Position Summary: Reports to Pharmacy Department OR Emergency Services
As an integral member of the Emergency Department team the Medication History Specialist (MHS) is responsible for working with patients who have been identified for admission to the hospital in order to obtain and document the best possible medication history. The MHS role is essential to assure high quality and safe care by reducing medication duplications, ommissions and possible errors. The information obtained will be accurately entered into the computer system and it will be used by the physicians, pharmacist, nurse and other healthcare providers as an input into treatment plans and medication orders during hospitalization, and in discharge planning for post hospital transition and care.

Must be capable of passing the medication reconciliation competency test for Pharmacy Technicians. Must be able to pass annual competency and quality assurance metrics.Interview and inquiry techniques, with the patient, family, pharmacy's) to compile the best possible medication history. Responsible for entering all home medications into the electronic medical record. Monitor the tracking board for potential admissions and highly complex patients to complete the medication history to assist in the delivery of quality care.Upon notification of patient admission responsible for updating home medication record in electronic medical record to match ordered and or active prescription medications. Possess service excellence skills, setting patient and family at ease, demonstrating, patience and compassion at the bedside. Will work under direction of Pharmacy Leadership, ED Leadership, Medication Safety Officer, and nurses as needed to resolve any discrepancies/errors in home medication orders.

Position Requirements: High school diploma or GED and 2 or more years of experience as a Pharmacy Technician. Typing skills, basic computer skills, good oral communication skills, good 'problem solving' skills, good interpersonal skills, ability to lift a minimum of 30 pounds, ability to push a transportation cart of approx. 100 pounds. - Sends all studies to PACS immediately after completion, prints CDs as needed.
- Studies that have been reported after hours by teleradiography are printed and scanned into PACS, if applicable.
- Completes all studies in RIS before completing in PACS.
- Prints completed log sheet from teleradiography and submits to Radiologist, if applicable.
- Performs and maintains quality control logs for probe disinfection and follows infection control policies.

Operations

- Restocks linens and supplies, and maintains the department in clean and orderly condition.
- Transports patients, positions patients, and helps them on and off procedure table.
- Performs patient charging activities on the day the service is rendered.
- Applies supportive and immobilization devices and equipment if needed.
- Willingly lends knowledge and assistance to others upon request as needed and routinely as time is available, without prompting, so that all working time is used for departmental advantage.
- Performs routine duties of clerical and support personnel in accordance with procedure when warranted to ensure continuity and quality of services.

About the Job

- Perform physical examination and establish medical history to determine donor suitability

- Build rapport with donors to ensure overall customer satisfaction

- Ensure donor and staff confidentiality

- Responsible for donor awareness to potential hazards

- Provide donor education regarding general health and provide counseling regarding unacceptable test results

- Evaluate & manage donor injuries and adverse events

- Perform evaluations of any history of illness or medications to ensure continued donor suitability

- Assist in employee training

- Administer employee Hepatitis Vaccine program

Job Requirements

Educated and currently certified/licensed in the state of employment and according to state requirements as a Registered Nurse. HS diploma or GED. Current CPR certification required.

Attributes:

-Work is performed both standing and sitting for up to 2 to 4 hours per day each.

-The position does require bending and twisting of neck up from 1 to 2 hours per day.

-Frequent hand movement of both hands with the ability to make fast, simple, movements of the fingers, hands, and wrists.

-Ability to make precise coordinated movements, of the fingers to grasp and manipulate objects.

- Frequent foot movement; may squat, crouch or sit on one's heels on rare occasion. Occasionally walks, bends and twists at waist.

Job Summary:

Registered Nurses provide quality nursing services by working with the patient care team to assess, plan, implement, and evaluate patients' care and by providing a positive, supportive environment. The Registered Nurse is the front-line
representative of the patient care team often serving as the intermediary between the patient and family, the provider(s), and other care team members. Core Responsibilities and Essential Functions:

Patient Care and Care Coordination: Monitors, examines, and evaluates the patients conditions. Assists with the care plan and takes appropriate actions to promote the patients short/long-term outcomes and wellbeing. Delivers care plan as outlined, applying independent judgment where appropriate. Collaborates with and communicates patient care plans with other staff, as appropriate. Prioritizes and coordinates optimum daily patient flow and patient experience. Delivers safe and effective care according to policies and procedures. Maintains appropriate documentation of patient assessment, changes in condition, care delivered, education provided, and any incidents or exceptions related to standards of care or compliance with hospital policy.

Patient Advocacy and Satisfaction: Listens to concerns, probes in-depth to understand the needs and concerns, and responds to patient needs in an appropriately timed manner. Serves as an educational resource for patients and families on health-related issues. Communicates necessary patient information to family, licensed care providers, and other staff. Prioritizes actions according to patient needs and provides input into the plan of care by reporting pertinent information involving the patient and family to the appropriate individuals in accordance with confidentiality standards.
