PRIMARY JOB DUTIES
1. Complete the initial, comprehensive, and ongoing assessment of patient and family/caregiver needs and provide direct or supervised nursing services based on a developed plan of care.
2. Develop an individualized plan of care, in collaboration with the hospice interdisciplinary team, patient, and family, based on assessment, identification of needs, and patient and family/caregiver goals and preferences. Incorporate palliative nursing interventions.
3. Communicate with the physician (Attending and Hospice) regarding the patient’s needs, response to treatment, and changes in the patient’s condition; obtain/receive physician orders as required.
4. Work closely with other members of the IDG, and in cooperation with family/caregiver to provide holistic physical, psychosocial, emotional, and spiritual services, including bereavement support to the patient and family to achieve the highest quality of care.
5. Demonstrates positive interpersonal relations in dealing with all members of the agency.

Utilizing all aspects of the Nursing Process (assessment, planning, implementation, evaluation) with appropriate skill to effectively carry out the Plan of Care for each patient as evidenced by:
- Assessing the patients’ and family/caregivers’ physical, psychosocial, bereavement, environmental, safety, and developmental needs.
- Implementing the Hospice Plan of Care as prescribed by the physician and in conjunction with the other members of the interdisciplinary group, and revising the Hospice Plan of Care as patients’ needs change.
- Providing care to patients and families through the utilization of interventions and evaluation of outcomes of care.
- Managing all aspects of the patient’s plan of care, based on the changing needs of the patient and family/caregiver, to anticipate, prevent, treat, and manage pain and other undesirable symptoms through ongoing communication of collected data and assessment findings, and collaborating with other interdisciplinary team members.
- Provide support, instruction, and education of the patient, family and other caregivers who participate in the care of the patient in relation to needs identified on the plan of care, including, but not limited to: disease process and progression, medications, pain, symptoms associated with disease, oxygen safety, hospice philosophy, and care of the terminally ill.

Effectively and efficiently assists in the care of hospice patients and assists in the coordination of care with the interdisciplinary team.
- Maintains open communication and coordination of care, acting as a liaison between hospice medical provider, patient/family, and hospice team.
- Performs ongoing assessment of the patient based on the Plan of Care and communicates findings to the Hospice Physician and other interdisciplinary team members as appropriate.
- Performs and reports assessment findings of the patient’s pain and other undesirable symptoms to the Hospice Physician in a timely manner.
- Maintains an accurate and updated medical record, including all coordination of care notes, on-going assessments, communications with others involved in patient’s care, and any other information pertinent to the care of the patient.
- Provides supervision of patients’ assigned LPN and CNA, in accordance with Medicare guidelines and agency policy.
- Attends and participates in regularly scheduled interdisciplinary group meetings.
- Ensures that arrangements for equipment and other necessary items and services are available.
- Reports abuse and neglect in accordance with state laws and regulations as well as organization policy and procedure.

Appropriately communicates information in accordance with agency policies and procedures and discipline specific guidelines.
- Documents nursing assessments, identified problems, measurable goals of care, and limitations to provision of care, care interventions, and response to care in the electronic medical record.
- Documents all patient related visits and phone calls within the EMR and ensures documentation is completed and synchronized after visit completion.
- Completes, maintains, and submits accurate and relevant notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
- Documents all physician orders received within the EMR.
- Consults and collaborates with the hospice interdisciplinary team and others involved in the patient’s care.
- Maintains close contact with the patient’s family/caregiver to provide information, support, and continuity of care.
- Maintains collaborative relationships with long-term care facility personnel to support patient care and ensure quality symptom management.

Contributes to program effectiveness as evidenced by:
- Provides holistic, patient/family-centered care across treatment settings to ensure continuity of care and facilitate attainment of goals of care.
- Provides medication review and reconciliation of medication list within the EMR. Ensures medications necessary to ensure patient comfort are available.
- Maintains a high level of knowledge pertaining to the management of pain and symptoms in the dying process.
- Participates in the provision of 24/7 on-call nursing services.
- Assumes responsibility for personal growth and development and maintain and upgrade professional knowledge and practice skills through attendance and participation in continuing education and in-service classes and completion of required annual training.
- Actively participates in quality assessment performance improvement teams and activities.

- Scheduling self to reduce driving time and mileage and utilizing resources to prevent duplicate driving or trips by determining if others are in the area.
- Ordering only supplies that are needed and is conscientious of minimizing on-hand inventory.
- Ensuring Durable Medical Equipment (DME) is ordered in bulk to reduce delivery cost.
- Ensuring provision of medical supplies is limited to only what is needed in the home.
- Assessing and cleaning medical supply care boxes, bags, and electronic devices every month, maintaining appropriate documentation
- Practicing personal cost containment by responsible use of equipment, supplies, and resources.

JOB SPECIFICATIONS
1. Education: Graduate of NLN accredited school of nursing and current license to practice professional nursing as a Registered nurse in the state; Bachelor’s Degree preferred.

