ESSENTIAL FUNCTIONS:
- Administers nursing care to patients; includes assessment, planning, implementing, and evaluating the patient’s care plan in coordination with the physician.
- Provides patient/family or caregiver education and counseling in recognition and solution of physical, emotional, and environmental health problems.
- Supervises health care members (i.e., licensed practical nurses, patient care technicians, etc.) with patients.
- Utilizes computer applications and communication technologies appropriate to the work setting to enter, retrieve, and analyze information and to facilitate communication and education.

- Maintains knowledge of equipment set-up, maintenance, and use (i.e., IV pumps, monitoring equipment, drainage devices). Utilizes safeguards such as IV pump drug library and monitor alarms whenever available to maximize patient safety.
- Initiates emergency support measures (i.e., cardiopulmonary resuscitation, protecting patient from injury).
- Provides consultation in specialty area to all levels of health care providers throughout the hospital, affiliated schools of nursing, and the community.
- Works independently with limited direct supervision using the Nurse Practice Act, Mercy Medical Center – Des Moines policies, nursing standards, and professional judgment.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
- Possession of a current or compact state license as a registered nurse issued/defined by the State of Iowa
- Bachelor degree in nursing preferred.
- Minimum of 3-5 years current registered nurse experience preferred; preferably in the area of practice.
- Proof of completion of Dependent Adult and Child Mandatory Reporter abuse training within three (3) months of hire
- Basic Life Support (BLS) certified or obtained within three (3) months of hire. Acceptable credentialing bodies and certifications include American Heart Association Basic Life Support.

The home health registered nurse Mentor uses the nursing process (assesses, plans,
implements, evaluates) to provide patient care in the home setting and to provide field clinical training to new nursing hires to Home Health. Provides
individualized patient care for patients in all developmental stages throughout the life
span including: Adult - 18-72 years, Geriatric - 72 + years, according to

- Providing nursing intervention based on physiological needs and clinical assessment appropriate for the patient’s age and developmental stage.
- Providing and/or facilitating education according to the Plan of Care and within the level of understanding and developmental age for both the patient and his/her family.
- Providing developmental interventions appropriate to patient’s age and clinical status.
- In collaboration with the patient/family and the physician, the nurse performs and documents a thorough, timely initial assessment to determine the eligibility for home care and to identify needs and problems.
- Reassesses the patient at the minimum of every 60-62 days or when the patient demonstrates a significant change in clinical status, support system or care environment. Reviews and accurately updates the overall plan of care (CMS 485) at least every 60-62 days, incorporating all pertinent changes in the physician summary letter, concisely summarizes the significant facts of care and the progress toward achieving goals.
- Obtains physician orders and utilizes data collected during the admission assessment; agency teaching guidelines and appropriate nursing skills to implement and follow an established plan of care. Evaluate and revise the nursing and aide plans of care, when there are changes in the patient’s condition, psychosocial status, and home environment; when no progress toward stated goals is evident and when there is a change in physician orders.

Responsibilities:
- Admission assessment consistent with guidelines and interview resident/significant others to complete health history
- Utilize resources to assess residents with conditions that are new, unfamiliar or not commonly seen on unit
- Interpret overt/covert assessment data to determine when resident is at acute psychological and/or psychosocial risk then report and record the interpretation
- Inform and educate resident what care or service is to be provided
- Develop and contribute to a plan of care based on potentially existing problems, expected resident response and the medical plan of care
- Communicate the plan of care by using methods that include action plans, goals and time frames

- A current license to practice as a Registered Nurse (RN) in the State of New York
- Associate’s degree in nursing or Degree of Nursing from an accredited school
Pay Range: $40.00 - $52.81 per hour
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

This position will replace a busy, retiring physician. Has a lot of patients. Would like to have some overlap in arriving physician and departure.
Practice is at our main clinic. This group started in town and has grown over a 20 year time frame. The clinic has an attached ASC with four ORs and the cusp of enough volume to operate in the fifth one.
Two year partnership track. Compensation will be base or eat what you kill, ancillary distribution, OCP Management, and call pay (1:8 for now depending on if trauma doc joins pool). When partner will have opportunity to invest in ASC.
