EDUCATION
BSN is required. Recent patient care experience in transplant or at least one year of experience as a registered nurse is required. National certification in transplant is preferred.

Position Summary:
Working with Care Alliance Health Center patients as part of the Patient- Centered Medical team, Patient Care Coordinators address and reduce barriers to care. Activities include health care navigation, benefits enrollment and usage, population-specific interventions, community case management, and patient advocacy.

· Based on provider referral and patient screening, connect patients to available community resources such as reduced fare bus tickets, housing support, income and food support, job training, etc.
· Support patient comprehension of their diagnosis, treatment plan, and next steps, and connect patients to the appropriate licensed clinical professional.
· Work with providers to coordinate specialty care outside Care Alliance
· Assist patients with scheduling appointments; coordinating prior authorizations or insurance benefits; basic understanding of procedures; retrieval of specialty care reports, results, or visit summaries and appropriate follow up.
· Support enrollment and use of private benefits such as pharmaceutical assistance programs
· Identify and address non-medical barriers to health and self-sufficiency such as transportation, housing, income, recreation, and education.
· Establish and maintain positive relationships with community resources and social service agencies to link patients appropriately.
· Link patients to other experts such as Legal Aid or housing case managers and follow up with patients and external providers accordingly.
· Conduct daily review of outstanding referrals including appointments to be scheduled, patient navigation, and specialty visit results.
· Support engagement in primary care and provide support and assistance to clients in gathering and completing all necessary documents, submitting to applicable agencies, and appropriate follow-up.
· Educate patients on how to use new insurance options such as basics on managed care network coverage and drug formularies.
Minimum Education and Experience:
Required:
· Associate’s degree and/or commensurate experience may be considered, BA degree in social work, family development, healthcare administration or a related field preferred.
· 1-3 years’ experience in at least one of the following areas: case management or counseling, experience, experience working with vulnerable populations, mental illness, and/or substance abuse.
· Experience using EPIC or another EHR is an added plus.
· Knowledge of relevant community resources and ability to work collaboratively with community service providers.
· Ability to work independently and as part of a multi-disciplinary team of staff at various skill and professional levels.
· Strong problem-solving skills
· Ability to plan, organize and complete paperwork in timely manner and maintain confidentiality

Summary

Job Duties

Performs comprehensive subjective and objective ongoing assessments of patient's status that includes physical, psychosocial and environmental parameters. Relays significant changes in patient status to the physician, RN Case Manager, and other members of the interdisciplinary care team as appropriate in a time period consistent with patient needs.
- Reviews, evaluates, and as appropriate, revises the established plan of care to reflect progress toward achieving patient outcomes/goals in collaboration with the RN Case Manager and Home Health Care Navigator.
- Provides educational opportunities for patients, families, and clinical staff focusing on end-of-life issues, palliative care, advance directives, chronic disease management, pain management, symptom control, home care, hospice, and discharge planning. Promotes patient/caregiver autonomy. Evaluates effectiveness of teaching and modifies education based on patient needs and goals.
Minimum Qualifications

UPMC Home Health in Williamsport is hiring a Patient Care Coordinator Nurse to support their team. This is a Monday-Friday, 8:00AM-4:30PM shift with an on-call weekend rotation.
The Patient Care Coordinator is an on-site clinical team member that assists in coordinating care for the assigned team. The Patient Care Coordinator provides care navigation through daily triage and follows up with referral sources, patients, clinicians, and clinical leaders through the management of incoming communication to the Clinical Manager.
Responsibilities:
- Participates in on-call coverage rotation for the agency.
- Oversees the ordering and approval of medical supplies to ensure that all supplies are included on the physician orders and ordered in the appropriate quantity.
- Monitors lab results, following up with physicians and team members.
- Covers from other Patient Care Coordinators when needed.
- Participate in interdisciplinary team conferences as needed.
- Performs medication reconciliation on all therapy only cases as needed
- Monitors admissions, discharges and re-hospitalizations and communicates with the appropriate clinical liaison(s) or facilities to facilitate continuity of care between settings.
- Aids clinical staff in follow up communication with MD offices and referral sources as applicable.
- Coordinates all incoming orders associated with census to ensure appropriateness and accuracy and timeliness of care, enters into Medical Record, and coordinates with scheduling as appropriate.
- Assists in document control processes, including but not limited to, review of Face-to-Face documentation and physician orders for home health. Reports problems immediately to the Clinical Manager.
- Triages and directors all incoming communications during workday hours for the current census assigned and follows up to facilitate appropriate service delivery.
- Current Registered Nurse license in Pennsylvania.
- One-year recent home health experience preferred.
- Must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with standards and regulations; must have exceptional interpersonal and customer services skills; must be able to effectively solve unique problems as they arise or identify appropriate resource for hand off.
- Must have requisite knowledge of current Home Health Medicare, Medicaid, State and Federal regulations and Joint Commission accreditation standards.

