Job Summary
As a provider in our Atlas Medical Team, you will be entrusted with delivering mobile on-site primary care services to our vulnerable patient population residing in assisted living and other senior communities. This role demands autonomous management of complex healthcare scenarios, including multiple co-morbid conditions. Your expertise will extend beyond medical treatment, as you educate patients and their families on self-care, disease management, and end-of-life care. Collaboration will be a daily aspect of your work, liaising closely with facility staff and Atlas Patient Care support teams. Excellent communication and interpersonal skills, with the ability to collaborate effectively with a diverse team of healthcare professionals.

9.
Proven experience as a system compliance quality leader.

PROSCI Change Management Certificate or equivalent.
