Essential Duties and Responsibilities: By following the care plan that has been completed by the nursing supervisor, the home health aide provides assistance with the following:
- Maintain a healthy, safe environment. Providing nutritional and environmental support.
- Assist with personal hygiene, toileting, dressing, walking, and feeding the client.
- Position the client when instructed to do so.
- Assist with certain treatments and therapy as ordered by the physician and approved by the nursing supervisor.
- Remind the client to take medications.
- Assist with payment of bills only when authorized to do so and it is documented on the assignment sheet.
- Run errands when authorized to do so. Documentation on the aide charting sheet daily.
- Report any important information or changes in the client's condition to the agency and/or the nursing supervisor when necessary.
- Use of medical supplies and equipment when appropriate.
- Emotional support and the development of a trusting relationship between the aide and the client.
- Encouragement of client independence.
Qualifications/Education:
- Must be eighteen (18) years of age or older.
- Must have a certificate from a home health aide training program consistent with state requirements with evidence of ongoing in-service education to maintain the certificate.
- Must have the maturity necessary for establishing and maintaining a good work relationship with the client, the client's family, and agency personnel.
- Have a sincere interest in and a sympathetic attitude towards caring for the sick and disabled.
A home health aide must:
- Get along well with others.

Provides comfort measures (Proper positioning, back rubs, oral care, wrinkle free bed sheets and ensuring that bedside table, water pitcher, urinal are within reach.)
Provides activity needs of assigned patients (i.e. dangling, turning, chair, ambulating, walker, and crutches).
Performs measurements of assigned patients (I&O,TPR, B/P, weights, empty drains, abdominal girth, hose measurements).
Performs additional skills as directed and supervised by the nurse, after training and certification, i.e. intermittent urinary catheterization and clean dressing changes (varies by unit or area).
Assists with admission, transfer, and discharge of patient.
Completes assigned aspects of admission process including vital signs, height and weight, room setup, room orientation, assists patient to bed, assures call light available etc.
Sets up equipment for care (IV poles, admission kit, others)
Stores and transfers belongings (dentures, hearing aids, glasses and other personal items).
Packs up all patient belongings at discharge.
Achieves patient satisfaction when providing patient care.
Makes rounds minimally every two hours to assure that patients are comfortable, positioned properly, call light available, room orderly.

Compensation and Benefits
The compensation range for this position is $17.89per hour - $28.98per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model. Conducts comprehensive clinical reviews of adverse determinations related to medical necessity. Initiates outreach to providers to obtain clarification or additional documentation in alignment with established clinical criteria and organizational policies, to support Medical Director decision making. Provides support for claim appeals in relation to medical necessity. Ensures the timely and accurate resolution of appeal cases and supports organizational adherence to all state, federal, and accreditation standards. Facilitates member second level appeal process.
EDUCATION/EXPERIENCE
Graduate from an accredited school of professional nursing is required. BSN preferred. Minimum 2 years acute care experience or managed care experience is required. Basic knowledge of Medicaid, Medicare preferred. Knowledge of InterQual screening criteria, ICD-10, CPT coding preferred.
LICENSURE
Current Registered Nurse (RN) license to practice professional nursing issued by the Board of Nurse Examiners for the State of Texas is required. Active Certification in Case Management (CCM) is preferred.
