In this program the Psychiatrist and the Care Manager meet with each of the primary care providers each week for 30 minutes to discuss each of their patients who are up for review.
The psychiatrist and the primary care provider work together to make recommendations on medications and the care manager follows up with the patient.--Program 2: the patient can be referred without a medical condition and only a behavioral health condition that meets criteria for collaborative care.
In this model the psychiatrist meets weekly with the Care Manager without the primary care team.
Both programs have been well received by our primary care physicians.--Most of the work week would be in the psychiatry department.
