Field Care Manager – Behavioral Health
Become part of our client’s compassionate community and help put health first. As a Field Care Manager – Behavioral Health, you’ll be the steady hand and the guiding voice for members navigating behavioral health challenges. You’ll assess needs, craft care plans with care, and connect individuals to the resources that genuinely move the needle.
This role is remote with 25% local travel to visit members in Wayne and Macomb Counties, Michigan.
What You’ll Do
- Manage a caseload and conduct behavioral health assessments in members’ homes, community spaces, or by phone.
- Develop and coordinate individualized care plans that address the full picture: physical, social, and behavioral.
- Partner with interdisciplinary care teams (ICT), physicians, and caregivers to ensure members receive consistent, appropriate support.
- Help members break down barriers to care—whether that’s housing, transportation, benefits access, or simply knowing where to start.
- Serve as the member’s anchor: the main point of contact and advocate for steady, sustainable health outcomes.
Plus:
- 2+ years of clinical behavioral health experience.
- Experience working with adult populations, care coordination, and community health resources.
- Strong communication, documentation, organizational, and computer skills (MS Office proficiency required).
- Ability to travel locally (25%) throughout Wayne or Macomb County.
- A private home workspace with secure internet access — quiet, steady, reliable.
Preferred
- Master’s level Social Work license (LMSW/LCSW).
- Experience in managed care, Medicaid Long-Term Care, or in-home care coordination.
- Language skills in Spanish, Arabic, or Chaldean Neo-Aramaic.
- Familiarity or certification in Motivational Interviewing.

About the Job:
The Estimator will provide and direct full estimating services in the company’s General Contracting/ Construction Management division and its Self-Perform Concrete group when needed. Estimating services will be for smaller sized projects as defined by Executive Management as well as be part of the estimating team on larger projects. The estimator will be required to perform and direct all duties on lump sum and conceptual formats.
Primary Responsibilities:
- Will lead all aspects of each project assigned including but not limited to:
- Will utilize and maintain project estimating templates.
- Will set up each estimate and perform “accurate” in-house estimates for all or applicable trades of work, temporary facilities, winter/summer conditions, etc.
- Will engage, direct, and interact with subcontractors and suppliers in all trades.
- Will review all project documents including but not limited to specifications, geotechnical reports, drawings, contract & general conditions to the contract, schedule, etc.
- Understand bond and insurance requirements for each project and interact with the accounting department.
- Interact with project supervision for schedule, constructability, site usage, and site logistics.
- Interact with project management regarding schedule, constructability, staffing and site usage.
- Conduct fee analysis, develop general conditions, and staffing plans.
- Develop strategies to understand project challenges and secure project awards.
- Interact with the Self-perform concrete group to coordinate to perform takeoffs, understand scope and secure supplier proposals.
- Review and understand Contract and General Condition requirements of each project and interact with Colasanti General Counsel.
- Constant interaction with subcontractors for scope, current & forecast pricing, product availability, and development of relationships.
- Understand project safety requirements and interact with Corporate Safety.
- Maintain, review, and incorporate historical data in consideration of estimates.
- Develop subcontractor bid lists.
- On lump sum bid projects develop a bid strategy to secure work.
- Develop project schedules and analyze schedules created by others.
- Develop detailed and complete scopes of work for subcontractor bidding, Purchase Orders, and Subcontract execution.
- Develop relationships with project Owners, Design professionals and or General Contractors/CM’s (if applicable) to ensure repeat business.
- Draft purchase orders and subcontracts and ready all trades for awards.
- Perform productivity analysis, unit pricing and man-hour analysis in consideration of each estimate across all trades.
- Prepare proposals and clarifications for contracts with the Owner.
- Attend pre-bid meetings, walk the site, and conduct pre-bid meetings as necessary.
- Provide technical support to in-house and subcontractor personnel assisting and preparing discipline estimates for assigned projects.
- Ensure all working documents and data are maintained to back-up estimate costs.
- Evaluate all bids received from suppliers and subcontractors for completeness and identify missing scope.
- Lead and conduct post bid review meetings with all trades of work in partnership with the Colasanti project team members.
- Perform construction system cost analysis and value engineering.
- Initiate Committed Cost Reports for each project and maintain through the life of all project awards.
- Assist project team members during construction on scope and pricing efforts when requested.
- Perform additional assignments per supervisor’s direction.
- Lead and perform project handoff meeting to operations.
Qualifications / Skills:
- Has experience with Estimating or Project Management and has comprehension of how to read drawings/specifications, conduct takeoffs, quantity summarization, and knowledge of CSI divisions.
- Possess the working competency with the following software: Microsoft Office suite of products (Word, Excel, PowerPoint, Outlook, Teams, Project), Bluebeam, Procore, Egnyte, Building Connected, ProEst and Onscreen Takeoff.
- Strong analytical and problem-solving skills. Highly organized. Excellent communication skills including an excellent command of English (speaking, reading, writing).
- Knowledge of communicative behavior and lead discussions, negotiations, provide feedback, manage conflicts, and provide a positive, motivating environment for the project team.
- Self-motivated, willing to learn. Strong leadership and team-player can facilitate effective outcomes through others.
- General understanding of US (United States) regulations, local and national codes, legislation, and industry trends.
- Position requires the ability to work in an organized manner in a fast-paced environment and manage time to meet deadlines.
- Working conditions vary from that of a dry, well lite, and tempered office only environment to working onsite in a construction portable office that may include loud noise and extreme temperatures.
- Can commute to work in the Macomb office, Detroit office or on a project jobsite(s). In rare occasions, based on availability of personnel and project requirements, executive management may determine a hybrid management approach may be in the best interest of the company.

