Nurse Practitioner, Nurse Educator Diabetes
Key Responsibilities:
- Coordinating patient self-management initiatives in a family centric program for pediatric and adolescent patients in Boston Children’s Hospital’s Division of Endocrinology’s ADA recognized outpatient diabetes self-management education program. The program uses a multi-disciplinary approach in the management of children and adolescents with diabetes (Type 1, Type 2, steroid induced and cystic fibrosis related) and their families.
- Working with patients diagnosed with diabetes and their families, to facilitate improving self-management age specific goals for all patients; providing lifestyle coaching, and conducting one-on-one patient visits and group visits in a hospital clinic setting.
- Providing patient and family education; collaborating with outside community resources including but not limited to school and insurer-based case coordination services.
- Providing case management including oversight and interpretation of laboratory results for patients with diabetes; ordering of therapeutics, durable medical supplies and refills for prescription medications; as well as coordination of referrals and diagnostic studies as needed in consultation with the patient’s physician.

Minimum Qualifications
Education:
- Master’s degree in Nursing with FNP or PNP (for NPs)
Experience:
- Minimum 1 year RN experience required.
- Minimum 2 years as a Diabetes Nurse Educator with a strong clinical background and experience in content areas such as patient education, diabetes, diabetes technologies (insulin pumps, continuous glucose monitors, smart pens), nutrition, physical fitness and other health related topics preferred.
- Knowledge of community resources and previous experience working with diverse patient populations across the age spectrum.
- The ability to work independently in a multidisciplinary team which includes nutrition, social workers, physicians, and other diabetes nurse educators.
- Effective and energetic style to develop and maintain therapeutic relationships and self-directed skills with a strong desire to impact patients with diabetes and their families.

PURPOSE AND SCOPE:
Ensure the provision of quality patient care to all hospitalized patients in the most efficient manner in accordance with company policy. - Interacts with internal departments and external customers; particularly in problem resolution and acting as the industry expert on behalf of the customer.
- Ensures compliance with hospital regulations and participates in surveys conducted by TJC, CMS, OSHA and other agencies.
- Ensure a proper orientation and training program for all clinical and technical staff. Ensure that continuing in-services are provided to all clinical and technical staff. Maintain documentation of all training.
- Conduct and document acute staff meetings on a regular basis.
- Ensures provision of dialysis and related services such as hemoperfusion, apheresis, continuous renal replacement therapies, peritoneal dialysis, and UF complete if contracted to do so.
- Ensures that treatment information is documented appropriately by staff performing treatments and oversees billing for services performed is timely and accurate.
- If appropriately licensed, performs treatments as necessary to assist in times of high census, staffing shortages, and/or to assist with managing staffing costs as appropriate.
- Confer routinely with Technical Program Manager and Technical Operations Manager to ensure preventative maintenance program for routine machine maintenance, to ensure the timely repair of disabled machines, to coordinate infection control measures, routine culturing, water testing, logging of results, transportation of equipment and supplies and communicate technical information to the staff.
- Ensure all acute treatments are performed in contracted hospitals according to the established policies and procedures and in compliance with all applicable regulatory requirements.
- Ensure efficient utilization of supplies and equipment.
- Assist with various projects as assigned.

* Additional Info: -Passed Board Certified-Fully licensed in MA-DEA and MCSR license in MA (or in progress)-ACLS/PALS certified (or are willing to obtain this certification prior to starting)-Candidate wishes to work in Urgent Care primarily (the candidate is not "dipping their toes in the water" to try it out or applying as a back up to a Hospitalist position)-Ideal candidate has at least 1 year of experience (this is not mandatory)-Profile includes any skills gaps and what the provider has done in self-study to close those gaps- Profile includes an explanation for any gaps in employment or long gap between graduation and certification/licensure if the candidate is a new graduate- Must see patients of all ages.- Must be confident/ comfortable completing 30-40 patient notes per day in urgent care. - Every Provider works his or her proportional fair share of weekends and holidays, and a full time Provider will work every other full weekend (Sat/Sun).- EMR: DocuTAP
