POSITION SUMMARY:

The Registered Nurse of Maternity Services reports to the Clinical Nurse Manager.  Utilizing the nursing process, provides high quality direct and indirect patient–centered care.  Coordinates patient care by working collaboratively with the interdisciplinary team, patients and families.  Communicates, as appropriate, the patient’s clinical condition, responding quickly and accurately to changes in condition or response to treatment.  Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served.  Promotes the profession of nursing and professional practice through collegial support and interactions.

 

EDUCATION/CERTIFICATION/EXPERIENCE:

  • Graduate of an accredited school of Nursing.

  • Current licensure in the state of Connecticut.

  • Previous LDRP experience preferred.

  • Must have certification in ACLS, and NRP upon hire or receive both certifications within a year after hire date and maintain clinical competency during employment with ECHN in this role.

  • Must have certification in Basic Cardiac Life Support upon hire or receive certification within the new employee introductory orientation period and maintain clinical competency during employment with ECHN in this role.

  • Must have Electric Fetal Monitoring Certification within a year after hire date.

  • Labor and delivery experience preferred.

·         Assesses and reassesses pain.  Utilizes appropriate pain management tools and techniques.  Educates the patient and family regarding pain management.  Documentation reflects assessments, interventions and evaluation of effectiveness.

·         Develops and revises individualized plans of care as indicated by assessment and patient response to treatment.   Evaluates overall plan daily for effectiveness.  Involves the patient and family in the formulation of goals.

·         Follows the five (5) medication rights and reduces the potential for medication errors. Demonstrates knowledge of medications and their correct administration based on age of the patient and their clinical condition.

·         Performs all aspects of patient care which includes Labor and Delivery; Postpartum, Well Newborn, Scrub; Circulator; and Level II Newborn in an environment that optimizes patient safety and promotes positive patient outcomes.

·         Documents the nursing process and responses to care in a timely manner utilizing the appropriate tools.

·         Demonstrates an ability to be flexible, organized and function under stressful situations.  Utilizes critical thinking skills and sound judgment in priority setting and delegation.

·         Formulates and implements a teaching plan for patients and families based upon identified learning needs. Evaluates effectiveness of learning.

·         Demonstrates autonomous practice, consistent with evidence-based standards.  Pursues personal and professional growth and development.  Serves as a professional role model and mentor.

·         Demonstrates knowledge of the principles of growth and development over the lifespan and the skills necessary to provide age-appropriate care to patients served. The Surgical-Trauma Intensive Care Unit is a 14-bed unit. Under the supervision of the rounding Surgical-Trauma Intensivist, the unit has a team of Physician Assistants, Advanced Practice Registered Nurses, Registered Nurses and surgical house staff physicians consisting of interns and residents.

Job Summary

The Nursing Manager is a Registered Nurse (RN) who has responsibility for the supervision of patient care, clinical personnel, and unit operations. This position supervises a work group to ensure that a high quality, cost-effective patient and family centered care is provided in support of the hospital's strategic plan, and the function's goals and objectives.

This position works collaboratively with functional leadership and is responsible for the oversight of daily operations of the assigned department including off shift and weekend supervision of nursing and non-licensed staff. This position manages resources, quality improvement, payroll, creating and maintaining colleague schedules and monitors patient throughput to facilitate efficient clinical operations. In addition, this position is responsible for a safe and therapeutic unit environment, which includes working in the department as staff RN to assist as needed.

This position supports leadership in ensuring standards of excellence and professional development are fostered and met. This position also participates in and may lead Performance Improvement work in their assigned department.

Clinical Supervisor- Hospice

Supervises the program to ensure that a high quality, cost-effective service is delivered by the department and that is in keeping with Hartford HealthCare at Home's strategic plan as well as the Program’s goals and objectives. Provides direct patient care that is specific to each patient’s needs, including those needs that are age specific.

