Position Summary/ Department Summary:
Brief Overview
Nurse Practitioner, Nurse Educator Diabetes
Key Responsibilities:

- Coordinating patient self-management initiatives in a family centric program for pediatric and adolescent patients in Boston Children’s Hospital’s Division of Endocrinology’s ADA recognized outpatient diabetes self-management education program. The program uses a multi-disciplinary approach in the management of children and adolescents with diabetes (Type 1, Type 2, steroid induced and cystic fibrosis related) and their families.
- Working with patients diagnosed with diabetes and their families, to facilitate improving self-management age specific goals for all patients; providing lifestyle coaching, and conducting one-on-one patient visits and group visits in a hospital clinic setting.
- Providing patient and family education; collaborating with outside community resources including but not limited to school and insurer-based case coordination services.
- Providing case management including oversight and interpretation of laboratory results for patients with diabetes; ordering of therapeutics, durable medical supplies and refills for prescription medications; as well as coordination of referrals and diagnostic studies as needed in consultation with the patient’s physician.
- Working 40 hours per week, day shift M-F. (Availability to work at Weymouth and N. Dartmouth primarily as well as some time in Boston)

Minimum Qualifications
Education:

- Master’s degree in Nursing with FNP or PNP (for NPs)

Experience:

- Minimum 1 year RN experience required.
- Minimum 2 years as a Diabetes Nurse Educator with a strong clinical background and experience in content areas such as patient education, diabetes, diabetes technologies (insulin pumps, continuous glucose monitors, smart pens), nutrition, physical fitness and other health related topics preferred.
- Knowledge of community resources and previous experience working with diverse patient populations across the age spectrum.
- The ability to work independently in a multidisciplinary team which includes nutrition, social workers, physicians, and other diabetes nurse educators.
- Effective and energetic style to develop and maintain therapeutic relationships and self-directed skills with a strong desire to impact patients with diabetes and their families.

Licensure/ Certifications:

- A current license from the Commonwealth of Massachusetts to practice as a Nurse Practitioner
- DEA and MCSR licensure
- Current certification in diabetes education (CDCES) preferred but must acquire within 1 year of employment.

The posted pay range is Boston Children’s reasonable and good-faith expectation for this pay at the time of posting.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Monitor costs/spending in all areas including medical supply inventory, personnel and accounts payable. Determine innovative ways to reduce costs and maintain quality.
- Ensures proper staffing and operations at all contracted facilities. Ensure efficient staff/ patient scheduling and efficient staffing utilization following company policy while maintaining a safe environment.
- Provide leadership, coaching and development plans for all direct reports to maintain an engaged and productive workforce; partnering with Human Resources on employee matters.
- Acts as an advisor to subordinate(s) to meet schedules and/or resolve technical problems. Responsible for hiring, coaching and counseling employees, including performance reviews, development, disciplinary action and terminations.
- Technically proficient in the specific department and knowledge of industry practice and business principles. Works on issues of diverse scope where analysis of situation or data requires evaluation of a variety of factors, including an understanding of current business trends. Reviews profit and loss statements for greater understanding of specific department performance.
- Responsible for Acute Care Electronic System (ACES) include the following:
- Takes initiative and action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner.
- Collaborate with contracted facility representatives to maintain a state of survey readiness for internal and external agency surveys. Work with contracted facility (ies) to resolve specific department issues as suggested/directed as a result of a survey.
- Maintains and prepares QAI data and presentations. Participates and presents at meetings with internal and external representatives, often leading a collaborative effort among members of a project team. Assures provision of facility specific QAI information as appropriate.

- Interacts with internal departments and external customers; particularly in problem resolution and acting as the industry expert on behalf of the customer.
- Ensures compliance with hospital regulations and participates in surveys conducted by TJC, CMS, OSHA and other agencies.
- Ensure a proper orientation and training program for all clinical and technical staff. Ensure that continuing in-services are provided to all clinical and technical staff. Maintain documentation of all training.
- Conduct and document acute staff meetings on a regular basis.
- Ensures provision of dialysis and related services such as hemoperfusion, apheresis, continuous renal replacement therapies, peritoneal dialysis, and UF complete if contracted to do so.
- Ensures that treatment information is documented appropriately by staff performing treatments and oversees billing for services performed is timely and accurate.
- If appropriately licensed, performs treatments as necessary to assist in times of high census, staffing shortages, and/or to assist with managing staffing costs as appropriate.
- Confer routinely with Technical Program Manager and Technical Operations Manager to ensure preventative maintenance program for routine machine maintenance, to ensure the timely repair of disabled machines, to coordinate infection control measures, routine culturing, water testing, logging of results, transportation of equipment and supplies and communicate technical information to the staff.
- Ensure all acute treatments are performed in contracted hospitals according to the established policies and procedures and in compliance with all applicable regulatory requirements.
- Ensure efficient utilization of supplies and equipment.
- Assist with various projects as assigned.

- Other duties as assigned.

EDUCATION:

We have an extensive ancillary support including Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Medical Assistants (MAs).

We have onsite lab and radiology.

Our radiology studies are read by ED based radiologists within the hours.

We also have quick access to specialist for patients requiring expediated referrals.

Responsibilities: Provide high-quality, compassionate care to patients presenting with acute medical conditions.

Manage high acuity patients, including infants and newborns, with confidence and expertise.

Perform common urgent care procedures with skill and efficiency.

Collaborate effectively with multidisciplinary teams to ensure optimal patient outcomes.

Demonstrate strong clinical acumen and decision-making abilities in fast-paced environments.

Maintain accurate and timely medical records using electronic health record systems.

(Dragon and AI capabilities available for charting) Participate in continuous quality improvement initiatives to enhance patient care and safety.

Teaching opportunities for Medical Students and Residents.

Qualifications: Board certification in Internal Medicine/Pediatrics (Med/Ped), Family Medicine (FM), or Emergency Medicine (ER).

Comfortable managing high acuity patients across the lifespan, including newborns.

Proficient in common urgent care procedures and treatments.

Bilingual proficiency is advantageous.

Point-of-care ultrasound (POCUS) training is not required but considered a plus.

Strong interpersonal and communication skills.
