The Health and Wellness Manager oversees provision of health and wellness services including:

- Active in Center/Company Leadership program.
- Conducts center needs health/wellness assessments; plans and coordinates services and programs that respond to identified student needs.
- Provides health and wellness assessments and screening, nursing interventions, emergency care, education, and referral services in conjunction with health and wellness staff and health subcontractors.
- Performs duties as the H&W Co-Disability Coordinator to ensure services are provided to students with disabilities.
- Serves as member of center’s Sexual Assault Prevention and Response team.
- Serves as the center File Review Coordinator.
- Orders and maintains records on dispensing, inventory, and disposal of medical and dental supplies and pharmaceuticals.
- Initiates worker’s compensation documentation and maintains secondary OSHA logs. Performs follow-up surveillance of worker’s compensation cases.
- Performs physical and emotional wellness assessments. Establishes individualized student health and wellness plans according to protocol outlined in the center’s Health Standing Orders
- Works closely with counselors, residential staff, and medical trade instructors in providing wellness education to students.
- Responsible for updating center Health & Wellness Standard Operating Procedures.
- Works with center management staff to develop and implement center plans as needed.
- Works with center management to employ or subcontract qualified health care providers.
- Prepares and conducts health and wellness staff performance evaluations. Responsible for continuous quality improvement by seeking feedback from students, employing mechanisms to document quality care provided and quality improvement activities,
- On-call support required. Performs other duties as assigned.

Requirements:

Qualifications:Minimum: Minimum of Associates Degree in Nursing. Must have active, unrestricted Registered Nurse license in the state where the center is located.Preferred: Three years’ experience as a practicing nurse, one of which was in a lead or supervisory capacity.Knowledge: Strong working knowledge of nursing techniques/procedures and time management. We have an exciting opportunity for a full time Occupational Health Nurse to join the Human Resources Department. The Occupational Health Nurse promotes and protects the health, wellness, and safety of employees by preventing work related injuries and illnesses and supporting overall wellness in the workplace. The Occupational Health Nurse focuses on employee health, workplace injury prevention, exposure management, and return to work coordination to ensure a safe environment while ensuring compliance with regulatory requirements. The Occupational Health Nurse collaborates with employees and leaders to foster a safe, healthy, and productive work environment. Demonstrates an understanding of, and embraces, the mission statement of Christian Health.


Competencies:

  • Serve as a resource for employees regarding occupational health concerns and wellness initiatives.
  • Provide employee health services to employees, including assessment and management of work-related injuries and illnesses.
  • Conducts pre-employment physical exam and drug screen with new employee prior to start date and reviews with physician.
  • Monitors and follows up with employees who are out on extended leave.
  • Facilitate return to work certificates, work restrictions, and accommodations.
  • Reviews return to work certificates and notify employee’s leader and Human Resources of employee release to return to work.
  • Maintains records of Physician Return to work certificates.
  • Coordinate post-exposure evaluation and follow-up (e.g., bloodborne pathogens, communicable diseases).
  • Manage employee health programs such as immunizations, TB screening, and COVID Testing.
  • Notifies employees of mantoux test due date.
  • Collaborate with Infection Prevention, Human Resources, and clinical leadership to support outbreak response and staff safety.
  • Maintain employee health files with accurate employee health records in compliance with OSHA, CDC, TJC, and state regulations.
  • Participate in safety initiatives, injury prevention programs, and staff education.
  • Support regulatory surveys and audits related to employee health and workplace safety.
  • Administers flu shots to and receives consent forms from employees and volunteers.
  • Administers fit testing for employees and contracted employees.
  • Ensures compliance with the OSHA Blood Borne Pathogen requirements.
  • Examines employees who are injured or become ill while on the job (i.e. URI, GI, headache, muscle aches and pains).
  • Administers to employees Mantoux test, Hepatitis B Vaccine and Rubella titer if/when required (and provides follow-up based on result).
  • Oversees and sets up employees for lab work.
  • Works in conjunction with Leaders, Human Resources, and Occupational Health Clinic to establish modified work assignments for injured employees when appropriate.
  • Keeps up to date with legislative changes and new requirements regarding Employee Heath Regulations with Substance Abuse program.
  • Administers Mantoux test to all volunteers, outside contractors and day care center staff.
  • Orders medical supplies/vaccines.
  • Tracks employees who are out on Worker's Compensation.
  • For work related injuries, communicate with Occupational Health Clinic and employee's personal physicians if/when required.
  • Represents Employees Health at the following monthly meetings: Infection Control, Safety, Quality Review, Human Resources, and Insurance Provider NIM.
  • Presents Employee Health information at general orientation.
  • Completes the following monthly reports: Employee Health, Infection Control, Safety, and Payment for Drug Screens and Lab Work as well as chest x-rays.
  • Supports and is an active member of a Service Excellence Team, fulfilling the required obligations of that team.
  • Completes monthly and quarterly, Quarterly Review Report.
  • Reviews Employee Health Policies and Procedures and makes recommendations when changes are required.
  • Performs other related duties as required.


Qualifications:

  • Minimum of 5 years of experience required with previous training in employee health, long-term care, infection prevention, strongly preferred.
  • Knowledge of OSHA, CDC, TJC and state health regulations applicable to long-term care required.
  • Must possess a current, unencumbered license to practice as a RN in New Jersey.


Schedule: 8:30am-4:30pm Monday - Friday. Must be available to work beyond normal working hours, work on weekends and holidays, and in other positions temporarily, when necessary.

 As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. As a Home Health LPN, you will:

  • Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.
  • Implement individualized care plans in collaboration with the patient, family, and healthcare team.
  • Educate patients and their families on disease management, treatment options, and self-care techniques.
  • Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.
  • Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes. Participate in care coordination activities and discharge planning.

  • Handles reviews of prior claims to address potential impact on current case management and eligibility.

  • Ensures case management processes follow organization and regulatory requirements.

St. David’s North Austin Medical Center, part of St. David’s HealthCare, is a full-service medical center providing comprehensive medical care at two locations – St. David’s North Austin Medical Center and St. David’s Surgical Hospital.
St. David’s North Austin Medical Center is a 460+ bed, multi-specialty, acute care hospital. It provides advanced women’s health services, including maternity and newborn care with Level I, II, and III neonatal intensive care units (NICUs) at the nearby St. David’s Women’s Center of Texas. The hospital also offers minimally invasive gynecological surgery and comprehensive breast imaging through Solis Mammography, which is located on-site. Other services include a 24-hour emergency department, pediatric emergency care, and intensive care at St. David’s Children’s Hospital. The hospital also features the Texas Institute for Robotic Surgery, bariatric (weight loss) surgery, a Heart and Vascular Center, neurology and neurosurgery, a kidney transplant program, a Level IV Epilepsy Center, as well as inpatient and outpatient surgery and acute rehabilitation services for both inpatients and outpatients.

St. David’s Women’s Center of Texas, located at St. David's North Austin Medical Center, is recognized by the State of Texas as a Level IV Maternal Facility. This means it provides the highest level of care for pregnant and postpartum patients, especially those with serious or complex medical, surgical, or pregnancy-related conditions, including those at high risk for maternal morbidity or mortality. The center includes a 95+ bed Level III Neonatal Intensive Care Unit (NICU) with 30+ private rooms, 10+ rooms for very small babies, and 10+ beds for babies needing neurological care. It also has 35+ labor, delivery, and recovery rooms, including Central Texas’ first low-intervention birthing suites with tubs. There are 95+ postpartum beds, 20+ antepartum beds, and Cesarean section surgical suites. The facility offers 24/7 neonatology coverage, OB/GYN hospitalists, maternal-fetal medicine specialists, lactation consultants, and a midwifery program. It is part of the National Institute of Child Health and Human Development Maternal-Fetal Medicine Units Network and TexasAIM, and it also provides a high-risk pregnancy navigation and support program.

St. David's Surgical Hospital is part of St. David’s North Austin Medical Center and is the first Level 4 Epilepsy Center in Central Texas for both adults and children. It has its own 24-hour emergency department, a medical-surgical inpatient unit, and a pediatric intensive care unit (PICU) for patients ages 17 and under. The hospital is staffed with pediatric emergency physicians, pediatric hospitalists, intensivists, and other specialists who are specially trained to care for children. It also includes certified child-life specialists who help kids feel more comfortable during their hospital stay. St. David’s Children’s Hospital offers many pediatric services, such as pediatric surgery, pediatric radiology (including imaging and interventional procedures), and pediatric anesthesia for surgical support. It also has a wide range of pediatric specialists available for consultation and treatment, including general surgery, ENT (ear, nose, and throat), craniofacial and plastic surgery, neurology, neurosurgery, urology, and cardiology, among others.

Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love.

As a Home Health LPN, you will:

  • Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.
  • Implement individualized care plans in collaboration with the patient, family, and healthcare team.
  • Educate patients and their families on disease management, treatment options, and self-care techniques.
  • Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.
  • Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes. Participate in care coordination activities and discharge planning.

Required Qualifications:
  • Current nursing license in the practicing state
  • Valid drivers license, auto insurance and reliable transportation
  • Current CPR certification
  • Two years experience as an LPN/LVN in a clinical setting
Preferred Qualifications:

Driving Statement:

This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance.
