Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist in accordance with the patientβs Plan of Treatment.
Assist in the implementation of vocational/education programs and activities established by registered Occupational Therapist designed to restore, reinforce, and enhance task performances, diminish or correct pathology, and to promote/maintain health and self-sufficiency.
Design/adapt equipment and working and/or living environment.Β Fabricate devices to assist and improve function and independence and participation in the program and/or community where possible.
Provide therapeutic treatment and instruction to patients as directed by the qualified Occupational Therapist and in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.
Report information and observations to Occupational Therapist and/or Clinical supervisor, document observed information in patient records and prepares clinical notes.Β Β Assist with preparation of progress reports.Β Β Maintain and submit documentation as required by the Company and/or facility.
Instruct patients and family members regarding home programs as well as care and use of adaptive equipment.
Participate in care coordination and discharge planning activities and act as a resource to other health care personnel in meeting patient's needs.
Design community reintegration activities, as appropriate, to assist the client in the physical reconditioning effort, and/or the psychological adjustment and coordinate the plan with members of the interdisciplinary team.

As a Home Health Physical Therapist Assistant, you will:
- Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
- Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
- Document client records and submit clinical documentation to the office on time each week.
- Attend educational meetings and coordinate department in-services.
- Notify Physical Therapist or Branch Director of any equipment failures, lack of supplies, or staff or client changes.
- Apply concepts of infection control and universal precautions in performing patient care activities.

Host Home Providers who have NO small children living in the home
We are in need of a couple or a dual-provider home with no pets or small children to provide Host Home services to a Tier 7 individual. Higher negotiated rates of pay are available.
Parker Personal Care Homes (PPCH) is looking for dedicated, honest, and responsible individuals to provide compassionate care and support to adults with Intellectual or Developmental Disabilities in your home and as a part of your family.
We look for folks who can offer services and supports that might include personal care, problem solving, safety in the community, learning new skills, increasing levels of independence, managing challenging behaviors and/or medical needs.
You must be willing to provide a home that is safe and supportive while assisting with activities of daily living, recreation and socialization, health and safety concerns and medical appointments. We provide all of the necessary training for our providers to be successful, including training in the areas of developmental disabilities, CPR, First Aid, medication administration, behavioral safety training, members' rights, confidentiality, and more.
PPCH is seeking Host Home Providers with NO pets or small children living in the home. PPCH needs providers who are willing to provide services to members with specialized behavioral needs. Who lives in Host Homes?
PPCH offers host home settings for individuals requesting to reside in a home with their provider. Individuals who require verbal and/or physical assistance with daily needs often consider a host home setting.
What is a Host Home setting like?
PPCH has numerous host homes located throughout the metro area. Each host home is unique in its area of specialty, general layout and physical location.
What are the responsibilities of a Host Home Provider?
The Host Home Provider's main focus is to provide a safe and supportive environment for the member(s) in services in their home. Each Host Home Provider is responsible for ensuring the health and safety of the individual living in their home, while ensuring they will have plenty of opportunities for socialization and access to community resources.
What type of training does a Host Home Provider receive?
PPCH provides specialized trainings to all providers. These trainings include CPR/First Aid, Medication Administration, Customer Rights & many other areas. All host homes are monitored closely by PPCH to ensure that the member in services is receiving the highest level of care.
Qualifications
Must be able to provide services and support for 1 member in your own home on an on-going basis
Must successfully complete PPCHβs required training courses
Must be able to chart individual daily member activities and medications in a web-based system (training is provided)
Have professionalism and compassion
Able to focus on individualized care and support
Ability to teach others
Have good written and verbal communication skills
Have a valid driver's license and reliable transportation
Must clear background screening and reference checks
Be proficient with Microsoft Office, Google Office Suite, or similar software
Experience with intellectual or developmental disabilities is preferred, but not required

1.Β Admits/readmits patients and provides intermittent nursing service to patients and their families in compliance with agency policies and State and Federal Regulations, incorporating the following:
- A complete physical and psychosocial assessment.
- Develops and implements an appropriate care plan and therapeutic goals with the patient/family.
- Evaluation of patient safety on a 24-hour basis.
- Exercises appropriate clinical judgment in coordination of patient care by all services involved.
- Ongoing evaluation of significant changes in the condition of the patient and of progress toward the therapeutic goals.
- Use of Agency and community consultants/resources.
- Β Instruction of patient/family in procedures or responsibilities they are assuming and optimum wellness practices.
- Planning for the patientβs discharge from service.
2.Β Β Establishes and maintains case mix weight, utilization, patient satisfaction that aligns with industry standards and agency operational objectives for quality care.
- OASIS documentation correlates with the Plan of Care.
- Uses recommended Oasis assessment techniques 100% of the time.
- OASIS, documentation, and plan of care correlate to 95%.
3.Β Β Documents all services provided in the clinical record in accordance with State and Federal Regulations and Agency policy:
- Documentation, including activities and mileage is exported within 36 hours of visit or prior to next scheduled visit.
- Obtains physician verbal/written orders and complies with all reporting requirements.
- Participates in coordination of care with physicians and with other disciplines as appropriate.
4.Β Β Manages work assignment in effective and efficient manner:

- Visits all new therapeutic clients within 24 hours of referral unless otherwise specified
- by the physician.
- Reports any cases to the Supervisor of Clinical Services where goals can not be met
- or at-risk situations exist and responds appropriately.
- Remains flexible in daily planning, allowing time for emergencies or unexpected
- visits.
- Maintains productivity as established by the Agency.
- Utilizes payer sources correctly, reflecting changing levels of care.
- Delegates patient care responsibilities to Home Care Nurse (LPN) as appropriate.
- Demonstrates an understanding of the principles of case management.
- Demonstrates ability to manage cost and quality
5.Β Β Establishes and evaluates the Plan of Care to be delegated to the Home Health Aide, orients and supervises the Home Health Aide in accordance with State and Federal Regulations and Agency policy:

- Develops written plan of care for the Home Health Aide and updates this plan as patientβs condition warrants; plan of care must be reviewed every 60 days.
- Obtains input from Home Health Aide regarding patient progress.
- Prepares written performance evaluations of home care aides under his/her supervision.
- Provides supervision of home care aides at a minimum of every 14 days or as required by payer source.
6.Β Β Develops and maintains sufficient professional skills and credentials to meet the needs of patient care:
- Attends a minimum of 12 hours of in-service per year to include mandated annual in-services.
- Participates actively in self-appraisal and the formation of a plan of self-development.
- Adheres to Agency Corporate Compliance Plan and HIPAA Privacy Plan as per written agreement.
- Seeks appropriate clinical input to ensure appropriate patient care.
- Projects professional image through attitude and compliance with dress code.
- Capable of precepting new nurses and students.
- Functions as a team member with consideration to all staff.
- Provides Agency with current copy of professional license.
- Maintains CPR certification.
Ancillary Duties
1.Β Β Performs other duties as necessary to support Agency programs as assigned by the Supervisor of Clinical Services.
