POSITION SUMMARY/RESPONSIBILITIES

Provides professional level skills required in the clinical practice of mental health counseling and crisis intervention, affording direct services to individuals housed in the adult detention center. Screens and identifies inmates who are mentally ill, have substance use issues, and/or may be at risk for suicide and make the appropriate referral for further assessment. Works as a therapeutic agent as a member of a multidisciplinary team to provide evaluation, treatment planning and service coordination for those in adult detention. Counsels and assists patients with personal and environmental difficulties which predispose illness or hinder the patient from receiving maximum benefits from medical care. Actively supports the policies and procedures of the University Health System.

EDUCATION/EXPERIENCE

Master's degree in Social Work from a school accredited by the Council on Social Work Education is required. One year post master's degree experience in a health care setting is preferred.

LICENSURE

Current state licensure from the Texas State Board of Social Workers Examiners is required. Current CPR certification as a BLS HealthCare Provider is also required. Certification as a Correctional Health Care Professional is preferred when eligible.

Position Title Ambulatory Social Worker CM Landon Center on Aging Position Summary / Career Interest: The Ambulatory Social Work Case Manager has responsibility to provide care/service safely and efficiently for a full range of services to patients of all ages and their families.

Primary role is to collaborate, communicate and facilitate coordination of services as established by the healthcare team and executed by the case manager.

Responsible for the psychosocial assessment of patients, coordination of care and linkage to community resources and providing emotional support to patients and their families.

Ensure appropriate decision makers are informed of and involved in treatment planning.

Apply pertinent state and federal regulations regarding documentation and reporting requirements.

The Social Work Case Manager identifies, monitors, and reports opportunities for quality and performance improvement to the appropriate department.

The Social Work Case Manager takes an active role in performance improvement activities as it relates to their area of assignment.

Our direct client is seeking a Licensed Clinical Social Worker (LCSW) to manage their youth and young adult development programs. The LCSW will oversee participants in our programs including HHA and CNA certifications, focus on participants' mental health, well-being, self –management and other supportive services needs of participants through small groups and one-on-one sessions as needed. This full-time role requires flexibility in working mornings, evenings, and Saturdays at multiple sites, with hours, days, and locations varying based on seasonal changes.

Summary: The Licensed Clinical Social Worker will provide high-quality psychological services in an integrated care setting.
