evidence of illegal entry of persons or contraband.

Performing farm checks, building checks, traffic checks, city patrols, and transportation checks.

Patrolling the international boundary and coastal waterways using a variety of government assets such as vehicles, horses, vessels, watercraft, off-road vehicles, ATVs, snowmobiles, and motorcycles for the accomplishment of the USBP Mission.

Qualifications: GL-9: You qualify for the GL-9 grade level if you possess one (1) year of specialized experience, equivalent to at least the next lower grade level, performing duties such as: Performing physical searches of detained individuals, their vehicles, and their immediate surroundings for weapons, contraband, currency, and other evidence.

Apprehending, physically restraining, or working with law enforcement officials to identify or apprehend violators of state, federal, or immigration laws.

Proficient in the use of firearms, preparing investigative reports, serving court orders (e.g., warrants, subpoenas, etc.), and gathering evidence for criminal cases prosecuted through the court system.

If you have previous or current law enforcement or military law enforcement experience, you may qualify at the GS-11 grade level.

GS-11: You qualify for the GS-11 grade level if you possess one year of specialized experience equivalent to at least the next lower grade level, with the authority granted by the state or government to enforce laws, make arrests, and investigate crimes, performing primary duties in an official law enforcement capacity such as: Utilizing intelligence information to track illegal operations, criminal activity, threats to our nation, and/or contraband while serving as a state, federal, or military law enforcement official.

Leading investigations of fraud, contraband, criminal activity, threats to our nation, and/or illegal operations to determine sources and patterns while serving as a state, federal, or military law enforcement official.

Apprehending violators of state, federal, or immigration laws by utilizing various forms of complex technology that include surveillance, detection, situation awareness systems, and/or communications equipment.

The above experience will be applied in connection with the following: Make arrests and exercise sound judgment in the use of firearms; deal effectively with people in a courteous manner in connection with law enforcement matters; analyze information rapidly and make prompt decisions; or develop and maintain contact with a network of informants.

There are no education substitutions for the GL-9 or GS-11 grade-level Border Patrol Agent opportunities.
