About the Roleย
Weโre seeking Masterโs-level professionals who feel confident facilitating groups, collaborating closely with therapists, and thriving in a part-time, fully remote telehealth environment.
Clinicians at Charlie Health begin making a difference on day one, working alongside a highly skilled therapy team to translate psychoeducation into meaningful, real-world change for clients. While each team member brings unique strengths and experience, all staff are expected to be fluent across multiple modalities. Our clients face complex behavioral and emotional challenges, and weโre looking for group facilitators who are deeply aligned with Charlie Healthโs mission to provide life-saving mental health treatment.
Responsibilitiesย
- Complete all Onboarding requirements within 2 weeks of start dateย
- Check-in with your assigned Charlie Health Group Quality Supervisor or Group Quality Director at a minimum of 1x/month
- Respond to all email and Slack communication promptly (within 48 hours)ย
- Review the curriculum aligned to group assignment prior to group start timeย
- Arrive ~10 minutes early toย scheduled group time and facilitate all groups for the entirety of the hourย ย
- Facilitate groups using the current Charlie Health curriculum and best practices
- Facilitate groups across age groups and cohorts, including Integrative curriculum, Support Staffing and Wellness Hour as neededย
- Foster client engagement and group cohesion, encouraging client participation and fostering camera-on culture
- Following all operational policies and procedures as indicated by Charlie Health best practicesย
- Participate in collaborative Therapy Treatment Team (Tuesdays) and Group Supervision (every other Friday) with your assigned Charlie Health Group Quality Supervisor or Group Quality Director, peers and other Clinical Leadership team members
- Collaborate closely with Primary Therapists, Care Experience Specialists and Care Coachesย
- Communicate professionally and promptly with all clients, staff, families, agencies, and referents
- Monitor your treatment documentation to ensure all of your notes are completed within required 24 hour timeframe and meet agency and professional standards per DHCS and The Joint Commission standards
- Demonstrate professional ethics, including appropriate boundaries and confidentiality
- Other duties as assigned
Requirementsย
- Availability to work weeknights (3-8pm MT on Monday to Thursday) and Saturdays
- Masterโs degree in mental health or related field (see examples below)
- Experience working with diverse age demographics in intensive treatment settings
- Demonstrated proficiency across multiple treatment modalities, with the confidence to thoughtfully integrate them into clinical practice (e.g., DBT, CBT, EMDR; MI certification a plus)
- Strong belief in and advocacy for group-based treatment alongside individual therapy
- Ability to facilitate effective, engaging telehealth sessions
- Reliable high-speed internet connection for client sessions
- Proficiency with cloud-based communication software (Gmail, Slack, Zoom, Dropbox) as well as EMR and outcomes survey software
- Part time, 1099 contractor role

Role Description:
Oak Street Health's graduate Behavioral Health Specialist Fellowship is a 12-month paid training program in the revolutionary collaborative care model in a value-based setting, which rewards healthcare companies for the quality of their care not the quantity.
This program will include time spent building your knowledge of evidence-based interventions and psychotropic medications, earning clinical supervision hours from a licensed clinical social worker, and engaging in didactic content that aims to build on, not repeat, graduate training. All fellows will work at Oak Street for 24 months following the completion of this program, but we hope will continue to spend their careers at Oak Street improving the health of communities across the country.

Currently holds or will hold by the fellowship start date, an active, non-probationary state lower level social work license in the state they will be working in.
Selection Timeline ย

Role Description:
The purpose of a Behavioral Health Specialist at Oak Street Health is to provide care management services and to provide evidence-based short-term psychotherapy to our patients with behavioral health needs.ย The Behavioral Health Specialist is a key resource for Care Teams across a region of Oak Street Health primary clinics. They are responsible for coordinating and supporting behavioral healthcare for our patients, in collaboration with Care Teams and a psychiatric consultant. The Behavioral Health Specialist will assess the needs of patients, make referrals to appropriate behavioral health resources and specialists, and provide short-term counseling and evidence-based treatments as indicated. This role has the potential option for remote work days based upon achievement and maintenance of program metrics.
Core Responsibilities:
- Collaborate with the primary care teams to identify appropriate patients to refer to the Oak Street Behavioral Health Program
- Screen and assess patients for common mental health and substance use disorders
- Develop care plans and provide or facilitate referrals to Oak Streetโs psychiatric consultant and/or external resources
- Provide short-term counseling and evidence-based treatments
- Coordinate care with Oak Streetโs psychiatric consultant, including warm handoffs for telehealth visits and collaboration on care plans
- Develop relationships with primary care teams and consistently communicate changes in behavioral health care plans
- Accurate and timely documentation of patient encounters and counseling sessions in Oak Streetโs electronic medical record
- Participate in regional and organizational efforts to advance Oak Streetโs Behavioral Health Program through workshops, feedback sessions, and surveys
- Possibly covering multiple clinics, depending on the maturity and panel sizes of those clinics

What are we looking for?
Required:
- Masterโs Degree in the field of mental health counseling/healing arts required from an accreditedย school (Social Work, Counseling, Marriage and Family Therapy, etc.)
- Highest clinical level of license (or local equivalent)
- Experience with screening for common mental health and/or substance use disorders
- Experience with assessment and treatment planning for common mental health and/or substance use disorders
