A Brief Overview
The CDL Driver II is responsible for the safe, efficient transportation of materials to various job sites and other locations. This role requires maintaining a Class A or B Driver's license, inspecting and maintaining trucks, reporting shortages and claims to supervisors, and assisting in loading and storing of materials. Drivers must perform pre- and post-trip inspections, manage logs, and adhere to hours-of-service requirements. The CDL Driver II is typically a more experienced driver who may train new associates.
What you will do
- Operates Class A commercial vehicles in a safe, compliant, and efficient manner.
- Responsible for deliveries to both active construction sites and completed build locations; city-based deliveries and inter-state travel may be required.
- Performs thorough pre-trip and post-trip DOT inspections to ensure vehicle safety and compliance with federal regulations.
- Provides exceptional customer service by maintaining a professional demeanor, addressing client needs, representing the company positively during all interactions, and relaying messages received from customers while on job sites.
- Organizes and secures loads to ensure safe transport and verifies orders for accuracy before departure and upon delivery.
- Adheres to all regulations and safety guidelines established by the U.S. Department of Transportation (DOT) and complies with all regulations and conditions outlined in applicable permits.
- Prepares and maintains accurate DOT-compliant logs of driving hours, on-duty time, and rest periods in accordance with federal regulations.
- Delivers and unloads materials accurately and safely, ensuring customer satisfaction at the point of delivery.
- Records all merchandise returned by truck and assists in unloading materials returned for credit, ensuring proper documentation and handling.
- Safely and efficiently operates a truck-mounted forklift to load, unload, and transport materials at job sites.
- Maintains the cleanliness and organization of the truck, ensuring it is well-kept and ready for operation.
- Assist with stocking and organizing inventory in the warehouse and production areas.
- Trains and mentors incoming drivers on company policies, safety procedures, and operational best practices.

Education Qualifications

- Work Schedule – Monday through Friday, 8am to 5pm; Modified schedules are possible based on patient caseload at the facility.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties and tasks may be assigned.
- Knowledge of and comply with federal, state, local laws and regulations, including health care professionals practice act requirements as they pertain to home dialysis program and patient care requirements.
- Train patient (and/or care partner) in the practice of self-care Peritoneal Dialysis or Hemodialysis upon meeting federal and state regulations governing Registered Nurse qualifications.
- Conduct home visits to assess the patient's home environment per policy; initially and minimally annually thereafter. Routinely evaluate patient performance and assess the home environment more frequently as needed to improve care.
- Communicate results of patient assessment, reassessment, and ongoing monitoring to the physician, team members, and others as appropriate to the individual needs of the patient.
- Document all nursing services in the Electronic Medical Record including but not limited to training sessions, routine and non-routine in-person interactions, and phone conversations. Documentation should accurately reflect the patient status and nursing interventions and be written to ensure continuity of care.
- Participate in infection control monitoring, implementation, and recording as requested.
- Be familiar with emergency equipment and all emergency operational procedures. Communicate and regularly review Emergency Preparedness procedures with all home patients, including but not limited to emergency disconnection from dialysis equipment, what to do and who to contact if displaced from home.
- Ensure clinic and patient supply inventory and usage is appropriate for patient prescriptions and place orders in timely manner to avoid fees and penalties.
- Continually seek to improve patient and quality outcomes through tracking and trending of program and patient performance, use of critical thinking skills and root cause analysis to improve patient and quality outcomes. Assist in obtaining data for the continuous quality improvement activities.
- Obtain routine and non-routine laboratory tests as ordered and communicate critical lab values and urgent patient needs to the responsible physician or physician extender in a timely manner.
- Complete and document monthly review of patient medication profiles. Administer medications as ordered by the physician.
- Responsible for performing on call nursing services, nights and weekends, on a rotational basis as needed or assigned.
- Flexible with staffing locations and hours to accommodate patient and USRC home program needs.
- Regular and reliable attendance is required for the position.
Home Hemodialysis Only:
- Coordinate home evaluation with technical services department to ensure that the necessary electrical, plumbing and drainage requirements for proper equipment operation are met prior to patient acceptance into home program.
- Ensure required and appropriate water sampling is complete per policy and product requirements. Monitor and report water sample and culture results.
QUALIFICATIONS/REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
Requirements include:
- Current RN license in applicable state. License must be maintained as current and in good standing.
- 12 months of clinical experience in providing nursing care and 3 months of experience in the specific modality for which the nurse will provide home dialysis training is preferred, except in states with a more stringent requirement such as Texas, which requires at least 6 months experience in the specific home modality.
- Previous experience in providing nursing care to patients on maintenance dialysis preferred.
- CPR certification required prior to patient care assignment.
- Confirmation of ability to distinguish all primary colors.
- Must successfully complete the Home Therapy Nurse Orientation program and maintain annual demonstration of skills and competency applicable for the modality assigned.
- Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
- Must have basic computer skills; proficiency in all USRC clinical applications required within 90 days of hire.
- Must meet any practice requirement(s) for the applicable state.
- Ability to meet minimum hiring standards which may include additional background clearances and orientation requirements if applicable to the program assigned to for employment.