Education:

Job Summary
Reporting to the Group Supervisor - Air Operations, the Group Air Support Coordinator will play a key role in ensuring Group Air reservations are accurately booked and aligned with guest expectations and tour requirements. This position supports the Group Air teams and Supervisor by assisting with daily tasks to maintain a smooth and efficient workflow.
Primary Functions:
- Assist Group Air Specialists with queue management and securing guest seat assignments.
- Perform quality checks on Group Air reservations after seat reduction dates and as needed.
- Action airline schedule changes promptly. Assist with protecting passengers when changes no longer meet minimal connection times and/or time restrictions on the tour package booked.
- Support Quality Control Air Coordinator(s) when necessary.
- Assist with fulfillment of Group Air requests across all office locations.
- Participate in special projects and other duties as assigned by the VP Air and Logistics, Air Operations Manager, Supervisor, or Airline Contracting and Pricing Manager.
- Maintain a clean, organized, and safe work environment in accordance with company standards and policies.
Knowledge and Skills:
- Bachelor’s degree preferred (educational assistance available through approved programs).
- Minimum of 2 years of relevant experience in a related field.
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook, etc.).
- Familiarity with, or willingness to learn, air operations, quality control procedures, and reporting documentation standards.
- Strong verbal and written communication skills.
- Ability to create, compose, and edit professional written materials.
- Effective team player with the ability to work collaboratively across diverse teams and departments.

Purpose Statement
The Materials Group Leader is accountable for the suitable handling and processing of the entire supplies inventory in the warehouse during his shift. In addition to supervising and assigning tasks to workers and maintaining seamless functions in the warehouse.
Key Accountabilities and Outcomes
- Loading and unloading of freight and products
- Overseeing the handling of consignments and the supervising of the loading and unloading of the same.
- Maintaining an account of the entire supplies, raw and packaging materials inventory in the warehouse.
- Ensuring the proper labeling and tagging of goods and materials.
- Following all necessary documentation and legal procedures for inbound and outbound.
- Assigning work and functions to all warehouse workers and staff during his/her shift.
- Supervising the functions and working of the warehouse workers when the supervisor is absent.
- Adhering to the compliance of all legal, food safety, food quality and safety procedures of the company.
- Supervising proper shift allocations and ensuring efficiency of shift warehouse staff when supervisor is absent.
- Organizing and conducting necessary training activities for his/her shift staff and workers.
- Report to Supervisor any improper functioning of warehouse tools and equipment.
- Supervising the proper and smooth functioning of all activities.
- Assist in the shipping department when the Shipping Team Leader is absent.
- Supervise the deliveries on supplies to the lines according to SOP’s
- Overseeing the proper loading, unloading and handling of goods and materials.
- Validating the received materials and goods with the invoices to ascertain the precision of the goods delivered.
- Ensuring the completion of all paper work relating to the received and loading of goods.
- Ensuring the proper handling and stocking of goods, in compliance with warehousing and storage norms.
- Monitoring the warehouse records and inventory and filing reports of discrepancies in quantities and damages to the supervisor.
- Coordinate the transportation needs in Supervisor’s absence.
- Communicate with the Cold Storage facilities to coordinate inbound finished product loads when the supervisor is absent.
- Coordinating with suppliers regarding the shipments and delivery of orders.
Knowledge, Skills, and Experience
- HS degree or GED
- 3 years of experience in a food-processing, pharmaceutical or chemical manufacturing organization, preferably using computerized manufacturing equipment
- Demonstrated knowledge and application of qualifications in a pharmaceutical or food manufacturing environment
- Working knowledge of FDA regulations, Good Manufacturing Practices (GMPs), HACCP, food allergens and pest control
- Demonstrated knowledge of efficient and safe manufacturing operations to include product and associate safety, OSHA standards, product quality (success rates), cost control (minimizing downtime and waste, optimizing yield)
- Demonstrated ability to analyze and resolve problems
- Demonstrated mechanical and spatial aptitude, including the ability to disassemble and reassemble equipment
- Read, write and speak English
- Basic knowledge of SAP or similar software
- Good teamwork and leadership skills.
- Ability to effectively manage time and meet necessary targets.
- Knowledge and capacity to efficiently delegate work.
- Analytical and operational skills and the ability to adapt in a fast pace work setting.
- Proficiency with the MS Office (WORD & EXCEL)
- Previous experience using Syspro and/or riteSCAN is a plus.
COMPENSATION

The Employee Benefits Large Group Account Manager is responsible for providing prompt, accurate, first in class service to clients, providers, and partner companies. In this role you will assist the benefits consulting team in providing stellar service/administrative support to the book of business.

Essential Duties and Responsibilities:
- Provide complete support to the benefits consulting team
- Assist in servicing customers with all questions including but not limited to obtaining card or other information, claims questions/submissions, correspondence, and all related activities
- Support benefits consulting team with the sale of new business from start to finish including management of all documentation, applications and implementation
- Attend enrollment meetings and meet with clients when necessary
- Assist in management of renewal process including information gathering, processing, coordinating with providers, and accurately maintaining databases
Essential Requirements for Education and/or Experience:
- Bachelor’s Degree
- 3+ years’ experience in a consulting, brokerage or insurance company environment
- PA Life, Accident & Health Insurance license (or able to acquire within 90 days)
- Self-Funded experience strongly preferred
