The Registered Nurse (RN) is responsible for facilitating the patient's hospitalization from preadmission through discharge. - Provides "Choice Letter" and assists the patient with selecting a DME company, Nursing Home, Assisted Living facility, Home Health agency, Hospice, etc.; obtains signature on the choice Letter by the patient/family and ensures placement in the chart and documents.
- Provides adequate communication of relevant issues to the interdisciplinary healthcare team and initiates referrals to service providers as identified in the discharge plan. Coordinates discharge teaching.
- Ensures that the interdisciplinary care/discharge plan is consistent with the patients clinical course, continuing care needs and covered services and modifies, as indicated.
- Reports and discusses with attending physicians and or physician advisors the appropriateness of resource utilization, consultations, and treatment plan.
- Assists with establishing Advance Directives, Medical power of Attorney, etc. as indicated. Identifies and establishes legal guardian/decision maker.
- Utilizes the Patient/Visitor Safety Learning Report to document patient safety issues and complaints related to care.
- Engages patients to actively participate in meeting short and long-term healthcare goals and identifies appropriate community resources and support services to assist the patient.
- Tracks and ensures that the Important Message from Medicare has been provided to the Medicare patient on admission and that a follow-up copy has been provided, initialed and placed in the chart.
- Review's the patient's progress as described by the various disciplines involved on an ongoing basis to ensure an effective plan is in place.
- Ensures discharge prescriptions, orders, and appointments are made, DME, OP services, Nursing Home care, etc. have been arranged and discharge, transfer, and referral forms are as complete as possible prior to patient departure.
- Develops a discharge/care management plan in collaboration with other members of the healthcare team, the physician and 3rd party payers, as indicated.
- Communicates the plan with the patient and family/significant other and adjusts the plan based on the patient's progress, input, and needs.

- Aligning the rollers in the equipment to meet all dimensional requirements of job order. This includes adjustment for taper, TIR, roller surface finish, and finish dimensions.
- Measure all required dimensions and understand the relationship of the dimensions to the specific requirements of the job.
- Utilize all inspection and measurement equipment per procedure and instruction, including durometer gauges, pi-tapes, P&J gauges, micrometers, vernier diameter tapes, dial indicators, profilometers, temperature-indicating devices, and others as needed.

- Observe, report, and document symptoms and conditions of residents for changes in condition such as skin, behavior, alertness, weight, diet, and participation in activities.
- Attend daily Cross Over meetings by the lead care manager.
- Notify supervisor and resident care director if a resident has increased care needs.
- Inform supervisor of any resident changes in condition.
- Respond to security system and resident call bells promptly and immediately. Take appropriate action including resetting call bells.
- Greet guests, family members, residents, and team members.
- Answer, direct and/or respond to phone calls in a timely, courteous, and professional manner.
- Assist with continence management and dispose of all continence products properly to ensure sanitation of resident suite and community restrooms using standard care procedures.
- Communicate with families and is a resource as needed.
- Help residents maintain independence and physical safety and promote dignity of each resident adhering to the standards of Resident Rights and Sunrise Principles of Service.
- Participate/lead and assist residents with activities of daily living (ADLs) and Invite, Encourage, and Assist (IEA) residents to participate in activities as indicated on the ISP.
- Engage residents in life skills and other life enrichment activities throughout the day in reminiscence.
- Strive to understand and respond to each resident with empathy, always remaining mindful of the resident's unique communication patterns or history and basic human needs.
- Maintain an atmosphere of warmth, personal interest, and positive emphasis as well as a calm environment.
- Ensure the established safety regulations are always followed.
- Practice routinely good Standard Care Precautions of cleanliness, hygiene, and health.
- Host and engage in activities with the residents daily.
- Partner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements, promoting Risk Management programs and policies, and adhering to safety rules and regulations.
- Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
- Report all accidents/incidents immediately.
- Reports all unsafe and hazardous conditions/equipment immediately.
- Ensure any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits, and in compliance with fire codes.
- Understand and practice the proper method of attending to and disposing of, and the possibility of exposure to, blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials.
- Report occupational exposures to blood, body fluids, infectious materials, sharp sticks, and hazardous chemicals immediately.
- Ensure oxygen tanks are stored safely, exchanged when empty and monitored to make sure liters of oxygen are at prescribed levels.
- Maintain and clean resident's room as assigned including making beds, emptying trash, putting clothes away appropriately, spot sweeping, and mopping as needed.
- Wash resident's laundry as noted in the ISP and as needed.
- Wash and fold dining room linens and napkins.
- Complete assigned housekeeping tasks.
- Maintain common areas in a clean and tidy manner at all times.
- Serve meals in the dining room and work in the dining room as assigned.
- Promote and ensure a pleasant dining experience during all meals.
- Assist with dining room set up and clean up as assigned.
- Participate in pre-meal meetings.
- Follow residents' diets as indicated on ISP and Confidential Diet Board/Chart.
- Observe, note, and document in daily log any resident changes in dining habits.
- Respond to the dining needs of the residents and guests while maintaining Sunrise hospitality and service standards.
- Provide room service delivery as needed.
- Practice safe food handling in compliance with universal care precautions at all times.
- IEA residents in life skills and other life enriching activities as indicated on ISP and demographic profile.
- IEA residents to attend the afternoon social.
- Incorporate the concept of 'Creating Pleasant Days' into the resident's daily routine.
- Integrate the individual resident's life skills into their daily routine.
- Blend a variety of multi-sensory experiences into the resident's day.
- Participate in monthly letters home and letter writing with the residents and their families.