Responsibilities
- Interact with customer, exhibit account management skills to independently facilitate preventative maintenance, repair issue resolution, and discuss new repair estimates with customer. Coordinate work schedule as required by customers. Road test vehicles to identify cause of complaints for repairs, ensure quality of repair work performed, and deliver customer satisfaction. - Cranking System (electrical): Inspect and test batteries; Visually inspect cables and cranking motor; Clean the battery cables, connections and starter; Replace batteries; Charge and check batteries; Replace or repair defective cables
- Lighting System and Electrical Accessories: Repair minor wiring problems; Replace bulbs, lenses and simple light assemblies; Repair minor lighting problems; Replace and adjust sealed beams; Install or replace accessories such as heated mirrors, fans, spotlights
- Drive Axles: Inspect housings for leaks; Change oil and filter, if required, and clean breathers
- Driveline: Lubricate drive line; Inspect components for wear or damage
- Gas Engines-All Engines: Check and adjust oil levels; Identify unusual noises, and oil leaks
- Fuel Systems: Replace and prime fuel filter; Check gas and diesel fuel system components such as fuel lines, pickup tubes, return lines, etc, for proper integrity and condition
- Preventive Maintenance: Proper lube techniques; Minimal tire inspection; Brake check/applied stroke; Battery check; Under vehicle check/loose components; Initial Fault code identification; All PMs (PM298) including C supplements to exclude overheads, engine brake overhauls, DPF servicing, clean doser injector, servicing ArvinMeritor unitized hubs, inspect Lece Neville alternator brushes, inspect modulator system for proper operation, inspect/adjust shift selector cable, pop test Isuzu injectors
- Steering and Non-driven Axles and Alignment: Lubricate grease fittings, check and adjust fluid levels; Check and adjust toe; Check all steering components for wear; Check and adjust trailer axle alignment; Adjust wheel bearings including replacement of wheel seal
- Suspension-Chassis and Cab: Identify makes and models; Identify normal operation; Identify and repair minor air leaks; Identify malfunctions of basic nature, broken springs, excessive wear on bushings, etc.
- Tire and Wheel Maintenance: Perform air pressure checks; Check tread depth; Identify tires that need to be changed or repaired; Perform tire P.M. checks; Replace tire and wheel assemblies; Mount/dismount and rotate tires; Perform tire/wheel checks/repairs per (TW298)
- Vehicle Coupling System (PM): Lubricate fifth wheel plate and slider mechanism; Operate slider mechanism
- Utilize key functions of Shop Management System and electronic documentation available. Perform work in accordance with all federal/provincial and corporate compliance guidelines (i.e. safety, DOT). Use internal and external maintenance documents
- Ensure proper maintenance of all service, safety and environmental records required to meet and/or exceed corporate, federal, state, and local government mandated requirements
- Accountable for key maintenance measurements established by company policy, related to the inspection and performance of PMs, brakes, tires, wheels, including any maintenance items directly associated with the safety of the customer
- Act as a mentor for the Mobile Technician II level
- Performs other duties as assigned

SUMMARY
- Position Type: Full-time, non-exempt, hourly supervisor role.
- Scope of Role:
- Supervises a single program or unit within a program, typically supporting 4-8 individuals in residential settings or 10-15 individuals in periodic, vocational or day programs. Duties are split between the provision of direct support, professional or program activities and supervision.
- Key Responsibilities:
- Supervise Direct Support Professionals
- Oversee the day-to-day operation of the program including delivery of supports, implementation of person-centered plans, health care, advocacy, customer engagement, regulatory compliance and when applicable, oversight of the physical environment.
- Generally works 8 to 16 flexible hours per week in a supervisory capacity; and provides direct support for the remaining hours.

