Job Summary
The Palliative Care Advanced Practice Provider (APP) provides direct patient care services and performs medical acts under the supervision of the supervisory physician to patients >18 years of age requiring palliative care and symptom management. The APP performs physical exams, collects and documents data, conducts diagnostic and therapeutic procedures, orders and schedules laboratory studies and professional consultations, and prescribes appropriate interventions and medications. The Palliative Care APP contributes to excellence in patient care, research, teaching, and provides leadership in the organization.
Job Duties and Responsibilities
- In collaboration with attending palliative care physicians, diagnose, plan treatment, set priorities and realistic outcomes, and evaluate the effectiveness and cost efficiency of patient care.
- Provide and coordinate clinical care through established protocols and under the supervising physician or back up physician who is either on site or available by pager.
- Assess patients for change in status and institute appropriate interventions. Initiate emergency interventions to stabilize the patient until physician support arrives.
- Develop patient care pathways, protocols and guidelines for the management of specific problems in conjunction with physicians and other members of the health care team.
- Analyze clinical practice patterns to provide the best medical care and to increase effectiveness and efficiency.
- Communicate verbally and in writing all pertinent patient information to supervising MD, nursing staff, patient resource manager, home health agency, hospice agency and other relevant healthcare providers.
- Document in the patients' medical record all relevant data including but not limited to results of diagnostic tests; laboratory results; patients' condition and response to therapies/interventions; communications with supervising MD.
- Implement a specific plan of palliative care to alleviate suffering and promote the physical, emotional, psychological, and spiritual comfort of the patient and family.
- Utilize prescriptive privileges only for identified patient population and in accordance with DUH and NC Board of Nursing/NC Medical Board regulations.
- Prescriptive authority for prescribing, ordering, and or administering drugs; including (but not limited to) schedule II, III, IV, and V drugs and necessary medical devices.
- Role model competence in all nurse practitioner / physician assistant advanced skills.

JOB SUMMARY:
Collaboratively coordinates and provides care of the hospice patient population. Assesses staff educational needs related to the provision of clinical care services. Defines appropriate staff competencies in collaboration with the Duke HomeCare and Hospice (DHCH) Medical Director. Provides care and performs medical acts as a Nurse Practitioner, under the supervision of the supervisory physician, in the home or hospice inpatient care facility or partner facilities.
JOB DUTIES AND RESPONSIBILITIES:
- Provide direct patient care and may perform medical acts as a nurse practitioner under the supervision of the supervising physician in the home, community-based care setting, or hospice inpatient care facility or partner facilities. The supervising physician will be available by pager, mobile phone, or other electronic means.
- Assist in the coordination and development of the care plan collaboratively with the clinical staff, patient, family, physician and medical director, and establishes mutual goals.
- Assess and provide patient/family teaching based on patient and caregiver's needs.
- Participate in on-call rotation for medical coverage of hospice.
- Participate in weekly interdisciplinary patient care conferences. Establishes, reviews, updates, and summarizes care plans in keeping with Medicare CoPs and Licensure requirements.
- Partners with DHCH to identify learning needs of DHCH Staff and provides appropriate education in collaboration with the Directors of Patient Services/Hospice/Home Health/Infusion and the Clinical Nurse Educators.
- Participate in community educational projects.
- Participate in the performance Improvement Program of DHCH.
- Perform complex diagnostic and therapeutic procedures in diagnosing medical and surgical problems and prescribing necessary treatment and services for quality patient care; record normal and abnormal data and tentative conclusions for review by the physician.
- Coordinate and schedule laboratory studies and diagnostic procedures; explain the necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient.
- Monitor patient's status, maintain a record of the patient's condition and consult with supervising physician when patient's response does not meet anticipated and/or predetermined criteria.
- Participate in basic science and clinical research projects conducted by supervising physician; prepare statistical reports and analyses setting forth progress, adverse trends and appropriate recommendations or conclusions.

Duke Life Flight is a CAMTS accredited critical care and Emergency Air and Ground transport agency in Durham, NC, providing services to North Carolina and its surrounding states.
We operate two state-of-the-art rotor wing aircraft and a fleet of critical care, and advanced life support ambulances.
We are one of few rotor-wing aeromedical Instrument Flight Rules (IFR) capable flight programs in North Carolina.
Our respiratory care practitioners with our Life Flight team transport pediatric, and neonatal patients 24 hours a day, 7 days a week.
We provide rapid access to health care for patients when time, specialized skills, and/or specialized resources are critical for survival.
General Description of the Job Class Perform and evaluate diagnostic and therapeutic clinical procedures as defined in the Respiratory Care Services scope of clinical practice, performing work in accordance with physician orders in the neonatal, pediatric and adult patient populations.
Duties and Responsibilities of this Level Perform all duties and responsibilities required of a Respiratory Care Practitioner I.
Evaluate appropriateness of medical orders.
Proficient in the application of mechanical ventilator modalities.
Proficient in the application of Non-Invasive Positive Pressure Ventilation (NPPV); Continuous Positive Airway Pressure (CPAP) and Bi-Level Positive Airway Pressure (BiPAP) Perform endotracheal intubation under the observation of a Supervisor or Advanced Practitioner.
Perform extubations.
Serve as member of the code/ emergency team.
Administer medical gases.
Insert nasogastric and orogastric tubes.
Perform tracheotomy tube changes.
Perform capnography monitoring.
Transport patients requiring mechanical ventilation or airway protection.
Interpret clinical data to optimize patient clinical objectives.
Participate in clinical rounds with physicians, nurses and other health care providers Assist with Bronchoscopy.
Assist with orientation and training of Level II RT.
Assure safe operations of devices in the MRI.
Qualified for certification in the hyperbaric chamber if applicable to the institution scope of practice Required Qualifications at this Level Education Completion of a two year AMA approved Respiratory Care Program