Gestational Carrier Requirements
To ensure a safe and successful journey for both carrier and baby, applicants must meet the following criteria:
- Between 21–39 years old
- U.S. citizen living in a surrogate-friendly state
- Have delivered at least one child and are currently parenting
- No more than two (2) C-sections
- Not receiving government or public assistance (including Medicaid, WIC, SNAP/Food Stamps, or state-funded health insurance)
- No history of pregnancy complications (including gestational diabetes, pre-eclampsia, or pre-term labor)
- No history of schizophrenia, bipolar disorder, borderline personality disorder, or major depressive disorder
- Not currently taking medications unsafe for pregnancy (including certain mental health medications)
- Able to travel for medical screening, monitoring, and embryo transfer appointments
- Live a healthy lifestyle free of illicit or recreational drug use
- Have a stable home environment and strong support system
All qualifications will be reviewed in detail during your initial consultation.
Compensation & Benefits
We recognize the commitment, time, and care involved in being a gestational carrier. Compensation includes:
Base Compensation
- $60,000 – $70,000(First-time carrier to experienced carrier)
Additional Benefits
- $250 monthly expense allowance
- $500 maternity clothing allowance
- $10,500 for multiple birth
- Up to $10,000 for loss of organs (per contract terms)
- Lost wages (carrier and spouse, if applicable — based on employment verification)
- Travel reimbursement
- $200 per week for housekeeping (if medically necessary)
- $20 per hour for childcare (if medically necessary)
All medical expenses, legal representation, and services related to the surrogacy journey are covered.

In this role, the RN Case Manager will conduct comprehensive member assessments, develop individualized care plans, and collaborate with providers and care teams to promote optimal, cost-effective health outcomes.
The position focuses on managing member needs through clinical review, care coordination, and patient engagement.
Key Responsibilities Conduct comprehensive telephonic assessments of member health needs and eligibility using clinical tools and data review.
Develop, implement, and monitor individualized care plans in collaboration with members and interdisciplinary care teams.
Coordinate care and services based on member benefit plans and available internal/external resources.
