This crucial role is a transitional position towards becoming a center clinical P&L leader (i.e., Senior Medical Director [SMD]) in the organization. The incumbent Center Medical Director (CMD) will leverage his/her prior leadership experience to expedite learning and delivering the ChenMed clinical model. The CMD may be simultaneously completing PCP Black Belt Training and Mastering the Medical Director Essentials within a combined and accelerated PCP and medical director training pathway.
We are an outcomes-focused, value-based organization. This position will be subject to, and have center oversight for, the following metrics/competencies that are regularly assessed to help our clinicians become successful and reach Partnership status: panel size, patient outcomes (admissions and hospital sick days), CG CAHPS and net promoter score (patient experience), HEDIS and clinical gaps closures, and medical cost effectiveness measures. Each primary care provider (PCP) will have goals for these metrics and will be expected to work towards those targets with their center and market leadership as well as their care team. Culture is also very important in the medical centers and CMDs are expected to ensure PCPs are leaders in our organization and helping champion a positive culture of love, accountability, and passion.
The CMD will demonstrate and lead the center’s providers towards:
- Accountability and oversight for outcomes: The CMD demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of at least 300 risk adjusted patients (75% partial panel). They understand that they can strongly influence the patient’s outcomes by building a trusting relationship and helping them change behaviors.
- Coaching for health: Act as a health coach, rather than just a consultant for sickness, by helping patients set short and long-term health goals, partners with the patient to work toward the goals, and frequently follows up on those goals on the path to improved health for their patients.
- Simplifying for action: Simplify and prioritize appropriately so that behavior change is more actionable, both for the patient in helping them achieve their goals, and when leading their care team towards their performance goals.
- Leadership: Lead center Super Huddle (SH) and Transforming Care Meeting (TCM) weekly care meetings, as well as support Center Manager/Center General Manager. The goal is for the CMD to increasingly assume responsibilities of the SMD to directly supervise, performance manage and train PCPs in his/her assigned center; as well as be accountable for maximizing overall clinical value while engaging physicians to improve clinical quality, efficiency, outcomes, cost effectiveness, and physician/patient satisfaction. This will partially be accomplished by leadership rounding (monthly 1:1), which initially will be jointly conducted with the market clinical leader. In addition, the goal for the CMD will be over time to increasingly manage day-to-day clinical and business operations to improve the financial performance of the center, in partnership with the Center Manager/Center General Manager.
The CMD will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The CMD will work closely with the applicable center and market leadership to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals.

The CMD will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES (for own panel initially and progressively is accountable for center):- Independently provides care and oversight of outcomes, as applicable, for patients with acute and chronic illnesses encountered in the older adult patient.
- Takes full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed; remains involved in, and responsible for, the detailed care of the patient.
- Engages with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).

- Drives a positive culture that supports a cohesive team approach, drives excellent results and relationships, and promotes "best place to work" culture measured by employee engagement scores.
- Upon successfully making progress on attaining consistent primary care performance trending towards that of a ChenMed PCP Partner, maintaining trajectory of medical director training, the CMD will be asked to increasingly assume the role and responsibilities of a Senior Medical Director, which may include:
- Managing, mentoring and coaching PCPs in his/her assigned center to deliver outstanding clinical outcomes, including sampling other PCP’s daily huddles within their center
- Leadership rounding with the PCPs (reduced involvement of market clinical leader)

Regional Medical Director, NJ
Middlesex, Monmouth, and Ocean Counties
JOB SUMMARY:
The Regional Medical Director (RMD) provides strategic clinical leadership and oversight for multiple BoldAge PACE sites across a geographic area that can include several states, maintaining active medical licensure in each applicable state. Working in partnership with the Chief Clinical Officer, the RMD ensures BoldAge Pace consistently delivers exceptional participant outcomes, regulatory compliance and operational efficiency. The Regional Medical Director role combines direct participant care with a local panel of patients at the provider’s home office with broader regional leadership responsibilities, including the ability to provide coverage for other regional locations in the event of an emergency or when state regulations require active patient practice. In addition to maintaining a clinical panel, the Regional Medical Director provides administrative leadership by overseeing the Regional Quality Assessment and Performance Improvement (QAPI) program, collaborating with national clinical leadership to develop and maintain medical policies, guidelines, and standing order protocols, and ensuring the efficient use of resources to achieve program goals. This position provides direct guidance and supervision to primary care physicians, advanced practice providers and other providers as assigned. By fostering collaboration across physicians, advanced practice providers, interdisciplinary teams, and community providers the RMD advances BoldAge’s mission to deliver participant-centered, high-quality care while positioning BoldAge PACE for growth and innovation.
ESSESNTIAL DUTIES AND RESPONSIBILITIES:
Clinical Leadership & Oversight
- Carry a panel of participants, providing direct care in region (home and facility), including, but not limited to, taking on-call shifts), providing emergency coverage in the event of provider absence and interdisciplinary team (IDT)participation.
- Oversee and evaluate participant care delivery 24/7 through established clinical leadership structures.
- Ensure timely completion of participant assessments and individualized care plans in compliance with PACE requirements.
- Collaborate with site Medical Directors and interdisciplinary teams to ensure participant needs are met and outcomes optimized.
- Collaborate and participate in national Clinical Leadership Meetings (CCO), acting as the regional representative and expert. Provide regional expertise for national clinical and operational development and represent regional needs at the national level.
- Participating in OPPE/FPPE with national team and providing feedback, coaching and mentoring as needed to regions providers.
- Participates in national and regional interdisciplinary committees and teams as necessary.
- Serves as a role model in delivering high-quality, participant-centered care, ensuring alignment with best practices and BoldAge’s values.
Quality & Regulatory Compliance
- Lead regional QAPI activities, analyze performance data, and implement improvement plans.
- Maintain understanding of and compliance with national CMS, state and regional, licensing requirements, and accreditation standards.
- Oversee regional infection control programs, OSHA safety compliance, and public health guidelines.
- Performs regular chart audits and provides constructive feedback related to charting, coding and opportunities.
Staff Supervision & Development
- Recruit, orient, supervise, and evaluate physicians, advanced practice providers, and other providers as assigned.
- Provide coaching, mentorship, and performance feedback.
- Maintain in coordination with national leadership accurate job descriptions (HR) and policies, guidelines and standing orders (VP of Clinica Ops), aligned with regional regulations and BoldAgePACE policies.
- Acts as collaborating physician to advanced practice providers as necessary and appropriate by local state regulations.
Operational & Financial Oversight
- Establish and oversee effective 24/7 on-call coverage systems in collaboration with site leaders.
- Monitor budgets, KPIs, and financial reports to ensure efficient resource utilization.
- Support accurate coding and documentation in collaboration with the coding team.
Community & External Engagement
- Represent BoldAgePACE at PACE associations, industry events, and with community providers.
- Maintain professional development through participation in training, continuing education, and networking.
Other Responsibilities
- Maintain participant confidentiality and comply with HIPAA standards.
- Performs all other duties as assigned to meet organizational needs.
EXPERIENCE EDUCATION AND CERTIFICATIONS:
- M.D. or D.O. with current medical license and the ability to obtain and maintain active licensure in all states in which BoldAgePACE operates. Must also hold current DEA registration and have the ability to obtain and maintain staff privileges at PACE-contracted agencies. Board certification in Internal Medicine or Family Practice required; advanced certification in Geriatrics preferred.
- Demonstrated experience in a managed care environment, collaborating with peers and other healthcare providers to address utilization management, quality management, performance improvement, pharmacy and therapeutics, peer review, credentialing, and physician leadership matters.
- Minimum of 3 years in a lead administrative role with responsibility for clinical oversight across multiple locations or programs.
- A minimum of 3 years’ experience working with frail and elderly populations in acute care, primary care, long-term care, or community-based settings.

- Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
- Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Numerous trade spaces compatible with existing system capabilities are explored and possible solutions discussed before a final answer is determined.
An Embedded Developer then begins decomposing the individual specifications to the necessary hardware instructions and desired system configurations.
The coding effort starts with special attention given to unique system parameters critical to ensuring the radar is controlled within strict operating parameters, operates correctly and provides the expected data for collection.
The testing aspect of the development effort includes initial measurements of the generated command signals using various test equipment and subsequent follow-on verification of a myriad of system parameters that could cause system or subsystem faults resulting in damage to the radar.
The waveform is then put through a series of system level tests to verify the waveform provides exact control timing and data collection instructions to all radars in the system.
Data is collected and analyzed using locally developed tools to verify the radar systems are operating correctly, individually and collectively, and producing the intended results.
Test Equipment, Hardware and Software used: Oscilloscope, Spectrum Analyzer, Single Board Computer (SBC), VxWorks OS, Local software created on Visual Studio (C#).
In an analogous position, the Embedded Software Developer would create code commanding sensors and collecting sensor data for later analysis.
Remoting a Digital Quadrature Detector (DQD)
- The DQD consists of a Single Board Computer (SBC) and a sampling card used to sample radar data at very high rates which is then sorted and collected for processing.
The DQD currently resides in a VME chassis shared with other hardware and this effort removed the DQD from the VME chassis while providing the same capabilities.
The primary task moved messaging traffic between the chassis and the DQD into Ethernet to allow the DQD to be remoted and not reliant upon the VME chassis and hardware contained therein.
After discussing the task with teammates and stakeholders the Embedded Developer created a set of requirements which specified, and added enhanced capabilities to, the data flow and messaging required to meet overall system specifications.
The development created new messages and data injection into the framework to allow remote DQD operation.
Continuous integration and testing was performed to ensure incremental success during development.
Test Equipment, Hardware and Software used: Oscilloscope, Spectrum Analyzer, Wireshark, Single Board Computer (SBC), VxWorks OS, Local software created on Visual Studio (C#).
In an analogous position, the Embedded Software Developer would take an existing device and remote it within a system to perform the original functionality without influencing the overall system's operation.
Control of an IR camera
- This task required an IR camera is to be installed on a system.
Most of these cameras usually have proprietary commands and controls which need to be presented to users for proper operation of the camera.
After discussing the IR camera needs of the group with teammates and stakeholders, the Embedded Developer created a set of requirements which specified the necessary functions to control the camera.
The Developer used this knowledge to conduct market research to find a suitable host device and determine an appropriate software framework.
A Panel PC with Windows IoT OS was selected to host the applications controlling the camera and Visual Studio (C#) and .NET selected as the software framework.
The development started with reading vendor information to understand the commands and controls of the cameras and then bench testing with emulators to verify correct understanding of the instruction set.
Software development required defining numerous threads and the software architecture to support the low level control requirements.
Screen layout and features (UI/UX) were developed to provide a link between the user and camera to provide an easy-to-use interface.
Wireshark was used to debug and verify messaging traffic.
Profiling tools were needed to identify and correct "quirky" camera interfaces and responsiveness.
Security requirements necessitated the Panel PC be configured to boot from a locked disk.
After booting, a script is run to connect to a server and download applications for camera operation and to start automatically.
Test Equipment, Hardware and Software used: Wireshark, Emulators, Panel PC, Visual Studio (C#), .NET.
In an analogous position, the Embedded Software Developer would be using a new device, understanding its command/controls, implementing a user interface to it, and providing a secure boot and remote application loading capability.
