Essential Functions & Responsibilities
- Provider is responsible for rounding and managing the clinical care of the population at the assigned SNF.
- The provider is responsible for rounding daily and prescribing medications and treatments for residents residing at SNFs.
- Provider will provide high quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
- Provider is responsible for timely documentation of all patient care/encounters
- The provider will be available for phone/secure messaging consultations with facility nursing staff.
- Provider documentation is completed in Theoria's proprietary electronic health record (EHR) system, ChartEasy?.
- PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems.
- Provider is expected to be on-site on and on time for their in-person scheduled shifts.
- Timely attendance is imperative.
- Reliable transportation required.
Shift Structure
- Rounding time is 7AM-1PM local time, during which the provider must be available to nursing staff and address acute/emergent issues that arise.
- Shifts begin around 7AM local time with flexibility within reason.
- Shifts are daily Monday-Friday
- Charting/documentation can be completed off-site.
- This is an in-person position.

Saint Joseph Medical Group (SJMG) is seeking a compassionate and mission-driven Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing Gastroenterology Care team in North Central Indiana. Saint Joseph Medical Group is the employed arm of Saint Joseph Health System (SJHS). The Medical Group includes over 20 practice sites with over 100 primary care and specialist physicians in St. Joseph, Marshall, and Elkhart Counties. This position will be employed by Saint Joseph Regional Medical Center, a regional health system of Trinity Health and an affiliate of Loyola Medicine.
The ideal candidate would be seasoned in GI and have an interest in program development. The selected provider will take part in seeing patients in the outpatient and inpatient setting.

ResponsibilitiesAs an APP at The South Bend Clinic, you will get the opportunity to:
- Perform patient and examinations and treatment and records physical exams, analyze data, develop care plans, and treat minor illnesses and injuries per established protocols. Assist with procedures or conduct invasive/non-invasive procedures within scope of practice
- Orders laboratory, x-ray, and/or other non-invasive diagnostic tests and procedures. Initiate collaboration with supervising physicians, referrals to appropriate physicians or other health care professionals of problems that exceed the NPs scope of practice. Finish a communication to referring provider or supervising physician.
- Prescribe medications within scope of practice, with collaborative physician oversight and necessary mid-level practitioner-controlled substance license
- Instruct patients and families on healthcare, illness prevention, and home care, adjusting communication to the patient's age, psycho-social, and cultural needs to achieve optimal health outcomes in accordance with practice protocols
