• Is aware of and develops a mechanism or process for knowing the specific situation of each patient, including hospitalizations,
no- shows, catheter use, and any significant change in patient care status.
• Develops action plans for unexcused and missed treatments in collaboration with the Director.
• Plans, coordinates and acts as the liaison for patient care as per the disease management agreement, including initial and
ongoing validation of member eligibility. Facilitate timely workup of patients for access management, dialysis services, patient
