Position Title: Care Manager (RN)

Work Location: Remote OH (North West - Lucas, Fulton, Wood, Ottawa)

Assignment Duration: 6 months (Possibility to extend or convert)

Work Schedule: Monday-Friday 8a-5p EST

Training Schedule: Monday-Friday 8a-5p - 4 weeks training - classroom & 1on1 - virtual - CAMERAS ON - NO TIME OFF during training

Work Arrangement: Remote (Field Visits: 50-60% of the time)

Occasionally (once a year) will need to travel to the Columbus Centene location for team meets.

Position Summary:

Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families.

Background & Context:

Everyone works together to ensure our members get the care and services they need to remain as independent as possible. This is part of a larger hiring initiative of 30 staff.

Key Responsibilities:

* Log on laptop by 8am, review emails, tasks, and voicemails and respond to any urgent needs.

* Review calendar for possible meetings and/or scheduled visits.

* Staff typically arrange their days with the expectation of 5-7 visits required weekly.

* Documentation must be completed within 24 hours.

* Performance expectations: 5-7 visits weekly, ensuring documentation is completed within 24 hours.

* Meeting required turn-round times for processes, and completing trainings timely.

Qualification & Experience:

* Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2 - 4 years of related experience.

* RN - OH (Not compact state).

* 2-4 years of experience nursing, case management, home health.

* Computer Literate (knowledge of Microsoft) Excel, Word, Team, Outlook, One note, One Drive, Powerpoint, Explorer, Chrome.

* Critical Thinker.

* Works well independently, troubleshooting.

* Someone who works well independently, able to travel occasionally for meetings/gatherings, personable, strong communication skills.

• Utilizes file maintenance to ensure newly arrived vaccines are entered in the I lot manager system as needed. Assures immunization clinics comply per health plan, federal and state guidelines - i.e., compliance with Health department site visits. Position Overview

We are seeking a highly skilled Visual Merchandising Freelance professional to support a large-scale retail network of 1,000+ store locations. This role is responsible for developing and executing monthly marketing and visual merchandising guidelines, as well as supporting store communications and in-store execution tools.

The ideal candidate will have a strong graphic design background and be an expert in Adobe Creative Suite, particularly InDesign (INDD), as they will be responsible for building monthly store tools and visual guideline materials. This position requires exceptional attention to detail, strong project management capabilities, and the ability to manage complexity across a diverse store environment.

Key Responsibilities

• Execute all marketing and visual merchandising guidelines using Adobe Creative Suite, with a primary focus on InDesign
• Build and update monthly store communication tools and materials
• Support prop strategies and space planning initiatives as needed
• Develop and maintain guidelines for all visual merchandising setups
• Assist with New Store Openings, Remodels, and Relocations (NRR), including creation of new guidelines when required
• Manage digital platforms for guideline uploads and distribution
• Ensure accuracy and consistency across all visual and marketing materials

Basic Qualifications

• 2–4 years of experience in a similar role, preferably within a multi-brand retail environment
• Strong background in graphic design and retail visual merchandising
• Advanced proficiency in:

Position Summary

Under the direction of the facility Coding and Reimbursement Manager, conducts coding quality reviews and audits of chart documentation to assess accuracy, ensure compliance with federal and payer policies, and identifies areas for improvement for hospital outpatient coding. Develops and delivers training on coding accuracy and compliance, staying updated on regulations and providing expert guidance to coders. Provides ongoing coding education and training to coding team and serves as mentor to all new coding team members. Serves as a subject matter expert and resource for coders, providers, and other staff on coding questions, regulatory changes, and best practice. Prepares reports of findings and meets with coders and Coding Leadership to provide education and training on accurate coding practices and compliance issues.

Has thorough knowledge of acute care facility guidelines, modifiers, sequencing rules and the NCCI (National Correct Coding Initiative) edits, OCE (Outpatient Code Editor) edits, Official Guidelines for Coding and reporting for ICD-10-CM/PCS, CPT-4, and HCPCS coding conventions, APC payment classifications and Medicare Conditions of Participation. Will assist the Coding and Reimbursement Manager on preparing presentations and/or interdepartmental feedback.

Responsible for conducting coding and billing training programs for billing and coding specialists. Other duties will include implementing coding department policies and procedures and assisting with reviewing and appealing coding denials.

• Communicates outcomes to the coding team to improve the accuracy, integrity and quality of patient data, to ensure minimal variation in coding practices and to improve the quality of physician documentation within the body of the medical record to support code assignments. Responsibilities also include assisting Coding Leadership in root cause analysis of coding quality issues, performing account reviews, and preparing training documents to assist with coding quality action plans.
• Assists in the review, improvement of processes, education, troubleshooting and recommend prioritization of issues. Researches coding opportunities and escalates as needed. Communicates Coding topics and/or question trends to Coding Leadership for global education.
• Prepares and presents coding compliance status reports to the Coding and Reimbursement Manager and Health Information Management AVP.
• Assists in ensuring coding staff adherence with coding guidelines and policy. Demonstrates and applies expert level knowledge of medical coding practices and concepts.

The Physical Therapist, School Based serves students with various physical, behavioral, and developmental challenges. Ages range from 3-21 years old. Age specific experience and/or special training and/or expertise is required to serve this population.

RESPONSIBILITIES/ACCOUNTABILITIES:

Implements treatment plans and programs through consistent daily and weekly schedules that focus on functional and measurable outcomes to be shared with the school and/or supervisors.

Delivers specialized individual and group physical therapy instruction as mandated in students' Individualized Education Plans (IEPs).

Collects data and completes reports regarding the student's goals and progress.

Consults with other professionals (Teachers, SLP, OT, PCP, etc) on the student's treatment team to ensure a cohesive and comprehensive therapy team.

Assesses and documents each student's progress through formal and informal tests and measurements.

Writes and prepares annual review reports and goals for each student in accordance with their IEP dates and requirements in order to remain in compliance with state guidelines.

Provides physical therapy expertise when developing FBA/BIP.

Provides parent and caregiver support and education to discuss the student's progress and strategies for continued support outside of school.

Attends all school related meetings as scheduled, such as, but not limited to, Triennials, Annual Reviews, Domain, Referrals, and pre-IEP meetings for each individual student.

Ensures a suitable learning environment by setting up and preparing materials and supplies as needed.

Supervises PTA(s) in accordance with state practice act guidelines, as needed.

Communicates and follows up with schools and families to obtain appropriate physician signatures/orders for school-based physical therapy referrals in order to initiate and continue with services.

Follows the safety guidelines and directives for the student in regards to functional mobility and orientation (i.e. transfers, wheelchair or walker mobility, etc.) and exercises provided by other related service providers (i.e. occupational therapist, vision itinerant, hearing itinerant, speech and language pathologist, orientation and mobility instructor, etc.).

Utilizes communication devices and assistive technology as outlined by the speech language pathologist and itinerants.

Follows any fire drill or safety guidelines outlined for each individual student.

Provides screenings and evaluations to determine requirements for skilled physical therapy intervention relative to student/client's level of function. This includes, but is not limited to the following areas: school function, range of motion, strength, positioning, contracture management, conducting home assessments, discharge planning, development of maintenance programs and other related client care issues.

Provides documentation to maintain the quality of therapy records, as it relates to reimbursement guidelines, accuracy of billing information, and school requirements.