- Contribute to the overall engagement programs and processes (customer and team member engagement) including participating in the team member engagement survey and engagement improvement planning workshops.
- Attend regular meetings; Town Hall, Department Team, Cross Over, and others as directed by the Supervisor/Department Coordinator.
- Attend regular training by Assisted Living Coordinator (ALC), Resident Care Coordinator (RCC) or Reminiscence Coordinator (RC) using mini modules.
- Comply with all infection control techniques, placement of bio-hazard containers, and removal techniques as listed in policies and procedures.
- Maintains compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.
- Perform other duties as assigned.

Job Summary The Manager, Global Oncology HEOR & RWE will work cross-functionally to implement HEOR and RWE projects for the assigned assets under the direction of a Director or Sr.
Director of Global Oncology HEOR & RWE.
The person will work with clinical, market access, commercial, regional and medical affairs colleagues to support the clinical development and value creation of the company's oncology assets.
Responsibilities include the implementation of outcomes research strategies, health economics research planning, and process improvements to ensure patient access to Daiichi Sankyo's products.
This individual will ensure that rigorous HEOR studies are conducted to support the clinical and economic value proposition associated with the Daiichi Sankyo Oncology products.
The work involves developing global cost effectiveness and budget impact models and to fill data gaps to support access needs, as well as to design and implement global outcomes research & health economic studies that supports key publications, reimbursement strategies.
The person will work closely with other members of the Global Market Access & Pricing department to develop RWE and support market access initiatives aligned to Daiichi Sankyo's products, and represent the department in cross functional working groups and committees from time to time.
Responsibilities HEOR and RWE Deliverables Support the preparation of global value dossiers which present the product value propositions and the supporting evidence, aligned with the product development strategies and agreed/endorsed by the GPTs, GBTs, Regional HEOR and market access, to realize an optimal value demonstration of the products Conduct or support HEOR and RWE studies such as early economic models, burden of illness studies, external control arms to support single arm trials, and support the development and analysis of patient-reported outcomes (PRO) instruments in clinical trials Conduct literature reviews to build payer evidence, economic models, and Global Value Dossiers Support the analysis of clinical trial data for cost effectiveness models and market specific ad-hoc requests for reimbursement submissions Conduct RWE studies to support the value story for payers and reimbursement submissions Support the development of global cost effectiveness and budget impact models for HTA submissions to support reimbursement of indications for Daiichi Sankyo’s oncology products Develop HEOR and RWE communications (e.g.

Co-Worker Health Nurse - LPN or RN
Full-Time, 40 hours per week
Mercy Hospital Oklahoma City
Under the general supervision of the Manager – Co-worker Health, the Co-worker Health Nurse performs daily activities to provide health services to Mercy co-workers. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
The Co-worker Health Nurse is a critical job that works closely with all co-workers from day one.
- Plans, organizes, directs and performs all activities of the co-worker health services in concert with federal and state requirements and regulatory agencies.
- Processes new co-workers for a health assessment at hire, which includes pre-employment drug testing, health medical history, lab work, chest x-ray if indicated and immunization status.
- Supports and administers co-worker TB skin screening program in compliance with state regulations.
- Performs respirator fit testing and training as indicated.
- Promotes and administers annual Influenza and other vaccines to all co-workers working in the facility. Maintains knowledge of all immunizations being administered in order to communicate and assess contraindications to giving the vaccine, possible side effects, and precautions the co-worker should know about before taking any vaccine.
- Provides first aid to employees as necessary and works closely with co-workers following on the job injuries to provide quality care and follow up to injured workers. Communicates and works with departments, supervisors and team leaders regarding ill or injured co-workers as necessary. Works to return co-workers injured on the job to restricted duty.
- May serve as Workers' Compensation coordinator, depending on work location, following state commission rules and regulations. Responsible for the recording of injuries/illness according to regulatory agency guidelines; educating and informing injured employees and managers; facilitating medical treatment and return to work of the injured employee; communicating with the System Office, treating physicians, and legal counsel.
- Maintains strict confidentiality of co-workers regarding drug screens, illnesses, injuries and health records.
- Compiles annual and monthly reports and presents these reports to appropriate committees in order to assist these committees to comply with requirements of Joint Commission, The applicable state Department of Health and other government agencies.
- Keeps current on Co-worker Health related issues; adjusting policies and procedures as indicated to meet changes in requirements for co-workers working in a health care facility.
- Works with co-workers to promote committee guidance and education in multiple committees, i.e., wellness committee, needlestick safety and environmental safety.
- Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
- Communicates effectively by providing concise, effective and accurate verbal and written communications regarding co-worker health operations.

Qualifications:
