Summary:

The Specialty Clinical Pharmacist is responsible and accountable for the provision of safe, effective, and prompt medication therapy. Specialty Clinical Pharmacists provide proficient direct patient-centered care in a decentralized practice setting. In this clinical role, Specialty Clinical Pharmacists participate in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to the patients in their assigned areas. Pharmaceutical care services include but are not limited to assessing patient needs, incorporating age and disease-specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Specialty Clinical Pharmacists serve as departmental resources and liaisons to other departments, or external groups. They also conduct clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. Specialty Clinical Pharmacists provide medication and practice-related education/training and actively serve as preceptors for Doctor of Pharmacy students and pharmacy residents. The Specialty Clinical Pharmacists will participate in the quality management program with the goal of improving services by monitoring processes, analyzing data, implementing interventions to improve, and evaluating the effectiveness of those interventions.

The Specialty Clinical Pharmacists will be responsible for the clinical management of patients on a variety of specialty therapies for medical conditions including but not limited to Hepatitis C, inflammatory conditions (gastroenterology, dermatology, asthma, and allergy), pulmonary conditions (idiopathic pulmonary fibrosis, cystic fibrosis), neurology (multiple sclerosis, migraine headache), dyslipidemia, osteoporosis, and oncology.

Job Requirements:

Education/Skills

  • Doctor of Pharmacy degree from an ACPE accredited school of Pharmacy is required.
  • Must accrue American Society of Health-System Pharmacists (ASHP) Residency Preceptor status within 18 months of hire.

Experience

  • PGY1 residency or 5 years of clinical practice (community, ambulatory care, PBM, or hospital) required.
  • PGY2 residency preferred.
  • Specialty pharmacy experience preferred.
  • EPIC suite products experience preferred (EMR, Compass Rose, Willow Ambulatory)

Position Summary: 

  • The System Antimicrobial Stewardship – Infectious Diseases Pharmacist (ASP-ID) is a clinical pharmacy specialist and leader responsible for the development, implementation, and documentation of the antimicrobial stewardship program (ASP) across Rush University Health System (Rush). This includes providing multidisciplinary education on safe antimicrobial use, creating and implementing guidelines, collaborating with multidisciplinary clinicians, administrators, and support staff across the system, and analyzing metrics for antimicrobial utilization to satisfy new and existing CDC and Joint Commission standards. The ASP-ID pharmacist will be responsible for developing and coordinating innovative clinical pharmacy programs across RUSH sites conducting scholarly and research activities and interdisciplinary teaching. To support the effectiveness of ASP initiatives, the ASP-ID pharmacist will maintain on-site service coverage rounding with inpatient ID team(s). The ASP-ID pharmacist will serve as a key faculty member in postgraduate pharmacy training programs and affiliated colleges of pharmacy. This position will focus their efforts on ASP support at Rush Oak Park hospital and Rush Copley Medical Center. 

Essential Clinical Duties and Responsibilities of the ASP-ID Pharmacist: 

  • Develops metrics for performance and maintenance of ASP. 
  • Participates in the provision of pharmaceutical patient care through ASP staffing and clinical rounds. 
  • Participates in restricted/protected antimicrobial approval.

* a. Participate as a member of WHS Pharmacy and Therapeutics (P&T) Committee.

* c. Participate on committees, teams, or task forces relating to area of preceptors individual practice.

* e. Attend Directors meetings (e.g. Pharmacy Directors, Partners).
* f. Assist with coordinating clinical pharmacy activities, schedules, programs and projects.

Required Minimum Education:

The Pharmacy Technician Medication Coordinator position at Genoa is an essential role that impacts the lives of an underserved population that needs a voice. This onsite position is a liaison between a partner clinic and a Genoa mental health specialty pharmacy.


Primary responsibilities include building strong relationships with the clinic partners and communicating with consumers to assist them with their medication plan, insurance and the full services of the Genoa pharmacy. A key function will be promoting Genoa services to obtain new consumers and increasing the number of consumers Genoa serves.

This clinic is located inside the doors of Cascadia Health either location: Woodland Park - 10373 NE Hancock St, Portland, OR 97220 or Plaza Health Center - 4212 SE Division St, Portland, OR 97206
The hours for this position are: Monday-Friday: 8:30am-5pm, Closed for Lunch: 12:30pm-1pm


Primary Responsibilities:

  • Communicates with all consumers of the mental health center regarding the medication services Genoa provides
  • Recruits and enrolls consumers utilizing enrollment forms and copy the consumer's insurance card
  • Facilitates the collection of prescriptions to be faxed to the pharmacy for dispensing
  • Ensures all consumer insurance information is up to date in the Pharmacy system and is properly charged for the medications dispensed
  • Assists consumers in contacting the pharmacist regarding all their medication questions and ensure a prompt response to their questions
  • Assists clinic staff and pharmacy with prior authorizations and Patient Assistance programs as needed
  • Monitors compliance by contacting the consumer at least monthly to ensure compliance and determine refill needs as applicable
  • Prescription refill management by utilizing the no refill report and contacting the appropriate prescriber to ensure timely refills are completed and dispensed to the consumer
  • Medication delivery when needed and appropriate. {Only applicable in states that are allowed.}
  • Checks for expiration dates (both on consumer's medications as well as house account products/standing order meds.)
  • Ensures that medications are stored properly (refrigerated drugs are kept in the refrigerator, pen-tips are not stored on insulin pens, etc.)
  • Checks for discrepancies (dose changes, discontinued medications, etc.)
  • All areas of the job description are subject to state regulations and allowances {CMC will receive a state specific document that shares state specific regulations and allowances.}
