We’re different than most primary care providers. The Nurse Practitioner (NP) acts as part of the clinical operations team and is responsible for providing direct patient care in ChenMed/Jencare medical centers, nursing homes, skilled nursing facilities (SNF) and home settings depending on the nature of the assignment or providing assessments to members in SNF and home settings. The responsibilities include but are not limited to: geriatric assessment, medical history, physical exam, diagnosis and treatment, development of the nursing plan of care, health education, physician referrals, case management referrals, follow-up and clear documentation according to ChenMed standards for quality, service, productivity and teamwork. The Nurse Practitioner must demonstrate the ability to function both independently and in collaboration with other health care professionals. Consults with the manager, physician, and medical director to ensure compliance with guidelines. This position may require participation in risk and quality management programs, clinical meetings and other meetings.
The Nurse Practitioner will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
Functions independently as a certified nurse practitioner for a patient population in collaboration and consultation with a licensed patient care team physician. Practices in accordance with a written or electronic practice agreement. Performs and documents physical assessments and patient histories, analyzes trends in patient conditions, and develops, documents and implements a patient management plan in response to the data obtained. This also includes assisting in the development of the nursing plan of care in addition to providing appropriate patient/ family/significant other counseling and education.
Plans patient care based on in-depth knowledge of the specific patient population and/ or protocol, anticipating and identifying physiological and/ or psychological problems commonly encountered including the consideration of the patient’s cultural background, level of understanding, personality and support systems. Serves as patient advocate.
Refers patients for consultation when indicated i.e. dermatology, neurology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, psychiatry, social service, dietary, etc.
May perform invasive procedures independently upon the completion of documented competency. 
Participates in patient care rounds and conferences. Communicates patient management strategies to members of the patient care team. Collaborates with members of the multidisciplinary team to ensure that patient management strategies are successful in meeting patient care needs.
Recognizes situations which require the immediate attention of a physician, and initiates life-saving procedures when necessary.
Collaborates with other multidisciplinary team members to analyze and evaluate current systems of health care delivery to identify and implement new practice patterns as appropriate.
Participates in outside activities that enhance personal and professional growth and development.
Works collaboratively with physicians, nurses, PT, social workers, family and key caregivers to transition the patient to a lower level of care as soon as medically appropriate. 
Introduces self to patient/family and explain nurse practitioner role.
Enhances a collaborative relationship to maximize the patient’s/family’s ability to make informed decisions re: goals of care, palliative care and hospice.
Facilitates discharge to appropriate level of care and uses preferred providers when additional services are required.
Demonstrated record of consistently achieving clinical performance metrics in current role
Must be detail-oriented, and able to pay close attention to patient charts, medications, and follow-up on details of patient care
Basic computer skills and some knowledge of Microsoft Office Suite
This position may require 50-75% of local travel
Current RN licensure and Nurse Practitioner Certification in the State of practice required
Minimum 2 years of clinical experience as a Nurse Practitioner required practicing in Family Medicine, Internal Medicine, or Geriatrics, including past level of autonomy to make independent care decisions.

Cook Position

The Cook position is responsible for the preparation, production, and presentation of a variety of foods, completion of production sheets and with other associated food service activities.

Responsibilities

1. Performs all main line food production duties with specific items determined by assigned shift.
2. Reviews the needs of the day with assigned supervisor(s), completes production sheets, reviews maintenance reports, and verifies supply needs and availability.
3. Assists assigned supervisors with menu and recipe development.
4. Is responsible for quantity and quality control in food production as well as overall sanitation of assigned areas.
5. May assist in the supervision and training of food service personnel.
6. Arranges for kitchen helpers/assistants to complete smaller item preparation.
7. Verifies and oversees food serving lines, areas, bars, etc., making sure they are properly stocked, set up, and cleaned.
8. Using a working knowledge of all general food service area positions, this position "fills in" for any one of these positions at a moment's notice.
9. Observes and instructs others on the proper rotation procedures to be followed for produce and dairy products.
10. Maintains food production equipment and storage areas following proper sanitation, safety, and operational procedures.
11. May assist other food service personnel at any given time.
12. This position has no supervisory responsibilities.
13. Other duties that are pertinent to the department or unit's success also may be assigned.

Qualifications

Minimum Requirements

• Must have a High School diploma or GED equivalent.
• Must have or obtain a Municipality of Anchorage Food Handler's Card or ServSafe Certification contingent upon 30 days of hire.
• Must have two (2) years of related work experience.
• A valid Driver's License and an acceptable driving record for the past three (3) years to be eligible under NMS' vehicle insurance policy
• Contract requires employees to speak, understand, read and write English.

Responsibilities

The Diagnostic Medical Sonographer provides diagnostic ultrasound tests for patients in the Maternal Fetal Medicine setting. The Diagnostic Medical Sonographer works independently and with other members of the healthcare team to provide comprehensive care to pregnant women.

- Explains exam to patient and family prior to the commencement of the test to assure cooperation and optimum test results.
- Acquires anatomical, pathological, and/or physiological data.
- Records/processes sonographic data and other pertinent observations made during the procedure.
- Creates a preliminary report for physician interpretation and review/signature.
- Assists physician with procedures such as amniocentesis and chorionic villus sampling.
- Recognizes equipment malfunctions and takes appropriate corrective action.
- Responsible for maintaining adequate inventory of ultrasound supplies.
- Follows the accredited laboratory protocols for obtaining two-dimensional images, color flow, Doppler, and m-mode where applicable.
- Rotates as needed to cover Sonographer absences.
- Performs related and other assigned duties as directed.

Qualifications

Education:

- Graduate of an accredited sonography program with a degree in ultrasonography or equivalent.
- Maintain or be eligible for certification as an American Registered Diagnostic Medical Sonographer (ARDMS) in OB/GYN.
- Must provide evidence of continuing education credits to fulfill accreditation requirements.
- CPR certification required.
- Fetal Echo certification - required to obtain within 1 year of hire.

Experience

- Minimum six months experience as an ultrasound technician working in OB/GYN and/or MFM setting preferred.

- Skilled in operating ultrasound equipment.

POSITION PURPOSE 

Provides and directs safe, effective and culturally competent care for high risk, critically acute patients with actual or potential life-threatening conditions.  Through evidence-based practice, utilizes critical thinking to assess, plan, implement and evaluate outcome-based care of patients. MINIMUM QUALIFICATIONS 

• Associate degree required, BSN preferred. 

• No experience required; critical care nursing experience preferred. 

• Current Iowa RN license, (current IL license if working in IL).  

• BLS, Mandatory Reporter.    ACLS within 12 months of hire.    Cardiac monitoring course and/or select units of advance Nursing / Critical Care Course or comparable training 

• TNCC  

• Non-Violent Crisis Intervention Training (NVCI) within 6 months of hire  

• High language, intermediate to high math, high critical thinking ability.

Job Summary / Purpose
