STARDARD V Position Specific Performance Criteria/Essential Job Functions Performs various nursing functions as necessary within scope of practice to facilitate the needs of the clinic. Services may include but are not limited to triage of clients, rooming patients, taking vital signs, patient assessment, taking history of injury, medical history review, blood drawing, TB testing and immunizations. Assessments include workers’ compensation injury case management as well as general health assessment of employees at contract companies. Provides assistance to the medical provider as necessary within scope of practice. Performs spirometry testing, audiometry, EKG, vision testing, drug testing, and breath alcohol testing as necessary. Maintains appropriate certification for drug screening and breath alcohol testing. Performs case management duties as necessary, including: ensuring claim number is obtained, verifying insurance information, providing status reports and medical records to insurance companies, communicating work status to employers, and obtaining pre-approval when necessary. Follows up on active patients to ensure compliance with treatment plan. Calling no-shows, reviewing active patients weekly for compliance with treatment plan. Organizes biweekly case management meetings and acts upon identified case management needs.
Sets up referrals for diagnostic testing and to specialty physicians. Obtains test results and medical notes from referrals. Enters all referrals and other case management information into Systoc for tracking. Performs professional duties in a variety of environments to include clinic, hospital and onsite at companies. Manages patient flow according to schedule. Establishes good relationships with client companies and payers representatives assessing OHS’s fulfillment of obligations and service provisions. Develops tracking and reporting systems for clients. Communicates to employers/referral source in a professional, accurate and timely manner. Demonstrates knowledge of New Hampshire Workers’ Compensation laws. Track immunizations and lab results for physicals. Perform TB reads and coordinates necessary follow-up. Mails lab results to patients as necessary. Processes the results of physicals and pre-employment post offer screens. Ensures accuracy, completeness and transfer of appropriate information to employers. Ensures positive relationships with timely and appropriate communication to employers. Identifies clinical services that are requested/needed and coordinates the staffing, supplies and services. Ensures compliance with reporting of patient safety errors. Coordinates and oversees the drug and alcohol screening program for OHS. Ensures the accuracy and timeliness of all screens from start to completion. Provides the MRO services for the non-DOT positive drug screens for THC. Provides training as needed for DOT and non DOT training. Acts as a resource for DOT physicals requirements. Perform other duties as assigned.

Job Summary
PetSmart’s Pet Associate (Retail Sales Associate) is responsible for engaging with pet parents and their pets while providing positive experiences and upholding the company’s vision, mission, values, and strategy. This role also shares responsibility of store cleanliness and maintenance, and pet safety standards as well as the direct care of pets within our store.
Essential Responsibilities
Job responsibilities include, but are not limited to, the following and may vary depending on store characteristics and needs:
- Responsible for the pet parent experience and outcomes conducted in person, over the phone, at the sales register and online.
- Greets pet parents and answers their questions throughout the store.
- Prepares online orders for pick-up in store and ensures a smooth, positive, pick-up experience.
- Responsible for the pet healthcare of store owned pets, which includes feeding, watering and cleaning all pet habitats (bird, reptile, small animal, cricket, and fish aquariums).
- Maintains total store cleanliness standards.
- Supports the store with weekly price changes and monthly promotional pricing.
- Stages and sorts new product to match sales floor planners.
- Faces, fills, and recovers products to meet sales floor standards.
- Recommends, informs, and sells merchandise, services and live pets.
- Administers store owned pet medications as directed by veterinarian
- Promotes special events such as adoption weekends, Treats enrollment, charity events, etc.
- Ensures a safe environment for our associates, pets, and pet parents.
- Responsible for taking immediate action when a sick/injured pet is identified in the store; transport to the vet as needed.
- Assists and works in various areas throughout the store (including services) as required. Other duties may be assigned.
- Participates in our culture of Belonging and Recognition.
- Follows all Company Policies and Procedures.
Qualifications
- Strong written and verbal communication skills.
- Flexibility in schedule, able to work evenings, weekends, and holidays as needed
- Ability to react under pressure and maintain composure.
Essential physical demands and work environment
- Associate required to stand, walk, climb a ladder, and use hands to handle, feel, hold, write, etc. The associate frequently is required to reach with hands and arms: stoop, kneel, crouch, talk and hear. Specific vision abilities required by this job include depth perception. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
- Associate must frequently lift and/or move up to 50 pounds and occasionally team lift and/or move more than 100 pounds. Specific vision abilities required by this job include depth perception.
- While performing the duties of this job, the associate is occasionally exposed to moving mechanical parts, as well as a wet environment with an abundance of pet hair. The noise level is moderate in the work environment; however, associates may be exposed to high levels of noise at times. Exposure to live animals and their handling is common.

Make face-to-face calls on cold and warm sales prospects.
Service customers in the manner outlined in Company training materials.
Submit complete and accurate daily business report detailing sales orders and prospect calls.
Present products and services to existing and prospective customers using sample boards, product demo, and the mobile store.
Maintain the cleanliness, operation, marketing, and functionality of the mobile store.
Continually maintain customer contact information through the use of company software, including customer notes, names, phone numbers, email addresses, and current physical addresses to be updated daily.
Provide an approved fully insured vehicle, the insurance, fuel, and maintenance, and various technological tools necessary to successfully operate an assigned sales territory, including but not limited to designated mobile devices (tablets, iPad, and phones).
Participate in ongoing professional development activities to continually improve job-related skills.
Other related duties as assigned.
Education and Experience Minimum high school diploma or equivalent.
Outside industrial sales experience preferred, especially in route or industrial sales.
Proven history of goal attainment.
Required Skills Excellent analytical, reasoning, and organizational skills.
Detail-oriented.
Ability to clearly articulate ideas and information in written and verbal communications.
Proficiency with databases, spreadsheets, email, and common business applications.
Working knowledge of the products we sell is helpful.
Other Requirements Must be able to purchase or lease an approved vehicle (mobile store).
Must reside within territory.
Above average mechanical interest.
Demonstrated ability to work independently.
Ability to kneel & bend down to the floor on a regular basis.
Clean driving history.
Conduct oneself in a professional manner when representing the company, i.e., driving approved vehicle, when wearing company attire, company functions.
