This Post Acute Care Sales Representative role offers a base salary of approximately $85,000, with an initial sales incentive of $35,000. This incentive is uncapped, and a starting point for all representatives entering the role. Field Sales Representatives have the opportunity to earn supplemental newbusiness bonuses on top of their incentive plan, providing further upside potential for strong performance. This structure rewards growth, strategic expansion, and valuedriven customer engagement across the western Virginia market. Sales compensation also includes our Auto Program. You may select a company vehicle or a monthly auto allowance of $668.00.

Job Summary and QualificationsAs a Registered Nurse for Admission and Discharge, the top priority is delivering exceptional patient-centered care. They are dedicated to ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings. Specialized assessment skills help ensure that patients are placed in the appropriate care areas and discharged. We offer education and post-hospitalization care as necessary. What will you do in this role:
- Obtains a nursing assessment history, and ensures proper inpatient care unit admission. Takes appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.
- Completes and facilitates the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
- Teaches patients/families/caregivers about a patient’s medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete knowledge.
- Completes extra discharge documentation, education, and discharge care for special patient populations, to ensure compliance with core measures for discharge.
- Ensures patients /families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.
- Coordinates with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