In general, most of the time will be spent in the following activities:

Provides staff guidance in making clinical and case management decisions focused on clinical    excellence for end of life care and symptom management. Directs staff in the Medicare Hospice Benefit guidelines

Assures the development of personnel qualifications and policies

Identifies and facilitates professional development needs and competency of staff through home visits with staff to supervise and evaluate clinical performance; identifies and follows through with development plan

Maintains compliance with regulatory agency policies and procedures by providing staff guidance in making clinical and case management decisions that are cost effective and focused on clinical outcomes

Accountable for team performance in achieving desired clinical and operational performance measures

Utilizes clinical software to identify opportunities for education, most appropriate utilization of services, oversee clinical progression of patients and trend team outcomes and performance.

Assures that clinical and functional outcomes are achieved at the highest level possible

Resolves patient care problems collaboratively with internal/external customers – collaborates with peers across the regions

Oversees the daily patient assignments for clinical staff based on client needs and staff competency to maintain team productivity standards; LEAN Daily Management process (Visual Boards)

Coordinates referrals and ensures timeliness of SOCs and timeliness of adjunct service delivery

Ensures that patient needs are continually assessed

Ensures that an individualized plan of care is developed, implemented and updated as needed for all patients.

Ensures coordination of patient care, including communication with all physicians involved in the plan of care          

Determines staffing needs and coordinates hiring to meet service demands for assigned geographic areas

Hires, trains, coaches and evaluates staff.  This role is also responsible for completion of personnel evaluations according to agency timelines.

Reviews and analyzes team and patient metrics to consistent with agency initiatives.             

Participates in preceptor programs

Anticipates customer needs and responds quickly, accurately and pleasantly

Participates in preceptor programs and student education regarding hospice and palliative care end of life issues                           



Qualifications

Licensure: A registered nurse with a license to practice in the State of Connecticut 

Education:  A Diploma in nursing.  Bachelors Degree in nursing preferred 

Experience: According to current state licensure regulations. Hospice experience with OASIS and case management certification preferred.

Basic keyboarding skills and ability to manage electronic calendar and email system.

This position reports to the Director of Adolescent Education and to the Director of Child and Adolescent Ambulatory Services. Responsible for monitoring and responding to the overall health needs of students in the Clinical Day School. Responsibilities include performing initial student assessments (safety status, physical and/or emotional complaints, hygiene, etc.), monitoring self-administration of medications (prescribed or otherwise), maintaining state-required student health assessment records, prioritizing daily telephone calls regarding student’s health issues and/or medications, and observing, documenting and reporting medication issues/concerns to the appropriate MD or prescribing doctor.  This position requires a holistic approach to adolescent health and a working knowledge of health issues (medical and psychiatric) pertaining to the adolescent population. 



Qualifications

  • CT licensure as a Registered Nurse is required.
  • Cardiopulmonary Resuscitation (CPR) Certification is required.

The Medical Imaging (Radiology) RN Clinician will be responsible for representing ECHN to the Radiology Intervention patient in both the outpatient and inpatient setting.  The Radiology nurse will report directly to the Manager of Vascular and Cardiology Services and indirectly to the Team Leader of Interventional Radiology. The Radiology RN Clinician provides nursing care by assessing the patient pre-intra and-post- procedure and assisting the Radiologist in the various diagnostic radiology testing. Examples are:  Interventional angiography, US guided thorocentesis / paracentesis, CT guided biopsies and any pt care needs that come up in the Medical Imaging Department.  The Radiology, RN Clinician, will administer moderate sedation and other prescribed medication or pharmacological agents pre,intra and-post- radiology procedure/imaging test.

Participates in the operational duties of the department, such as but not limited to; Quality Improvement, Policy and Procedure maintenance, revising and development of billing and gathering of statistical data, developing departmental educational opportunities and participating in the on-going growth and maintenance of the excellent customer services provided to all that ECHN serves as well as assisting the Manager in maximizing the department’s financial resources.

The Radiology RN Clinician also provides educational resources for patients and assesses each patient for understanding of procedures, post procedural education and lifestyle modifications necessary for maintaining optimum health and assists the department in planning and implementing new programs within the department.

