The Speech Language Therapist or Clinical Fellow will be responsible for the administration and direct application of Speech-Language pathology treatment modalities as prescribed by a referring physician. If a Clinical Fellow, they will be supervised by an onsite Speech Language Pathologist as per protocol until they obtain their SLP license following the completion of the Clinical Fellowship. Duties include evaluating, planning, directing, and treating all patients referred to Speech-Language Pathology. In this role, you will assist patients to obtain maximum performance and function in society while learning to live a routine "normal" life within the constraints of their disability. This position may also provide coverage at other affiliated locations as needed.
Primary Responsibilities:
* Evaluate and treat patients requiring speech therapy services in Acute Care
* Evaluate and treat patients requiring speech therapy services in Skilled Nursing
* Evaluate and treat patients requiring speech therapy services in Outpatient
* Documents appropriately in the medical record for all speech-language pathology treatments, assessments and evaluations performed for inpatient and outpatient care
* Submits timely reports to the ordering Physician and updates the Rehabilitation Director daily and /or weekly on ST caseload that is being treated
* Supports and maintains a culture of safety and quality by performing all aspects of patient care in an environment that optimizes patient safety
Location: Hillsdale Hospital in Hillsdale, MI
Schedule: This full time, exempt position will work 40 hours/week, Monday-Friday, 8:00am-4:30pm, in the hospital (skilled nursing unit and inpatient) and outpatient as needed.
Rehab Department Hours: Operating hours for the hospital are Monday-Friday, 8:00am-4:30pm, and Saturday, 8:00am-4:00pm. Outpatient hours are Monday-Friday, 7:00am-6:00pm. Software systems utilized include Agile (Outpatient), Brightree/Matrix (Home Health), NetHealth (SNF), and CPSI (Inpatient).

In this role, you will work as a Speech Language Pathologist with our Pediatric population.
Key Responsibilities:
- Provides therapy evaluation, treatment planning, treatment, patient/caregiver education, and discharge planning for a full caseload of patients
- Provides all ancillary/administrative requirements for a full caseload of patients, including documentation, billing, scheduling management, and other administrative duties
- Demonstrates appropriate communication, professionalism and supervision of support staff (licensed assistants, aides, volunteers) and complies with all organization policies.

Schedule is generally Monday - Friday day-shift. This role includes a weekend (Saturday and Sunday) on-call rotation approximately once every 4-6 weeks. On-call hours are generally in the mornings, averaging 4-6 hours per day based on census.

* Contribute to the improvement of Special Education Programs by participating in specialized teams developed by the BOCES.

Essential Job Functions / Responsibilities
Evaluation & Assessment
1. Perform comprehensive speech, language, voice, swallowing, and cognition assessments in the patient’s home.
2. Review medical records, labs, diagnostics, and collaborate with nursing to incorporate patient history.
3. Evaluate the home environment to support therapy success (safety, assistive devices, caregiver support).
4. Identify patient, family, and caregiver needs, preferences, and goals.
Treatment Planning & Implementation
1. Develop individualized, measurable therapy plans aligned with patient goals, family input, and physician orders.
2. Provide direct skilled therapy (speech, language, swallowing, cognitive) to patients.
3. Adjust therapy plans based on patient response and progress.
4. Instruct and train caregivers on carryover strategies and home exercises.
5. Coordinate with an interdisciplinary team (nursing, OT, PT, CNAs) to align therapy with overall care plan using google workspace applications and EMR system, known as HomeHealthMD .
Documentation & Compliance
1. Complete evaluations, treatment notes, progress notes, re-evaluations, and discharge documentation per agency policy and regulatory timelines (48 hours).
2. Document all therapy contacts, missed sessions, cancellations, and outcomes accurately.
3. Record therapy elements that support billing, justification, and compliance.
4. Communicate changes in patient status or plan to physicians and care team.
Communication & Collaboration
1. Participate in care team meetings, case discussions, and interdisciplinary conferences.
2. Report therapeutic findings, recommendations, and concerns to physicians, nursing, and leadership.
3. Provide clear, respectful, and timely communication with patients, families, caregivers, and staff.
Additional Duties
1. May cover on-call therapy as needed according to agency policy if both parties agree.
2. Assist in scheduling, travel coordination, and therapy resource management.
3. Maintain continuing education and fulfill licensure / competency requirements.
Licenses & Requirements
1. Valid Speech-Language Pathologist license in California
2. Minimum 1 year of experience in home health therapy, outpatient, hospital, or similar setting preferred
3. Certification with ASHA (CCC-SLP) or relevant credential
4. Current CPR / BLS certification
5. Reliable vehicle, valid driver’s license, and appropriate insurance
