Provides physical therapy interventions to designated patients under the supervision of a Physical Therapist (PT). Assists in monitoring patient progress through observations and follows up with supervising therapist. Works under PT supervision as required under NY State Education law.
• Follows established physical therapy plan of care, including physical therapy interventions and patient/caregiver education. Modifies specific treatment procedures in accordance with PT plan of care and Physical Therapy Assistant (“PTA”) scope of practice.• Documents all care provided and maintain records per VNS Health policy and standards.• Assists in monitoring patient progress through observation and follows up with Supervising Therapist in accordance with VNSNY policy.• Complies with scope of practice for PTA, including a co-visit with Physical Therapist on all initial PTA visits and keeps PT informed. Provides direct psychiatric evaluation, medical health assessment and intervention to clients admitted to Behavioral Health programs. Provides medication management and assessment on an ongoing basis. Provides consultation and education to other agencies and disciplines, including other VNS Health services. Assists in the overall management of the Behavioral Health program. Acts as a key resource in providing clinical and operational guidance and support to assigned teams and other staff to achieve and enhance team outcomes. Provides advanced nursing clinical care for clients in accordance with current State and Federal rules and regulations for nurse practitioner scope of practice and national standards of care, and specific program regulations/contract requirements. Works under general direction.

What You Will Do
- Performs psychiatric evaluations of identified clients.
- Assesses current administrative processes and clinical protocols; identifies opportunities for improvement relating to quality and cost while maintaining adherence to compliance and nursing standards of care.
- Provides medication management, ongoing assessment, and supportive counseling according to program requirements.
- Evaluates need for physical evaluations and/or medical services and make referrals, as indicated.
- Provides full scope of advanced nursing practice for targeted client populations. Evaluates client responses to therapy and interventions. Ensures revision of the inter-professional plan of care as necessary to achieve quality outcomes.
- Consults with other mental health providers and community agencies, including other VNS Health department/programs, as well as members of the Behavioral Health program, regarding individual clients.
- Participates in interdisciplinary team meetings and case conferences for the Behavioral Health program.
- Maintains client records in compliance with VNS Health, state, city and federal requirements.
- Evaluates clients for psychiatric hospitalization, as necessary.
- Provides clinical supervision and direction to Behavioral Health program staff, including on-site observation of direct patient care and participation as needed in direct care. Participates with Behavioral Health program staff and other appropriate VNS Health staff in the development and implementation of in-service trainings.
- Collaborates with program management to ensure the provision of the highest quality behavioral health services to clients. Participates in the formulation and update of clinical and administrative policies and procedures and the preparation of policy and procedure manuals.
- Encourages clients and/or family members to participate in the services they have chosen to include in their treatment plan.
- Participates in VNS Health and community programs and education, as requested.
- Assists program management and any related staff in the prevention, identification, and resolution of client crises.
- Participates in special projects and performs other duties as assigned.
Qualifications
Licenses and Certifications:
License and current registration to practice as a Nurse Practitioner in New York State requiredBoard Certified Psychiatric-Mental Health Nurse Practitioner within one year in role requiredDEA license requiredValid driver's license may be required, as determined by operational/regional needs.

What You Will Do
Monitor and follow patient progress and report health changes to supervising registered professional nurse through detailed observations, interviews, and physical/psychosocial care within LPN scope of practice.
Deliver personalized nursing and care management by constantly evaluating the evolving patient needs and adjusting the plan of care.
Carry out quality-driven and care coordination by communicating within the VNS Health interdisciplinary team and across providers and other resources to deliver the plan of care effectively and efficiently.
Practice in the community, traveling to patients’ homes or care facilities, using effective time management and agility to manage weekly caseload and other duties.

Qualifications

Works as a member of an interdisciplinary care team at a NORC (Naturally Occurring Retirement Community) or Senior Living Retirement Community location and is responsible to coordinate a broad range of health and social services to help support older residents to age in their own homes. NORCs do so by facilitating and integrating the health and social services already available in the community while organizing additional services and supports necessary to help meet the goal of enabling older adults to remain in their community.