 The Radiology RN Clinician accepts on-call responsibilities for the Medical Imaging Department.

 COMPETENCIES:

·         Must be proficient in cardiac monitoring, administration of moderate sedation and be IV competent.

Job Summary



CB3 is a medical-surgical unit, specializing in the care of liver and kidney transplant patients pre and post-surgery. The unit is one of the most diverse units in the organization caring for a diverse mix of complex medical and complex surgical patients.



Our Nurses work in collaborative environments with strong interdisciplinary teams, focused on a patient centered care model in a richly diverse work space. Under the direction and general supervision of the Nurse Manager, the registered nurse utilizes evidence based practices in accordance with the State Nurse Practice Act, policies and procedures of the hospital and as directed by the medical staff. The registered nurse who through knowledge and ability uses the nursing process to develop a plan of care which includes standards of practice to meet the physical, emotional spiritual, cultural and educational needs of the patient and family.

 



Qualifications

 

 

  • Current Connecticut RN License.
  • Graduate of an accredited nursing school
  • ASN Required (Must obtain a BSN within 6 years of starting in the RN role)
  • BSN preferred.
  • BLS certification by the American Heart Association required prior to date of hire.

This is a nurse residency position that upon hire or transfer into the program the nurse commits to a full time learning opportunity.  This program prepares the nurse for an operating room staff nurse position as well as forms the foundation required to successfully take pass the Association of periOperative Registered Nurses (AORN) certification program. The program commences with a six month AORN, Periop 101 on line didactic program consisting of 27 modules, followed by a 3-5 month clinical orientation. The nurse will be assigned to a nurse educator and preceptor and earn 41.7 CEUs for completion of the online program.  Upon completion the nurse will be offered a full time position at either Manchester Memorial Hospital or Rockville General Hospital. A commitment to the Operating Room will be required for two years.

 The candidate must be willing to undergo a rigorous application process that consists of surgeon, manager, nursing education and staff interviews, observation day and letters of recommendation.  The candidate must be willing to take and pass the Certified Registered Operating Nurse (CNOR) certification exam within six months from residency completion.

1.      Performs all aspects of patient care in an environment that optimizes patient safety and promotes positive patient outcomes; follows ECHN policy for Patient Identification, Surgical & Invasive Procedures Site Identification, and Universal Protocol.

1.      Documents the nursing process and responses to care in a timely manner utilizing the appropriate tools.

1.      Demonstrates an ability to be flexible, organized and function under stressful situations.  Utilizes critical thinking skills and sound judgment in priority setting and delegation.

1.      Demonstrates autonomous practice, consistent with evidence-based standards.  Pursues personal and professional growth and development.  Serves as a professional role model and mentor.

1.      Demonstrates knowledge of the principles of growth and development over the lifespan and the skills necessary to provide age-appropriate care to patients served. Able to interpret data about the patient status in order to identify each patient’s age-specific needs and provide care needed by the patient group.

9.    Provides an accurate and pertinent report of the patient’s condition for safe transfer of care.

10.  Prepares room in a timely manner with all anticipated instruments, sutures, supplies, and equipment for each surgical procedure.

11.  Demonstrates knowledge of and assures maintenance of aseptic technique throughout the surgical procedure; identifies and communicates, to surgical team, any breaks in technique and assists with corrective actions. 

12.  Effectively communicates with the surgeon in accounting for all sponges, needles, and instruments; reports discrepancies immediately to surgeon and Team Leader; appropriately documents all counts.

13.  Accurately records, handles, labels and processes specimens.

14.  Demonstrates ability to effectively function in the scrub role. (we have hired RNs that do not come with this experience) Demonstrates willingness to effectively function in the scrub role.

Job Description 

Under provider direction, the RN provides skilled nursing care to patients in a variety of clinical settings. Scope of responsibility is characterized by use of nursing process to assess, plan, intervene and evaluate human responses to actual or potential health problems utilizing appropriate practices, standards, protocols and guidelines.
