Community Health Care Manager - Evans
Community Health Care Managers (CHCM's) work with members enrolled in Medicaid to coordinate care and to connect them with essential community resources through partnerships with local physical and behavioral health providers. CHCM's aim to enhance individual health outcomes and the overall healthcare experience by identifying each person's health and wellness goals and improving their ability to navigate the healthcare system while also working to reduce high healthcare utilization costs. The CHCM reports to the CHCM Supervisor.

Job Responsibilities/ Essential Functions:
* Case Management and Care Coordination of assigned members
* Monitor members according to guidelines set by NCHA Policy and Procedure under the Care Management guidelines.
* Participate in home visits, telephone contacts, office/hospital visits, and other means of connecting with members to reach identified goals.
* Perform needs assessments and identify risk factors to develop person centered care plans.
* Assist members in scheduling appointments, follow-up care, referrals, medication refills, etc..
* Attend member appointments with providers and community resources as needed.
* Participate in health promotion and health education activities for members as identified in their health care plan.
* Facilitate collaboration, communication, and coordination among all members of an individual's multidisciplinary healthcare team, while ensuring consistent and ongoing correspondence.
* Optimize member and family self-management through education, community resources, and support.
* Review appropriate cost-effective care and decrease duplication of services for members.
* Enter all documentation, contacts, and assessments into multiple databases with a high level of accuracy.

Principal Accountabilities
- This role involves physical activity in a food processing environment, which generally requires a moderate amount of exertion on a fairly regular basis-involving bending, stooping, squatting, twisting, reaching, working on irregular surfaces, occasional lifting of objects weighing more than 50 pounds and frequent lifting of 10-25 pounds. Job Description
- Report directly to Service Manager for deployment from store to in the field equipment repairs on customer or company properties.
- Maintain, repair and overhaul farm machinery and vehicles, such as tractors, harvesters, construction equipment, hay equipment, etc.
- Examine and listen to equipment, read inspection reports, and confer with customers to locate and diagnose malfunctions.
- Ability to utilize electronic service tools to allow for proper troubleshooting
- Test and replace electrical components and wiring, using test meters, soldering equipment and hand tools
- Understanding of hydraulic systems for proper troubleshooting.
- Repair or replace defective parts using hand tools, lathes, welding equipment, grinders or saws.
- Ensure proper documentation on all repairs- most importantly, on warranty jobs per manufacturer requirements.
- Tune or overhaul engines.

The EM maintains regional facilities in compliance with all applicable federal, state, and local environmental laws, standards, and permits; and has responsibility for maintaining facility compliance with applicable environmental permit conditions.
This position has direct responsibility for conducting on-site fieldwork associated with sampling, monitoring, observing, and recording environmental data; and preparing reports, records, and plans required by regulatory permit, plan, or agreement. The EM assists in the overall planning, design, installation, and operation of pollution control equipment and associated metering and/or measuring devices. The Republic Services Polymer Center is the nation's first integrated plastics recycling facility. This innovative site processes rigid plastics from our recycling centers, producing recycled materials that promote true bottle-to-bottle circularity. We also formed Blue Polymers, a joint venture with Ravago, to develop facilities that will further process plastic material from our Polymer Centers to help meet the growing demand for sustainable packaging. We are building a network of Polymer Centers and Blue Polymer facilities across North America.
We continue to advance decarbonization at our landfills. As demand for renewable energy continues to grow, we have 70 landfill gas-to-energy projects in operation and plan to expand our portfolio to 115 projects by 2028.

PRINCIPAL RESPONSIBILITIES:
- Maintains advanced knowledge to diagnose and repair mechanical problems on heavy equipment.
- Performs repair or preventative maintenance on electrical, diesel and gasoline vehicles, including compactor, excavator, wheel loader, and other vehicles to maximize safe and productive operations. Typical repairs and maintenance would include work on engines, transmissions, chassis, drivelines and body components, hydraulic, brake, electrical and suspension systems, wheels, tires and undercarriages.
- Reads and interprets equipment condition reports and/or repair orders, communicate with supervisor and/or operator for clarification of defects and discuss preventive techniques with operators to minimize future repairs.
- Completes paperwork associated with repairing equipment, documenting parts usage and accounting for applied times (i.e., work order times compared to time card times).
- Completes daily consumables report to record hours, fuel and oil usage, and provide information for input into Dossier, Republic Services Fleet Management Software system.
- Collects and interprets analyses from oil sampling, on-board computer, and fleet management software.
- Follows all safety policies and procedures.
- Performs overhauls on gas or diesel engines.
- Advanced knowledge in air brakes, tires, hydraulics, advanced electrical, advanced drive train knowledge, suspension, steering and wheel-end components training as evidenced by ASE Certification.
- At least 1 year of experience as a technician working on heavy equipment.
- Minimum of 1 year of prior experience as a technician working on heavy equipment, demonstrating the ability to service increasingly more challenging maintenance repairs.Mechanical Trade School certificate or degree
- Certificate in electrical diagnostics.
- Applicable ASE certifications.
- Working knowledge of Tier 4 emission systems.
