Job Description

Under provider direction, the RN provides skilled nursing care to patients in a variety of clinical settings. Scope of responsibility is characterized by use of nursing process to assess, plan, intervene and evaluate human responses to actual or potential health problems utilizing appropriate practices, standards, protocols and guidelines. This position reports to a Practice Manager.

  • Provides office-based nursing care in collaboration with provider
  • Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care through appropriate nursing evaluation and intervention
  • Communicates with provider regarding patient needs, nursing assessments, and recommendations
  • Takes direction and initiates actions (cross/additional training) that will allow seamless patient care
  • Collects and documents current health status and medical history and assists in scheduling patient office visits
  • Reviews patient records, including preoperative records, to ensure completeness and reviews outstanding issues with provider
  • Demonstrates independent nursing actions based on assessment and problem identification
  • Under direction of provider, calls in prescription medications to pharmacies and observes and documents patient’s response to pertinent medications
  • Documents accurate and ongoing assessment of patient’s needs, problems, capabilities, limitations, progress toward goals, and response to care
  • Triages telephone calls and displays ability to initiate appropriate nursing intervention in prompt, precise, and professional manner
  • Assists patients and family members or other clients with concern and empathy; respects confidentiality and privacy

The Emergency Department at Hartford Hospital is one of the busiest EDs in Connecticut, treating patients of the highest acuity in the region.Β  Comprised of 108 licensed beds, with an additional 19-bed ED Observation Unit, HHED treats an average of 108,000 patients annually.Β  The ED works closely with the 7 Medicine inpatient units within Hartford Hospital to promote a collaborative continuum of care for our patients as they transition through their care within the hospital.Β  Our Medicine inpatient units care for a diverse patient population, allowing for RNs to gain a broad range of experience within the nursing field.


RN Bridge Program


A Medicine-to-ED Bridge Program has been established to provide nurses with potential future interest in working in an Emergency Department an opportunity to first build foundational nursing skills and knowledge within one of our Medicine units.Β  Upon completion of orientation on a Medicine unit, and successfully demonstrating competence in practice for the first 12 months, the RN will then begin a gradual transition to the ED setting with support of leadership from both the Medicine and ED teams.


Program components:



  • Creation of individualized development plan with managers of both Medicine unit and ED to track goals and progress
  • Collaborative coaching sessions with each RN and managers of both units at regular intervals through the program
  • Experienced RNs from both the Medicine unit and the ED assigned as mentors
  • Transitional orientation to care for inpatients in the Emergency Department
  • After completing each step of the bridge program, RNs will be evaluated for readiness to transition fully to the ED

The Medical Imaging (Radiology)Β RN Clinician will be responsible for representing ECHN to the Radiology Intervention patient in both the outpatient and inpatient setting.Β  The Radiology nurse will report directly to the Manager of Vascular and Cardiology Services and indirectly to the Team Leader of Interventional Radiology. The Surgical-Trauma Intensive Care Unit is a 14-bed unit. As a Level 1 Trauma Center, the mainstream patient population includes motor vehicle trauma, orthopedic injuries, vascular surgical, abdominal surgical, liver and kidney transplant, and extensive esophagectomy patients. Under the supervision of the rounding Surgical-Trauma Intensivist, the unit has a team of Physician Assistants, Advanced Practice Registered Nurses, Registered Nurses and surgical house staff physicians consisting of interns and residents.

Job Summary

The Nursing Manager is a Registered Nurse (RN) who has responsibility for the supervision of patient care, clinical personnel, and unit operations. This position supervises a work group to ensure that a high quality, cost-effective patient and family centered care is provided in support of the hospital's strategic plan, and the function's goals and objectives.

This position works collaboratively with functional leadership and is responsible for the oversight of daily operations of the assigned department including off shift and weekend supervision of nursing and non-licensed staff. This position manages resources, quality improvement, payroll, creating and maintaining colleague schedules and monitors patient throughput to facilitate efficient clinical operations. In addition, this position is responsible for a safe and therapeutic unit environment, which includes working in the department as staff RN to assist as needed.

This position supports leadership in ensuring standards of excellence and professional development are fostered and met. This position also participates in and may lead Performance Improvement work in their assigned department.

  • Current Connecticut Registered Nurse license required.
  • Bachelor's degree required.* Master's degree preferred. One degree must be in Nursing.

Strong clinical reasoning and excellent communication skills required. This role functions both independently and in collaboration with nurses, residents, and attending physicians. Β This position will report to the OB/GYN Hospitalist Director.Β 

Advanced Practitioner Role Scope:β€―β€―β€―β€―Β 

  • Care for patients admitted or being triaged across our Obstetric service line.Β 

  • Help on Labor and Delivery with admissions, magnesium sulfate checks and labor checks when residents unavailable, during sign-out and periods of higher volume.Β 

  • Rounding and evaluating our antepartum patients as needed based on volume and resident coverage.Β  Evaluation may include concerns for labor, elevated BP’s, infection or medication orders. Help with biophysical profiles (BPP’s) for non-reactive NST’s and limited obstetric ultrasoundΒ Β 

  • Typical postpartum care includes rounding, discharge planning, coordination of inpatient consults, responding to elevated blood pressures, postpartum or postoperative concerns, medication reconciliation and placement of Nexplanon.Β Β 

  • If needed in triage you will evaluate pregnant patients for pre-term/term labor, rupture of membranes, bleeding, high blood pressure, pre-eclampsia, abdominal pain and a variety of other complaints. β€’ Manage a caseload of patients receiving a variety of infusion and oral therapies on an outpatient basis.
β€’ Demonstrates high level skills to manage complex workload effectively to provide safest, high quality care.
β€’Demonstrates an excellent ability to communicate through various methods and with various audiences to achieve desired outcome(s).

Collaboration with teams is key to identifying trends and developing action plans to address them. Participation in quality and performance improvement initiatives is expected, along with supporting the agency’s compliance with data submission requirements for regulatory and payor standards.

JOB RESPONSIBILITIES

Reviews OASIS assessments for accuracy, completeness, and regulatory compliance, providing recommendations to ensure quality and reliability.

Guides staff on clinical, documentation, and utilization decisions to improve OASIS accuracy and care quality, recommending cost-effective corrections that support clinical and operational goals.

Reviews visit utilization for appropriate patient care, collaborates and consults clinicians on needed adjustments, and reports utilization concerns and financial risks to the clinical manager. Ensures timely completion of HCHB workflows and maintains a flexible schedule to support agency needs.

Collaborates to identify documentation trends, supports improvement planning, participates in quality initiatives, and helps ensure compliance and accurate data submissions across all settings.



Qualifications

Degree from an accredited educational program leading to licensure as a Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP). Required

Bachelor's degree in nursing, master's degree in PT, OT, SLP preferredΒ

The Float Pool at Hartford Hospital, also known as the STAR Team, is comprised of experienced, highly skilled Nursing professionals. Registered Nurses on the STAR Team have a variety of skills, allowing them to transition from one specialty to another with confidence and ease. Our team is broken down into two groups, Med/Surg and Critical Care. The Med/Surg team floats between approximately 18 units (including the Step Down units) while our Critical Care RNs primarily float between the six ICUs at Hartford Hospital (MICU, SICU, Cardiac ICU, Neuro ICU and Cardiothoracic ICU). Our RNs rely on extensive nursing skills, strong communication and collaboration skills to assimilate seamlessly into different units and provide the highest levels of personalized, compassionate care to our patients.
