Job Description:
1. Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
2. Regularly re-evaluates patient nursing needs.
3. Initiates the plan of care and makes necessary revisions as patient status and needs change.
4. Uses health assessment data to determine nursing diagnosis.
5. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
6. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct patient care as defined in the State Nurse Practice Act.
7. Counsels the patient and family in meeting nursing and related needs.
8. Provides health care instructions to the patient as appropriate per assessment and plan of care.
9. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
10. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
Education and/or Experience:
- Graduate of an accredited school of nursing. Minimum of one-year experience in a clinical setting.
- Current licensure in California and CPR certification.
- One (1) year of home health care experience preferred.

The Angio/Interventional Radiologic Technologist assists in performing minimally invasive, image-guided vascular and non-vascular procedures under the supervision of an interventional radiologist or other qualified physician. Key Responsibilities:1. Procedure Support & Imaging:
- Assist physicians in performing interventional radiology procedures, including angiography, stent placements, embolization, biopsies, and drainage procedures.
- Operate and maintain specialized imaging equipment such as fluoroscopy and digital subtraction angiography (DSA) systems.
- Ensure high-quality imaging while minimizing radiation exposure to patients and staff.
2. Patient Care & Safety:
- Prepare patients for procedures by explaining the process, obtaining consent, and positioning them appropriately.
- Monitor patient vital signs and respond to any emergencies during procedures.
- Maintain a sterile field and adhere to infection control protocols.
3. Equipment & Inventory Management:
- Prepare and set up sterile trays, guidewires, catheters, contrast media, and other necessary equipment.
- Perform routine maintenance and quality checks on radiologic equipment.
- Ensure appropriate inventory levels of interventional supplies.
4. Documentation & Compliance:
- Accurately document patient history, procedural details, and post-procedure observations in the electronic health record (EHR).
- Adhere to hospital policies, radiation safety standards, and HIPAA regulations.
Qualifications: Education & Certification:
- Associate’s or Bachelor’s degree in Radiologic Technology or a related field.
- Current ARRT (American Registry of Radiologic Technologists) certification in Radiography (R) required.

Primary Responsibilities:
- Maintain minimum billable hours of an average of 5 hours/workday (25 hours per week/ 110 hours per month).
- Supervise your Program Manager team to ensure minimum billable of 28 hours per week (120 hours per month).
- Provide quality clinical supervision in accordance with BACB standards to ensure treatment integrity and quality assurance across clients, settings and funding sources to include: direct observation and protocol modification, as needed.
