Role Description
This is a full-time, on-site role for a Physician Assistant Certified at Chester County Spine Care, located in West Chester, PA. The Physician Assistant will collaborate with physicians to evaluate and manage patients with spine and chronic pain conditions. Responsibilities include conducting patient assessments, performing physical examinations, interpreting diagnostic tests, and ordering and following up after image-guided spine and joint injections. The role also involves educating patients about treatment plans, monitoring their progress, and providing post-procedure care. About the Role
The Lead Sales & Kitchen Designer owns the full client journey—from initial consultation and
measurement through design, proposal, and close—while setting the standard for design excellence
and customer experience. This role is client-facing and hands-on, blending consultative selling
with thoughtful, precise design execution. You’ll meet clients in-home, in the showroom, and
virtually as needed; translate vision into functional, on-budget designs; and guide customers
confidently through selections and purchasing decisions. You’ll have direct access to design
support to collaborate on layouts, technical details, and complex design considerations—ensuring
accuracy, efficiency, and a best-in-class outcome—while maintaining full ownership of the client

Training & Support
Cabinet IQ provides structured onboarding and ongoing training in sales process and consultative
selling, design standards and best practices, and systems, tools, and operational workflows. This
role will also have direct access to design support to collaborate on layouts, technical details,
and complex design decisions—allowing you to deliver exceptional results while maintaining speed,
accuracy, and confidence with clients.
What You’ll Bring
• 3+ years of experience in kitchen and bath design and sales (cabinetry and countertops strongly
preferred).
• Proficiency with 2020 Design (Compusoft/2020) or similar tools.
• Strong spatial awareness, design judgment, and attention to detail.
• Confident communicator who can educate clients, simplify decisions, and close.
• Comfort using CRM tools and productivity software.
• Reliable transportation for local travel; ability to lift or move samples (approximately 25–40

The RN Case Manager will coordinate care and drive patient progression to establish a discharge plan. This includes functioning as an interdisciplinary team member, creating, implementing, and developing all aspects of discharge planning to establish appropriate, timely, and effective transitions throughout the care continuum.
The Care Management Model:
Our Care Management Triad Team Model is a collaboration between the following:
- RN Case Manager - Manages patient care, drives patient progression, and establishes a discharge plan.
- Social Worker – Resolves psycho-social barriers and supports discharge needs.
- Utilization Management – Reviews patient status for appropriateness and anticipated payer coverage.
CARE MANAGEMENT:
- Identify patients who have post-acute care, placement, and complex discharge planning needs based on a comprehensive assessment that includes physical, as well as psycho-social factors/needs.
- Anticipate, initiate, and establish a discharge plan for patients with post-acute care needs, collaborating with the physician, nurse, and other health care providers, the patient, their family/primary caregiver(s), third-party payers, and employer, following established clinical guidelines, standards, and pathways.
- Review the admission assessment and collaborate with the primary nurse and other health care providers to ensure a multidisciplinary care plan is in place to meet identified patient care needs and desired outcomes.
- Identify system issues that serve as barriers to care. Participates in the development and implementation of strategies to remove the obstacles and facilitate patient progression.
