About the Role:

The RN Case Manager Hospice plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses in the Snellville community. This position is responsible for coordinating and managing comprehensive hospice care plans that address the physical, emotional, and psychosocial needs of patients and their families. The RN Case Manager serves as a liaison between patients, families, interdisciplinary team members, and external healthcare providers to ensure seamless communication and continuity of care. By conducting thorough assessments and regularly evaluating patient progress, the RN Case Manager ensures that care goals are met while respecting patient dignity and preferences. Ultimately, this role contributes to enhancing the quality of life for patients during their end-of-life journey through expert clinical management and empathetic support.

Minimum Qualifications:

- Current and valid Registered Nurse (RN) license in the state of Georgia.
- Minimum of two years of clinical nursing experience, preferably in hospice, palliative care, or related fields.

Summary:

The Travel Cath Lab RN provides specialized care for patients undergoing cardiac catheterization and interventional treatments. The RN prepares patients for procedures, assists during interventions, and monitors immediate post-procedure recovery. Working closely with cardiologists and cath lab teams, this role requires advanced cardiac care skills, cardiac catheterization skills, strong communication, and the ability to adapt quickly while maintaining strict safety and regulatory standards.

Essential Work Functions:

- Conduct assessment of patients prior to procedures, including reviewing medical history and performing physical exams
- Assist in positioning patients correctly for the procedure and ensure EKG electrodes and cardiac leads are properly placed
- Assist physicians during procedures, including the placement of catheters, wires, balloons, stents, structural heart devices, and other cardiac interventions
- Operate and monitor hemodynamic equipment, recognizing and responding to changes in cardiac rhythms and hemodynamic waveforms
- Assist in arterial and/or venous sheath removal using closure devices, manual pressure, or mechanical compression, and transport patients to the appropriate recovery area
- Monitor vital signs, ECG, and hemodynamic parameters during cardiac catheterization and interventional procedure
- Support post-procedure care, including monitoring patient recovery and ensuring a smooth transition to the next phase of care
- Maintain and troubleshoot cath lab equipment, ensuring everything is functioning properly and fully stocked for procedures
- Communicate effectively with team members, patients, families, and physicians to provide seamless care
- Adhere to hospital safety protocols, infection control guidelines, and regulatory standards
- Perform other duties as assigned within the scope of practice

Client Operations Associate (Order Processing)

Join our team as a Client Operations Associate and become the backbone of our sales operations. You'll handle a wide range of tasks from processing sales orders and invoices to managing returns. Your attention to detail will shine as you calculate freight costs and coordinate shipments, ensuring smooth logistics for our team.


With a focus on collaboration, you'll interact with managers, sales representatives, customers, and vendors to facilitate seamless communication and allow our sales team to exceed our gross margin quota. Additionally, your proactive approach to customer service will be essential as you follow up on orders, provide tracking information, and ensure client satisfaction. Be Ready To…


Order & Transaction Processing

  • Actively monitor the incoming requests to process sales orders, purchase orders, invoices, and returns according to company policies and procedures
  • Handle RMAs, RTVs, and commercial invoices
  • Handle Accounts Receivable follow-up with customers
  • Calculate freight costs and coordinate courier/ LTL shipments
  • Manage most aspects of inbound purchases for the team


Communication & Customer Service

  • Interact with managers, sales representatives, customers, and vendors via email, telephone, and face to face
  • Communicate with customers to follow up on orders; send tracking information and credit card receipts


General Support & Special Projects

  • Answer phone calls from the inbound company call queue
  • Ad hoc projects as assigned


What You Bring to Summit 360:

  • Associate or bachelor’s degree preferred
  • 1-3 years of experience in an administrative role preferred
  • Impeccable attention to detail, accuracy, and organization skills
  • Excellent time management, problem solving, and prioritization skills
  • Disciplined work ethic with the innate ability to take initiative
  • Working knowledge of Microsoft Office Suite
  • General understanding of logistics coordination - domestic and international
  • Excellent oral and written communication skills
  • Exceptional customer service skills
  • In depth understanding of and ability to follow processes