Pediatric Nurse Practitioner – Hospice & Palliative Care

Position Overview
We are seeking an experienced and compassionate Pediatric Nurse Practitioner to join a highly respected hospice and palliative care program supporting pediatric patients and their families. This role has two primary focuses: providing NP-level hospice care to enrolled pediatric patients and supporting a specialist pediatric palliative care consultation service.
The vast majority of visits will take place in the patient’s home, with occasional visits to hospitalized pediatric patients as needed. Some visits may also be conducted via telehealth based on clinical appropriateness and patient needs.
This role is ideal for a clinician with strong skills in advanced symptom management, family-centered communication, goals-of-care discussions, hospice eligibility review, and interdisciplinary teamwork.
Key Responsibilities
• Provide comprehensive palliative and hospice assessments, exams, care plans, and interventions
• Conduct face-to-face hospice eligibility evaluations and reassessments
• Manage urgent and emergent medical needs for pediatric hospice patients
• Collaborate closely with physicians and interdisciplinary team members
• Lead or participate in family meetings and care goal discussions
• Support and educate hospice team members providing pediatric care
• Contribute to pediatric hospice education initiatives and curriculum development

AVP Care Coordination

Nuvance Health, now a part of Northwell Health, an award-winning non-profit health system, is seeking their next AVP of Care Coordination in Danbury, Connecticut!
The Position
- The AVP of Care Coordination will spearhead the strategic direction, management, continuous improvement, and overall day-to-day operations of the newly created centralized Denial Prevention Unit/Utilization Management team with dotted line reporting for local and entity-specific Case Management and Social Work departments.
- The AVP will strive to optimize patient outcomes, provided services, care plan development, complaint management, and length of stay metrics while guaranteeing alignment with best practices, hospital and system priorities, and regulatory requirements.
- Seeking a collaborative leader to energize teams and drive key initiatives around department structure, succession planning, and staffing throughout the organization.
- The AVP must be a passionate leader who will bring a level of urgency while upholding a high level of communication and collaboration with staff, physicians, and executive leadership to ensure delivery of evidence-based care.
- Ideal candidates will bring expertise in accreditation and regulatory guidelines, particulary the Medicare 2MN rule, Milliman and Interqual guidelines, to guarantee expectations are consistently met as well as relevant and demonstrable experience in collecting, analyzing, and aggregation of data to ensure that necessary action plans are being put into place.
- The ideal candidate will be a collaborative and highly visible leader with experience and desire to implement the best practices throughout Care Coordination while reporting to the Chief Revenue Officer.
- Preferred candidates will possess a robust background in Utilization Management, Physician Advisors, and Case Management and is able to demonstrate strategic thinking abilities backed by tangible evidence.
Requirements
- Bachelor's degree is required; a master's degree in nursing, health administration, and/or related field is strongly preferred.
- Active CT or NYS RN license is required.
- A minimum of five years of clinical experience within an acute care setting is required.
- A minimum of five years of progressive leadership experience in case management and/or utilization review is required.
Compensation Details
- The base salary range for this position is $200,000 to $220,000 annually. In addition, a leader may be eligible for other benefits, including but not limited to health insurance coverage, retirement benefits, and bonuses. The total compensation for the finalist selected for this role will be determined based on various factors, including but not limited to scope of role, level of experience, education, accomplishments, internal equity, budget, and subject to Fair Market Value evaluation. The base salary range listed above is a good faith determination of potential base compensation at the time of this job advertisement and may be modified in the future.
