Job Summary

As a provider in our Atlas Medical Team, you will be entrusted with delivering mobile on-site primary care services to our vulnerable patient population residing in assisted living and other senior communities. This role demands autonomous management of complex healthcare scenarios, including multiple co-morbid conditions. Your expertise will extend beyond medical treatment, as you educate patients and their families on self-care, disease management, and end-of-life care. Collaboration will be a daily aspect of your work, liaising closely with facility staff and Atlas Patient Care support teams. All activities will be guided by quality measures provided by Atlas, emphasizing our commitment to high standards of care.
