- Conducts designated interventions as defined in department Clinical Intervention activities (IV to PO, Renal Dosing, Pharmacokinetic dosing support, Anticoagulation)

A Brief Overview:

ChristianaCare is recruiting for a Pharmacy Technician to work full time, day/evening shift rotation. Every other weekend required; holidays as required.
Principal Duties and Responsibilities:
- Assist the pharmacist in providing timely, efficient, cost effective, and appropriate pharmaceutical care to the patient
- Refill patient medication bins through robot cart fill and manual picks; update carts by including new orders, removing discontinued medications, and transferring medications. Update labeling of bins on admission, transfer, and discharge of patients
- Deliver and exchange 24-hour cart fill to nursing units at scheduled times. Check medication storage areas for discontinued and discharged patient medications; ensure that returned medications are placed back into stock
- Reorder medications in short supply and restock the picking station and other areas of the pharmacy
- Send prepared medications via pneumatic tube system. Any medication that cannot be tubed is delivered to the appropriate location on the nursing unit
- Manage automated dispensing cabinet inventory, including accurate selection and loading of medications
- Ensure that medications are stored properly and that only in date medications are stored. Remove discontinued medication
- Receive and check incoming supplier order(s)
- Prepare IV medications using aseptic techniques and in following processes in full compliance with USP 795, 797, and 800 federal regulations
- Use appropriate Personal Protective Equipment (PPE) and closed system transfer devices (as needed) for the preparation of chemotherapy and other hazardous medications
Education and Experience Requirements:
- High School Diploma required
- Pharmacy Technician Certification Board (PTCB) certification required unless enrolled in a PharmD program.

Summary:
The 340b Program Coordinator is responsible for the oversight of 340b program operations at CHRISTUS Health acute inpatient facilities that qualify for program participation. - Assist team members with tasks to ensure department runs efficiently.
- Use the pharmacy computer to complete all actions necessary in order to maximize efficiency and provide optimal patient care at drop-off, will call, drive thru, phone, filling and order receiving, including but not limited to: searching for patients and profiles, determining dates of fill and pick-up, determining prescriptions sent to Central Fill, quoting cash prices of prescriptions, checking on-hand counts of medications and entering invoices.
- Assist team on actions plans in order to improve outcomes using knowledge of all appropriate service and business metrics.
- Assist in the execution of all company initiatives and programs.
- Complete required training program and accompanying exams.
- Follow all company, state and federal policies, laws and regulations, including HIPAA and Pseudoephedrine sales.
- Finalize sales using cash register according to established procedures, including the point of sale policies.
- Transcribe data from prescriptions accurately in order to generate a custom label for the customer.
- Assist with maintaining proper inventory levels by ordering, receiving, stocking and rotating drugs and other supplies per company policy and procedure.
- Answer telephone in a polite and timely manner, ensure accurate information is exchanged and the customer is satisfied.
- Resolve customer questions and concerns by communicating effectively with the customer and the management team and support staff in an efficient manner.
- File written prescriptions daily.

Summary:
All pharmacists fill orders for drugs, monitor patient drug therapies and provide drug information. Pharmacists provide pharmacy services with compassion, excellence, and efficiency.

Operational Duties and Responsibilities
Prepares and dispenses drug orders per physician request according to established policies, procedures, and protocols.
Interprets drug orders (verbal and written) and transcribes/verifies into computerized patient medication record accurately. Maintains, accurate, complete patient drug record.
Compounds and dispenses pharmaceuticals including non-sterile and sterile products, chemotherapy and parenteral nutrition products accurately.
Issues controlled substances to patient care areas and maintains records as required by law and institutional policies.
Provides services efficiently and in a timely fashion.
Maintains current pharmacist license. Ensures applicable CE records and licensure are maintained in department files. Attends staff meetings.
Completes all competency/skills assessment requirements.
Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians
Ambulatory care Pharmacist: Provides ambulatory pharmaceutical care services utilizing approved protocols and/or collaborative agreement with other providers, including managing patient drug therapy, anticoagulation management, providing patient education, and preparing and/or overseeing the preparation and dispensing of medications. Performs duties in the clinical areas assigned such as but not limited to anticoagulation clinics, intensive medical home, transition of care, Discharge Medication reconciliation, etc.
Clinical Duties and Responsibilities
Ensures safe, appropriate, cost-effective drug therapies for patients according to established policies, procedures, and protocols.
Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
Assists with pharmacokinetics consult service and renal drug dosing per hospital protocol.
Reads, extracts, and interprets information in patient medical records accurately.
Detects and reports suspected adverse drug reactions accurately and in a timely manner.
Sustains the hospital drug formulary, minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.
Provides clinical consultation and clarification to practitioners as appropriate.
Provides accurate, adequate, and timely drug information to the hospital’s professional staff.
Provides drug education to patients and their families per institutional protocol.
Participates in the quality improvement and medication use review activities of the department. Collects data, conducts monitors and inspections, and maintains logs, records and other documentation as assigned.
Conducts designated interventions as defined in department Clinical Intervention activities (IV to PO, Renal Dosing, Pharmacokinetic dosing support, Anticoagulation). Baseline departmental standards for reporting interventions for clinical productivity as defined by departmental goals.
Participates in antibiotic stewardship, opioid stewardship, and shortage management activities as assigned.
Attends interdisciplinary rounds when assigned.
Provides services efficiently and in a timely fashion.

Job Requirements:
Education/Skills
~ Doctor of Pharmacy or BS Pharmacy degree required
Experience
~ Hospital experience preferred
Licenses, Registrations, or Certifications
Pharmacy state licensure required within 60 days of start date
BLS or ACLS is required
PALS (for pediatric practice) is required
In accordance with the CHRISTUS Health License, Certification and Registration Verification Policy, all Associates are required to obtain the required certifications for their respective positions within the designated time frame.
