The Opportunity
- Oversee quality and safety initiatives.

The Opportunityย
- Responsible for supporting, organizing, conducting, and directing the Vituity HEART Thoracic Donor Organ Transplant Recovery Program.
- This role encompasses liaison activities between Administration, the Surgical Staff, the Medical Staff, performance measurement, protocol development, research, and quality improvement activities for the procurement of donor Heart and Lung grafts for thoracic transplantation.
- Responsible for the recovery of donor Heart and Lung grafts for the Heart and Lung Transplant Program in keeping with the policies, rules, and regulations of Vituity-contracted Hospital Systems, clients, and its medical and surgical staff.
- Responsible for adhering to all applicable State, Federal, and local laws, policies, and regulations.
- Aid in compliance with International Society of Heart and Lung Transplantation, (ISHLT), United Network for Organ Sharing (UNOS), and Organ Procurement Organizations (OPOs), surveys, and laboratory data collection.
- Help in preparation for federal, state, or JCAHO inspections.
- Collaborate with physicians and Thoracic Transplant Program team members to promote the concepts and practice of Quality Assurance by linking improvement activities, education, and information.
- Facilitate appropriate financial and operational efficiencies.
- Coordinate with hospital to ensure adequate training of nursing staff and clinical transplant coordinators in the care of donor thoracic transplant patients.
- Ensure that tissue typing and organ procurement services are available.
- Ensure that heart and lung transplant procurement is performed by, or under direct supervision of, a qualified surgeon.
- Participate in hospital and Medical Staff committees dealing with thoracic transplant donor issues.
- Follow safety standards in all aspects of performance of the above functions and attend applicable safety education sessions.
- Review operations, policies and procedures, and staff education and training, including a disaster preparedness plan.

Rquired Experience and Competenciese
- Board Certified/Eligible Thoracic Surgery physicians required.
- Valid license to practice medicine in one of the contiguous United States required.
- Providers who graduated from a Thoracic training program before 1990 and has at least seven (7) yearsโ experience in Thoracic Transplant Surgery will be considered.
- Providers who are newly graduated within the time specified by the American Board of Medical Specialists and Medical Staff by-laws will be considered.
- Maintain appropriate current professional knowledge in the care of Thoracic Transplant patients by obtaining at least 12 CMEs in Thoracic OR Surgery every three years and an additional training of one year formalized training in thoracic transplantation required.
- A minimum of two years documented experience with a transplant program that meets the qualification for UNOS membership required.
- Experience in pre-operative and post-operative patient care responsibility for an optimum of 20 and minimum of 10 heart transplants for adults required.
- Experience in pre-operative and post-operative patient care responsibility for an optimum of 20 and minimum of 10 lung transplants in a UNOS approved transplant program required.
- Cumulative experience of at least 20 thoracic transplants over more than two years required.
- Maintain current knowledge in the care of thoracic transplant patients by obtaining a minimum of 12 CMEโs in thoracic transplantation AND/OR Thoracic surgery every three years required.
- Additional training of one year formalized training in thoracic transplantation OR a minimum of two yearsโ documented experience with a thoracic transplant program that meets the qualification for UNOS membership preferred.
- Experience in pre-operative and post-operative patient care responsibility for at least 10 thoracic transplant recipients in a UNOS-qualified program OR cumulative experience of at least 20 thoracic transplants over more than two years preferred.

Job Description:
I. Position Summary:The Hebrew SeniorLife Hospice is designed to provide a Jewish Hospice service to terminally ill patients and their families in the greater Boston area.ย The philosophy of the Hospice is rooted in Hessed (loving kindness), and Kevod HaBeriyot (honoring life), but Hebrew SeniorLife Hospice Care is open to anyone regardless of religious affiliation; our mission is informed by Jewish values that are shared across many faith traditions and spiritual belief systems.ย
Hebrew SeniorLife Hospice works collaboratively with patients and families in the final months, weeks, and days of life, with the goal of achieving optimum healing of body and soul when curing is no longer an option. This means helping the patient and family with: physical comfort, emotional and spiritual support, and companionship. The Hospice goal is to help every patient at the end attain a peaceful and pain free death and support the healing and bereavement of the family.ย
As a member of the interdisciplinary group (IDG), the Hospice Registered Nurse provides professional nursing care to the Hospice's patients.ย Identify patient/family needs and provide supportive care in accordance with the attending physician's orders, the IDG plan of care and the Hospice's policies and procedures.ย The Hospice Registered Nurse provides care under the direction of the attending physician, the Hospice Medical Director, the Clinical Director, and in compliance with the state's Registered Nurse Practice Act.
II. Position Responsibilities:
- Assumes responsibility for a patient visit or on-call schedule which includes the assessing, planning, implementing and evaluating phases of the nursing process.ย
- Provides Hospice nursing care based on systematic assessment focusing on pain control and symptom management.
- Initiates communication with attending physicians, other Hospice staff members and other agencies as needed to coordinate care and use of resources for the patient/family.
- Assesses the patient and develops an individualized and comprehensive plan of care in partnership with the Interdisciplinary group and in accordance with Hospice regulations/guidelines.ย Routinely assesses patients response to plans of care and performs comprehensive assessments, hospice aide supervision, and coordination/oversight of hospice services provided in assisted living, LTC, SNF and hospital settings.
- Maintains regular communication with the Clinical Director to review assignments and with the attending physician/medical director concerning patient/family needs.ย
- Obtains data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporates that data into the plan of care.
- Maintains up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur in accordance with agency policy and Medicare regulations.
- Accurately documents assessment findings, observations, interventions and evaluations pertaining to patient care management and services provided in accordance with Hospice policy and procedures on the day services are rendered.ย Submits documentation in a timely manner.
- Teaches, supervises and counsels patients/families regarding physical care and comfort, and other problems related to the patient's terminal condition.ย
- Provides guidance to patients/families to assist them in preparing for and coping with anticipated physical and psychological events throughout the dying process.ย Provides appropriate support at time of death and period of bereavement.
- Meets regularly with Hospice nursing staff to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed toward enhancing professional growth.
- Participates in IDG conference as needed and facilitates discussion of issues from caseload for full staff discussion, consultation and evaluation.
- Supervises the Hospice Aide and the aide plan of care at least every fourteen (14) days and documents such supervision. If concerns are noted, observes the aide onsite providing care. If concerns are ongoing, reports to the Clinical Director of the need to repeat a competency evaluation with aide.
- Informs the Clinical Director of unusual or potentially problematic patient/family issues.
- Shares in providing 24-hour, seven-day a week coverage (evening and weekend on-call) to patients/families.
- Demonstrates working clinical knowledge and skills relative to hospice and end-of-life care, and a commitment to clinical excellence and ongoing training in the latest pain and symptom management interventions.
III. Qualifications:
- Graduate from an accredited School of Nursing, BSN preferred.
- Currently licensed in good standing to practice as a Registered Nurse in the State of Massachusetts.
- Must have no less than six months experience in acute care setting as an RN prior to home care assignment, or experience as required by state regulations.ย
- Minimum of one yearโs experience as an RN required.ย
- Experience in Hospice is strongly preferred.
- Demonstrated ability to assess and respond to the needs of patients and families in varied settings and to cope with emotional stress.

The Opportunity
- Act as the operational administrator for the site Vituity leadership and as the interface for the practice to the hospital and community.
- Act as the front-line liaison for the provider team with hospital C-Suite, nursing leadership, nursing staff, and Vituity support team.