- Direct Support: Works regularly scheduled shifts as a direct support professional (see DSP position description).

Health Care:
- General Health Care: Monitors health of persons being served; documents; communicates with medical professionals as appropriate; implements preventive strategies to limit illness and serious incidents; reviews and monitors documentation across all shifts.
- Appointments: When applicable, schedules or assists in scheduling health care appointments; may accompany individuals to appointments; may assist with communication; documents visits; oversees follow through of orders; communicates results as appropriate with families, guardians, case managers, day programs or others.
- Medications: When applicable, accurately administers or supports self-administration of medication and documents delivery of medications and treatments; reviews all other medication documentation for accuracy and completeness, daily or weekly as required; reviews medication errors with employees; communicates medication changes and provides training across all shifts; maintains appropriate security of medications and supplies.
- Medical Supplies and Equipment: Purchases and maintains medical supplies and equipment in consultation with Program Director; communicates with employees and may assist with training on proper use.
- Dietary planning: Develops menus based on diets, preferences, and occasions, and with the exception of some in-home services, purchases or oversees purchasing of food and supplies in accordance with planned menus and individual choice or need; directs and monitors food service, consistent with individual plans.
Stakeholder Relations:
- Communications: Maintains productive and professional relations with individuals, families, case managers and other stakeholders.
- Customer Service: Implements Sevita Customer Service Standards; trains DSPs in delivery.

- Living Environment: When applicable, schedules and monitors daily and seasonal housekeeping; oversees lawn and yard care. Maintains neat, clean and safe environment for individuals served.
- Maintenance and Repair: When applicable, makes or arranges for minor repairs; reports to and schedules repairs by maintenance personnel as required; monitors environmental safety; monitors water temperature, refrigerator and freezer temperature, and safety alarms and extinguishers on a monthly basis; replaces furnace filters monthly.
- Equipment: Monitors wheelchairs, lifts and other safety equipment; sees that repairs are made promptly and correctly in the event of faulty equipment.

- Contacting each referred family a minimum of 2 times a month during the first trimester and second trimester and until the family is assessed.
- Contacting each referred family a minimum of 1 time per week during the third trimester and until the family is assessed.
- Contacting each referred family a minimum of 2 times per week once the infant is born and until the family is assessed.
- Ensuring all necessary data related to referrals and screenings is inputted correctly into the Heathy Families America Entrance to Exit log that is submitted for accreditation.
2. Initiates and conducts screenings for families interested in the Healthy Families program.
- Conducting screening on all families using the Healthy Families Screening Tool and Contact Form.

3. Initiates and conducts welcoming, quality, and engaging assessments for all families enrolling in the Healthy Families program in accordance with the core Healthy Families America model standards, including.
- Completing the validated Family Resilience and Opportunities for Growth (FROG) Scale with newly enrolling families.
- Helping families identify their family strengths and areas of need
- Ensuring the FROG assessments are completed prenatally or within 3 months of the infant's birth.
- Preparing each FROG for review by supervisor.
- Reviewing each FROG with supervisor.
4. Promotes the Healthy Families program by.
- Maintaining quality and positive relationships with referring organizations.
- Providing referring organizations follow up on every referral received, including whether or not the family declined services.
- Represents the program at formal community venues to promote the program and facilitate referrals.
- Attending monthly Community Advisory Boards to ensure close communication occurs between Healthy Families program and community resources.

- Entering all screening, FROG, and Closure information onto tracking spreadsheets submitted for accreditation.

6. Maintains completion of required supervision, including.

- 28 hours of initial Foundations for Family Support Core Training.

- 15 hours of Infant Mental Health Training annually to maintain Endorsement.
8. Achieve and maintain Infant Mental Health Endorsement through the Tennessee Association of Infant Mental Health, including.
- If hired without an Endorsement, will submit application on Endorsement Application System, uphold Ethics Code, achieve mandatory references, and complete and submit 30 hours of required infant mental health training within first year of employment.
- Once Endorsement is achieved or if hired with an endorsement, will maintain endorsement every year but completing and submitting 15 continuing education hours and updating the Code of Ethics review process annually.

- The Association of Infant Mental Health in Tennessee (AIMHITN) Infant Mental Health Endorsement preferred.
- Required to apply for AIMHITN Infant Mental Health Endorsement within the first year of employment if hired without endorsement.
- Training and knowledge in infant and early child development and parenting skills required.
