Reviews plan of care and orders with primary physician, written and evidenced as read-back, completed immediately and transmitted within time frame established by policy.

Patient Care



  • Deliver physical therapy interventions as directed by a qualified Physical Therapist and in accordance with the physician’s plan of care and the state Physical Therapy Practice Act.
  • Treat patients to help relieve pain, maintain function, and support maximum safe performance.
  • Lead patients through active and passive therapeutic exercises, muscle re-education, gait training, functional mobility activities, transfer training, and prosthetic training.
  • Utilize physical agents such as heat, cold, water, light, electricity, ultrasound, and massage in alignment with state practice guidelines.
  • Observe and report patient responses, progress, or changes in condition to the Physical Therapist and appropriate agency personnel.
  • Instruct patients in the safe use and care of mobility and supportive devices, including wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.

Communication & Documentation



  • Document clinical notes and updates in a timely and accurate manner; ensure all documentation is incorporated into the patient’s clinical record as required.
  • Participate in interdisciplinary care discussions and collaborate with the hospice care team to support coordinated service delivery.

  • Attend home care team meetings, in-service programs, and interdisciplinary group (IDG) meetings.

Job Summary

Essential Functions


Patient Care



  • Deliver physical therapy interventions as directed by a qualified Physical Therapist and in accordance with the physician’s plan of care and the state Physical Therapy Practice Act.
  • Treat patients to help relieve pain, maintain function, and support maximum safe performance.
  • Lead patients through active and passive therapeutic exercises, muscle re-education, gait training, functional mobility activities, transfer training, and prosthetic training.
  • Utilize physical agents such as heat, cold, water, light, electricity, ultrasound, and massage in alignment with state practice guidelines.
  • Observe and report patient responses, progress, or changes in condition to the Physical Therapist and appropriate agency personnel.
  • Instruct patients in the safe use and care of mobility and supportive devices, including wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.

Communication & Documentation



  • Document clinical notes and updates in a timely and accurate manner; ensure all documentation is incorporated into the patient’s clinical record as required.
  • Participate in interdisciplinary care discussions and collaborate with the hospice care team to support coordinated service delivery.

Additional Responsibilities



  • Attend home care team meetings, in-service programs, and interdisciplinary group (IDG) meetings.
  • Participate in quality assessment and performance improvement activities.
  • Perform additional duties as assigned to support high-quality patient care.

Qualifications



  • Graduate of an accredited Physical Therapy Assistant program with an associate degree in science or applied science (approved by APTA).
  • Meets hospice personnel qualifications per §418.114(b)(8).
  • Current PTA certification or licensure as required by state law.
  • Strong verbal, written, and organizational skills.
  • Current CPR certification.
  • Licensed driver with a reliable, insured automobile that meets organizational requirements.

What You Will Do 

  • Monitor and follow patient progress and report health changes to supervising registered professional nurse through detailed observations, interviews, and physical/psychosocial care within LPN scope of practice. 

  • Deliver personalized nursing and care management by constantly evaluating the evolving patient needs and adjusting the plan of care.  

  • Carry out quality-driven and care coordination by communicating within the VNS Health interdisciplinary team and across providers and other resources to deliver the plan of care effectively and efficiently.   

  • Practice in the community, traveling to patients’ homes or care facilities, using effective time management and agility to manage weekly caseload and other duties.  

  • Use agility and empathy to deliver continuity of patient care including coordinating supplies and providing education.  

  • Use technology, including a tablet computer, phone, and other systems to coordinate and maintain accurate and up-to-date electronic medical records (EMRs) for patients in the community.   

  • Provide empowering education and support to patients and their families to help them understand and navigate end of life treatment.   


Qualifications

Licenses and Certifications:

  • License and current registration to practice as an LPN in New York State required

  • Valid driver's license may be required as determined by operational/regional needs


Education:

  • High School Diploma or completion of LPN Program required


Work Experience:

  • Minimum of one year work experience as an LPN preferred

  • Bilingual skills may be required as determined by operational needs

  • Home care/hospice experience preferred

Pay Range per Visit:
If you are applying to the per diem per visit version of this job, the hiring range is as follows: $50.68- $54.07 per visit.

Overview

Provides physical therapy interventions to designated patients under the supervision of a Physical Therapist (PT). Assists in monitoring patient progress through observations and follows up with supervising therapist. Works under PT supervision as required under NY State Education law.

• Follows established physical therapy plan of care, including physical therapy interventions and patient/caregiver education. Modifies specific treatment procedures in accordance with PT plan of care and Physical Therapy Assistant (“PTA”) scope of practice.
• Documents all care provided and maintain records per VNS Health policy and standards.
• Assists in monitoring patient progress through observation and follows up with Supervising Therapist in accordance with VNSNY policy.
• Complies with scope of practice for PTA, including a co-visit with Physical Therapist on all initial PTA visits and keeps PT informed. PT will re-assess patient every 6 visits or 30 days (whichever comes first).
• Manages assigned caseload; provides physical therapy services to patients as directed per PT plan of care.
• Participates in VNS Health meetings, including but not limited to, interdisciplinary team meetings, in-service and training programs, rehab services meetings and caseload reviews.
• Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient.
• Assists patients with therapeutic interventions and activities of daily living, which may require positioning, moving, transferring and lifting patients of varying body sizes and physical conditions, with and without assistance and as appropriate, instructing, demonstrating and verifying capabilities of family members and/or paraprofessionals. Ability to use and assist patient with use durable medical equipment as a component of the plan of care, including but not limited to wheelchairs, patient transfer lifts, ambulatory devices, slings, splints, orthoses, prostheses, and exercise/strengthening equipment.
• Provides care in accordance with Agency policies, practices, procedures and standards, including professional practice standards of care, which may require standing, stooping, sitting, crouching, twisting, reaching, bending and stretching to deliver patient care.
• Transports and utilizes required technology devices (tablet), infection control materials and supplies using VNS Health designated/supplied carrying case (backpack) weighing approximately 15 – 20 lbs to and from patient homes/care facilities, VNS Health offices and therapists’ home.

• Participates in special projects and performs other duties as assigned.

Education:
Associate's Degree as Physical Therapist Assistant from a New York State approved program or whose educational credentials have been approved equivalent by New York State required
