Role Description

Essential Duties:

  • Manage and ensure compliance with the Company’s Safety, Security, Health and Environmental Programs.

Position Overview


Reliant Net Lease is seeking a highly organized and proactive Transaction Assistant to support the Real Estate Transaction Team in coordinating acquisition and disposition activities across the company’s growing single tenant net lease portfolio. This role will play an important operational and administrative support function for the Real Estate Transaction Team and broader business teams, helping ensure due diligence, documentation, and closing processes run efficiently.

Key Responsibilities

Job Summary and Qualifications

Medical Assistant Certified will perform diversified clinical and administrative support duties. The Medical Assistant will utilize skills in performing technical skills and support skills for the nursing staff in order to care for the patient in an effective and efficient manner. 

 What you will do in this role:

  • Performs routine duties in caring for outpatient clinic patients as directed by the Registered Nurse and is able to demonstrate appropriate skills and/or competencies
  • Screens and collects data from patients, including patient histories. Assists with patient discharge preparations as instructed by RN. Reports and documents pertinent information such as vital signs, weights.
  • Calls and/or faxes prescription refills to the pharmacy following office protocols and/or after approval from physician. Retrieves medication and supplies from hospital departments and transports to the clinic as directed by the RN.
  • Maintains confidentiality of patient information and records at all times.
  • Performs Electrocardiograms, Non-invasive hemodynamic testing, vital signs, weights, phlebotomy, removes sutures/staples, simple dressing changes (clean and dry, sterile), assist with sterile procedures, simple, non-sterile dressings, and removes peripheral IV’s, records response to treatments and reports pertinent information to appropriate RN. Obtains and labels specimens from wounds, Guiac stool specimens, Urine and sputum collection in cups, blood collection and transports specimens to lab as directed.
  • Provides assistance to other nursing personnel as needed. Assists with the orientation of new personnel as appropriate. Runs errands, stocks examination rooms, maintains cleanliness of patient rooms, and triage area on a daily basis. Performs other duties as delegated by the RN

What we're looking for
Applicants should have a current state LCSW, LPC, LMFT or similar license and possess a Master's degree from an accredited school. Additional requirements include:

How you'll contribute  

A Therapist who excels in this role: 

  • Conducts individual and group therapy sessions to educate patients regarding psychological, emotional, or substance abuse problems
  • Displays active involvement in treatment planning process
  • Provides family session counseling to all patients to ensure appropriate communication and involvement of family members and support groups
  • Actively communicates with clients, families, and outside referral sources
  • Demonstrates proactive communication with those involved with the patient's treatment
  • Works well with the interdisciplinary team including physicians, utilization review and nursing staff members
  • Demonstrates active communication with team members
  • Effectively communicates to UR on treatment and LOS issues
  • Completes appropriate paperwork for clients in a timely, accurate and complete manner
  • Demonstrates appropriate crisis intervention and de-escalation skills
  • Acts as a member of team with crisis situation occurs, assisting others to ensure patient safety
  • Completes accurate assessments of patients utilizing clinical skills
  • Assists care coordination department as requested
  • Completes all documentation thoroughly, timely, and in accordance with hospital standards
  • Displays a thorough knowledge of confidentiality
  • Completes releases appropriately
  • Demonstrates knowledge of HIPAA regulations and abides by those standards
  • Demonstrates a working knowledge of all clinical regulations, including TJC, Medicaid, and state and federal laws
  • Displays active involvement in the discharge, transition, and after-care planning treatment processes Other duties as assigned

What we're looking for
Applicants should have a current state LCSW, LMFT, LMSW, LPC license and possess a Master's degree from an accredited school. Additional requirements include: 

  • Basic Life Support certification is required within 30 days of hire. 
  • ACLS and PALS are required within six months of hire.  
  • One year of experience preferred 
  • ASLS certification preferred 

  

Hourly range: $40-50 per Hour

Join our team in an 11-bed Women's Services Unit featuring a Level 1 nursery and 6 LDRP (Labor, Delivery, Recovery, Postpartum) rooms. With an average of 15 deliveries per month and a below-average C-section rate, our unit provides comprehensive care for expectant mothers and newborns. This role offers the opportunity to work in a supportive environment dedicated to promoting a positive birthing experience and ensuring the health and well-being of mothers and infants alike. 


How you'll contribute 
A Registered Nurse (RN) who excels in this role:

-    Accurately performs patient assessments and identifies patient needs
-    Identifies and initiates appropriate nursing interventions
-    Provides care appropriate to condition and age of the patient
-    Performs timely and appropriate documentation relating to medical necessity in the medical record
-    Responsible for completion and revision of the Interdisciplinary Care Plan for each patient
-    Performs timely and accurate QI assessments
