Job Summary:
The speech language pathologist evaluates patients and develops the speech/language plan of care in the areas of speech, language, voice, cognition and swallowing in collaboration with the physician. Services are provided across the full continuum and rendered in a cost conscious, quality focused and customer oriented manner. Compliance with all applicable regulatory standards is also required in this role.
Core Responsibilities and Essential Functions:
Treatment Planning and Provision of Care
* Implements the speech language therapy treatment plan
* Selects appropriate treatment activities to progress patient towards goals.
* Revises goals/plan of care with patient/family input
* Provides ongoing patient/family education and training
* Provides discharge instructions, follow-up and referral to community resources as appropriate
* Acts as a referral source for staff, families, physicians on services and equipment related to rehabilitation services.
Patient Assessment
* In collaboration with the physician, responsible for the establishment and ongoing evaluation of treatment programs, plan of care, goals and discharge plans for the patients as related to speech, language, cognition, voice and swallowing.
* Establishes a plan of care in conjunction with the patient and/or family members.
* Establishes realistic discharge plan incorporating patient and/or family member goals.
Documentation
* Documents the evaluation and plan of care according to departmental guidelines
* Documents each treatment session according to departmental protocols
* Completes and submits family education, discharge and other required documentation within facility and program guidelines

ESSENTIAL FUNCTIONS: Analyzes blueprints, schematics, and drawings for existing and proposed HVAC systems for the purpose of identifying the location of new and existing systems and determining the efficient installation of additional equipment/systems.
Confers with immediate supervisor and assists with the coordination of day-to-day activities (e.g.
periodic lubrication; filter cleaning; changing belts, repairing system leaks, checking water tower components, trouble/issues phone call response, etc.) for the purpose of ensuring the proper and efficient maintenance and repair of District HVAC.
Coordinates with administration and other trades for the purpose of completing projects/work orders efficiently.

SUMMARY
The Registered Nurse (RN) is a member of the Patient Care Services Team. The Registered Nurse is responsible for the satisfactory completion of nursing care by the nursing team. The Project Manager is responsible for the overall leadership and management of the project, in a role and reporting structure as designated for the project. The Project Manager is further responsible for the overall administration of the project, including subcontracting, procurement and purchasing of trades and vendors, design management, client management, scheduling, documentation, meetings, reports, document control and management, and cost management, including budgets and internal cost reporting, profitability, subcontractor and vendor payments, owner billings, and change orders. - Oversee the construction and renovation of pharmaceutical manufacturing facilities, specifically those involved in the production of Active Pharmaceutical Ingredients (APIs).

When project is 90% complete, assist the Superintendent in preparing \"Work Completion Lists\" to help ensure the project is fully completed in a timely manner.
- Establishes the project document control system to ensure the project is using the most current contract documents for construction. This includes the system to initiate and monitor Request for Information (RFIs) as well as the shop drawing control and management.
- Responsible for the overall communication and documentation on the project, including the filing system, letters, emails, meeting minutes, correspondence, and all reports.
- Attend weekly subcontractor meetings and prepare meeting minutes as documentation and follow-up. Involved as participant or lead, according to project responsibility.
- Staff leadership position for the Owner/Architect meetings with required minutes and documentation.
- All aspects of cost and profitability management. This includes change orders, internal cost coding, profitability analysis, and all other logs, reports, and tools to review, evaluate, and manage cost expenditures.
- Approval of subcontractor and vendor billings and invoices for payment after Superintendent has approved.
- Preparation and submission of monthly billings to the Owner.
- Monitor labor, material, and equipment costs with Superintendent to maintain budget. Prepare monthly status reports and progress reports.
- Review safety program being implemented on site with Superintendent to ensure compliance.
- Develop relationships with Clients for possible repeat business. Participate in business development activities and assist in preparation of proposals and promote productive client relationships.
- Provide leadership in the professional and career development of subordinates as well as timely and fair performance evaluations. Provide mentoring, training, and development as needed. Maintain effective communication with clients. Understand the client's needs to achieve an acceptable level of satisfaction.
- Perform all the closeout documentation required.
Education and experience:
- BS/BA degree in engineering, architecture, or construction management.
- Proven prior success at Assistant Project Manager level, or equivalent role. Already operating in general at Project Manager I level.
- As a general guideline, there is a minimum of five (5) years' experience in the construction industry, with at least two (2) in the position of Assistant Project Manager.

About the Role
Essential Duties and Responsibilities:
- Incorporates quality nursing practice initiatives, continuity and consistency for patient care services throughout the Masonicare Health Center continuum.
- Establishes policies, procedures, nursing standards of patient care, and standards of nursing practice consistent within Health Center, consistent with organizational priorities throughout the Masonicare Health Center continuum.
- Reviews, approves, monitors and evaluates the implementation of patient care programs, services and employee health programs/services.
- Represents the Nursing departments in organizational and strategic planning. Develops and implements action plans in order to meet established Health Center and Corporate goals and priorities.
- Develops a plan in collaboration with Health Center Administrator and patient care team for the provision of patient care services consistent with the organizational strategic plan, goals and objectives.
- Provides managerial and leadership development to Clinical Nurse Managers and direct reports. Motivates, mentors and establishes accountability expectations for quality nursing unit management and care outcomes.
- Supports customer and service excellence initiatives throughout the nursing department, setting high standards for successful resident and patient satisfaction survey outcomes.
- Develops, reviews, submits and manages departmental operating and capital budgets prepared by direct reports. Ensures budget performance and variance accountabilities.
- Provides leadership, direction and consultation in human resource management. Supports and initiates nursing recruitment and retention strategies in collaboration with the Human Resources department.
- Provides a formal liaison between patient care services, senior management, department managers and medical staff. Promotes effective communications among all customers including residents/patients, families, physicians, other Masonicare affiliates, board members and the community.

Qualifications
Minimum Qualifications:
- Education: Bachelor’s Degree preferred. RN Licensed in State of Connecticut with 1 year of additional education or experience in rehabilitation or geriatric nursing and one year of nursing service administrator.
- Experience: 5-7 years progressive experience in nursing administration which includes a minimum of three (3) years in a senior management and leadership position. Previous LTC experience is preferred along with a thorough up-to-date knowledge of LTC codes, regulations, MDS and reimbursement issues.
- Certificates, Licenses, Registrations: Registered Nurse in the State of Connecticut or eligible to be licensed.
