POSITION SUMMARY:
Works with interdisciplinary team to deliver high-quality, compassionate care to patients presenting to the Emergency Department. Must be self-motivated and flexible to meet the demands of the specialty

Must be able to communicate effectively with all members of the healthcare team, patients and family and demonstrate excellent interpersonal and organizational skills

Clinical management in collaboration with the physicians in the outpatient offices as well as in partnership with hospital-based APPs and physicians
Strong clinical background and the ability to diagnose, treat and manage complex cardiovascular care including acute and chronic disease, medication management and other therapies

The Advanced Practice Providers (APPs) are part of a multidisciplinary team that plans and delivers care for critical neuroscience patients with a focus on those with neurological and neurosurgical diseases and related disorders. Our critical patient population includes complex strokes, skull base surgery, status epilepticus, tumor resections, spinal cord injury, DBS, and other complex diseases. Nurse Practitioner and Physician Assistants Role Scope:
Under established guidelines and in collaboration and/or supervision with the practice physician(s), Advanced Practitioners perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.

Seeks consultation with the Wound Team RN and WOCN staff for patient problems/issues that require a change in the plan/interventions designed to solve complex problems involving client care.
Participates in the interdisciplinary approach to client care with Wound program/Agency staff. The Wound Program LPN will perform both Wound care and medical/surgical services Care required by the patient to achieve quality outcomes. Informs the Wound Team RN, CWOCN and the physician/wound clinics of changes in client needs, wounds that require a change to the plan of care for wound care treatments and disease management
Assists with staffing needs to meet service demands.
Accountable for team performance in achieving desired clinical and operational performance measures.
Follows trends in specialty area of wounds for home care industry; makes recommendations of treatment options for incorporation into the program.
Participates in training, coaching of staff or onboarding to the Wound program
Participates and facilitates professional development needs and competency for wound program.
In addition, the position involves other duties such as:
1. Will actively participate in the performance improvement process known as H3W.
2. Participates in clinical education area to provide programs aimed to improve clinical competencies of direct service staff.
3. Research new information related to wound specialty and communicates to team.
